A two-month-old white girl presented to our facility with increasing lethargy and new onset apnea and bradycardia following a week of upper respiratory tract infection symptoms. The patient had been receiving a cough syrup containing promethazine hydrochloride during the previous five days, which was temporally correlated with the onset of lethargy and apneic episodes. Upon further investigation, it was discovered that the patient's older sibling also may have experienced phenothiazine-associated apnea after receiving a combination of meperidine hydrochloride, promethazine hydrochloride, and c…

Obstructive apnea, especially OSA, is a common problem in pediatric practice. In young infants, craniofacial anomalies may be the most common contributor to OSA. Complications include failure to thrive, metabolic alkalosis, and respiratory distress which can be life-threatening. In children over the age of 2 years, adenoidal and tonsillar hypertrophy is more commonly the cause of upper airway obstruction. The complications of growth failure, cor pulmonale, and adult respiratory distress syndrome were discussed. Obstructive sleep apnea can cause significant complications in both infants and chi…

During sleep, oxygen consumption and systemic blood pressure decrease in normal subjects; during rapid eye movement sleep, irregular ventilation can be accompanied by brief periods of apnea. In patients with obstructive sleep apnea, alveolar ventilation during an apneic episode is immediately reduced to zero, and the metabolic demands for oxygen must be met from oxygen stores within the body. As the stores of oxygen within the lung are diminished, the rate of arterial oxyhemoglobin desaturation increases. The development of alveolar hypoventilation during wakefulness seems to be based on a bal…

The results of the treatment of the obstructive sleep apnea syndrome with tracheostomy (68 patients), continuous positive airway pressure (67 patients) or uvulopalatopharyngoplasty (14 patients) have been presented. The best results were after tracheostomy or during continuous positive airway pressure application, worse results gave uvulopalatopharyngoplasty.

The symptoms of hypothyroidism are protean and include apathy, somnolence, lethargy, personality change, and intellectual deterioration. Many of these symptoms may be related to hypothyroid-induced sleep disorders. Hypothyroidism is associated with abnormal ventilatory drive, abnormal sleep architecture, and sleep apnea. Central, obstructive, and mixed patterns of sleep apnea are commonly observed in hypothyroidism. A case of severe sleep apnea in a grossly myxedematous patient who improved dramatically following thyroid replacement alone is presented. Myxedema is a reversible cause of sleep a…

The upper airway was evaluated in eight patients with obstructive sleep apnea by using a rapid sequential CT scanner (Imatron C-100). Four patients also had simultaneous polysomnograms to determine the onset of sleep and apnea. The upper airway was scanned while the patient was awake (eight patients), asleep (four patients), and asleep and apneic (eight patients). Measurements of the cross-sectional area of the upper airway were correlated with the findings on sleep studies in four patients. During the awake state the airway was narrowed and showed increased collapsibility in all eight patient…

Sleep-related closure of the upper airways is the key feature of the obstructive sleep apnoea syndrome (OSAS). Excessive daytime sleepiness and loud snoring are the major signs. It is not possible to differentiate patients with the single complaint of habitual snoring from patients with sleep apnoea by their history alone; polysomnography is a mandatory procedure. Recently, uvulopalatopharyngoplasty (UPPP) has been introduced as a surgical procedure to treat OSAS and excessive snoring. This paper presents the results of UPPP in eleven patients evaluated by means of postoperative polysomnograph

Guidelines for the medical therapy of obstructive sleep apnea are difficult to define precisely. While some elegant investigations have been completed, most study populations have been small. Also, the long-term effects of most forms of therapy are not known. Some patients will respond to a given form of therapy or combination of therapies while others will not. In most instances the responders cannot be recognized prior to the institution of therapy and a cycle of trial and error ensues. One of the best nonsurgical approaches appears to be weight loss, albeit unsuccessful in most cases. Almos…

Home monitoring for apnea patients has been called a stressful situation for family members. A 28-item questionnaire was used to ascertain the impact of home monitoring on the families of children with infantile apnea. Forty of 71 families returned the questionnaire. The majority of respondents felt the stress of home monitoring to be minimal (42.5%) to moderate (37.5%). All respondents favored home monitoring with 80% of parents reporting that the monitor had alerted them to at least one significant apnea or bradycardia episode during the first month of use. Half of parents reported more than…

Until recently, snoring had been considered both a medical enigma and a psychosocial problem. Snoring is now considered to be an acoustic phenomenon produced by vibration of the soft palate and the tonsillar pillars. We describe 20 patients with a clinical complaint of excessive snoring who were referred to rule out obstructive sleep apnea. All patients were without symptoms of daytime sleepiness and failure of the right heart. Twenty subjects were studied, 18 of whom were males. All subjects were monitored for one full night in the Sleep Laboratory. The apnea rate ranged from 9.0 to 94.0 inci…

Patterns of rib cage (RC) and abdomen (ABD) motion obtained from a respiratory inductive plethysmograph were studied during disordered breathing events - obstructive apnea, obstructive hypopnea, mixed apnea, and central apnea - in 54 patients with sleep apnea. The type of disordered breathing event was verified by esophageal pressure and bias-flow mask recordings. Obstructive apnea occurred to a variable extent in all patients and was characterized by stereotyped paradoxical motion of the RC or ABD or both in 49 patients (91%). There was no paradox in 2 patients with "feeble" inspiratory effor…

We studied a 14-year-old girl who suffered fractures of her mandible and tegmen following a fall from a balance beam. Thirteen days after hospitalization, she developed severe, protracted, recurrent episodes of hyperventilation; subsequently, she suffered posthyperventilation apnea, which at times was prolonged and association with severe hypoxemia with an arterial oxygen pressure as low as 25 mm Hg. The patient was treated with added dead space and chlorpromazine hydrochloride (Thorazine). Postulated mechanisms for her disorder are discussed. The importance of close clinical and laboratory ob…

Obstructive sleep apnea (OSA), obesity, and cancer constitute an interconnected pathophysiological triad withi mportant implications for morbidity and mortality. Obesity is a major upstream driver of both OSA and tumorigenesis, while OSA contributes independently through intermittent hypoxia, sleep fragmentation, systemici nflammation, oxidative stress, immune dysregulation, and sympathetic activation. Emerging evidence links OSA-associated hypoxemia to cancer initiation, progression, and treatment response, with hypoxic burden and nocturnal oxygen desaturation metrics outperforming the apnea-…

This article provides an overview of central sleep apnea (CSA) in the context of cardiovascular diseases (CVDs), highlighting its common association with conditions such as heart failure, atrial fibrillation, and cerebrovascular accidents. While the detailed pathophysiology is addressed elsewhere, the article notes that central apneas generally arise during sleep when arterial carbon dioxide levels drop below the apnea threshold, despite variability in underlying mechanisms. It also emphasizes that affected patients are typically normocapnic or hypocapnic rather than hypercapnic. The article f…
