An assessment of the fixin tplo jig to generate effective compression using a transverse fracture model.

The objective of this study was to determine compressive loads that could be generated using a tibial plateau leveling osteotomy (TPLO) jig with a tensioned strand of 18-gauge stainless steel orthopedic wire in a simulated transverse fracture model. The wire was sequentially tensioned using heavy needle holders or an AO wire tightener. Recorded loads were subsequently compared to loads generated by applying a 3.5 mm limited contact-dynamic compression plate (LC-DCP) as a compression plate. Two segments of 2 cm diameter Delrin rod were placed in a testing apparatus and used to simulate a transv

Antibiotic cement coating in orthopedic surgery: a systematic review of reported clinical techniques.

BACKGROUND: Antibiotic-containing cement and bone graft substitute-coated orthopedic implants provide the advantages of simultaneous local antibiotic delivery and internal stable fixation, aiding in both infection eradication and osseous healing. Standardized protocols pertaining to implant coating techniques in various clinical and particularly intraoperative settings are scarce, and available literature is limited. This systematic review aims to provide a summary of the available current literature reporting on custom-made coating techniques of orthopedic implants, indications, outcomes, and

Surgical management of collapsed pelvis in a jaguar.

An immature jaguar was surgically treated for severe constipation caused by a narrow pelvic canal. This narrowing was attributed to nutritional secondary hyperparathyroidism. The ventral floor of the pelvis was widened by placing a piece of high density polyethylene between the 2 sides of the pubis after symphysiotomy and stabilizing the implant with orthopedic wires. This procedure provided stable fixation and permanent enlargement of the pelvic canal. The technique was easy to perform and could be used to treat pelvic collapse in other species.

Fractures of the carpus, excluding the scaphoid.

Carpal bone fractures comprise an important group of injuries that present difficulties in both diagnosis and treatment. These fractures may be missed for the following reasons: physician's low index of suspicion, the carpal bones' irregular contours and degree of overlap on radiographs making the fractures difficult to visualize, and the common occurrence of concomitant fractures of neighboring metacarpals, distal radius, or scaphoid, which misdirects the examiner. A high index of suspicion with careful clinical examination and elicitation of point tenderness is key to making the correct diag

Closed Limb Fractures With Compromised Vascularization: A Narrative Review.

Vascular compromised fractures typically result in a high rate of healing complications, such as avascular necrosis, nonunion, delayed union, and arthritis, which severely affect a patient's function and quality of life. The purpose of this review was to identify and describe the epidemiology and available treatment options for the most well-known vascular compromised closed fractures. The injuries discussed in detail in this review were scaphoid, lunate, femoral neck, and talar fractures. Current evidence suggests that optimal treatment for vascular compromised fractures is dependent on the d

[Fatal pelvic vein injury caused by guidewire used with the dynamic hip screw].

There are only a few reports about injury to vital anatomical structures by Kirschner wires in literature. In the case described here the guide wire of the dynamic hip screw caused a fatal injury to the external iliac vein which was not at first evident. The guide wire had obviously become caught up in the combination three-diameter drill and screwed by the drill into the pelvis. After a stable interval the patient suffered circulatory collapse. Emergency treatment and resuscitation attempts followed with no possibility of surgical intervention. The nature of the injury was established and des

Indirect open reduction through intercartilaginous incision and intranasal Kirschner wire splinting of comminuted nasal fractures.

The majority of nasal fractures have been managed by using closed reduction and intranasal packing. In comminuted nasal fractures, open reduction and internal fixation may be indicated for accurate reduction and rigid fixation, but it is a very aggressive procedure. We developed a new technique for comminuted nasal fractures: indirect open reduction and intranasal Kirschner wire splinting. A periosteal elevator is used to elevate the mucoperiosteum posterior to the nasal bone through intercartilaginous incision and to reduce accurately the nasal bone, at the same time detecting the fracture li

Finite element analysis and case series study of anchor loop plate fixation of patellar inferior pole sleeve avulsion fracture.

The purpose of this study was to compare the biomechanical stability of the Kirschner wire tension band combined with patellar cerclage and anchor loop plate ( ALP ) in the treatment of inferior patellar fracture with different fracture lines and to provide a new solution for the treatment of sleeve avulsion fracture of the inferior pole of the patella. The finite element model was established, and the lower pole fractures of different fracture block sizes were graded according to the distribution of the lower pole fracture line. The model was subjected to finite element mechanical test under

Excessive Heat Generation by Power-Driven Craniotomy Tools: A Possible Cause of Autologous Bone Flap Resorption Observed in an Ex Vivo Simulation.

BACKGROUND: Bone flap resorption is an issue after autologous cranioplasty. Critical temperatures above 50°C generated by power-driven craniotomy tools may lead to thermal osteonecrosis, a possible factor in resorption. This ex vivo study examined whether the tools produced excessive heat resulting in bone flap resorption. METHODS: Using swine scapulae maintained at body temperature, burr holes, straight and curved cuts, and wire-pass holes were made with power-driven craniotomy tools. Drilling was at the conventional feed rate (FR) plus irrigation (FR-I+), at a high FR plus irrigation (hFR-I

A Novel Method to Fix Paediatric Mandibular Fracture using 2-0 Polyglactin Suture: A Technical Note.

BACKGROUND: Reduction and stabilisation of fractures at the earliest is valuable in the paediatric age groups. This novel technique focuses on stabilisation of the fracture fragments using vicryl, which has an added advantage to conventional plating and wiring. This study aimed on a novel cost-effective technique of stabilising the paediatric mandibular fracture using 2-0 resorbable polyglactin 910 suture with minimal trauma to tooth buds and bone and adequate stability. TECHNIQUE: After raising a full-thickness mucoperiosteal flap, the fracture segments are identified and reduced. 1.6mm dril

Autologous Bone Graft Harvest Using the Reamer-Irrigator-Aspirator.

Initially developed to reduce intramedullary pressures during long bone intramedullary nail placement, the reamer-irrigator-aspirator (RIA) can be used to harvest large volume autologous bone graft from the intramedullary canal of a long bone. This case-based surgical technique video demonstrates the appropriate techniques that should be applied while using the RIA to obtain autologous bone graft from the femur. A large volume of bone graft, generally between 30 and 90 cm, can be obtained using a single RIA procedure. The bone graft harvested from the intramedullary canal is excellent regardin

Biomechanical analysis of initial incisor crowding alignment in the periodontally reduced mandible using the finite element method.

AIMS: To reduce remaining plaque niches due to dental malocclusion after periodontal treatment and to avoid reinflammation of periodontitis, severe anterior crowding can be treated orthodontically. The treatment indication is motivated by aesthetic and functional needs. In this study the biomechanical behaviour of crowded lower front teeth in reduced periodontium is analysed. METHODS: Using the finite element (FE) method, a model of the mandible was constructed with an anterior crowding of 4 mm and a vertical bone loss of 4 mm in the front tooth area. A 0.3 mm (0.012″) round superelastic nick

[Treatment of open fractures in children (author's transl)].

Open fractures in children with soft tissue injuries of second and third degree generally require osteosynthesis. Minimal osteosynthesis is sufficient. Articular fractures and juxtaarticular fractures are stabilized with Kirschner wires. In shaft fractures of long bones short SAO plates are used in most cases. For shaft fractures of the upper extremity alternatively intramedullary osteosynthesis with Kirschner wires or Rush pins may be applied. Indication, osteosynthesis, and operating technique are discussed.

[Long-term results after stapes surgery for otosclerosis].

In a retrospective study, hearing results before and after stapedectomy were compared with those of individuals not undergoing surgery. Subjects were 51 patients who were surgically diagnosed as having otosclerosis and who could be followed up for more than 5 years. Of these, 31 contralateral ears surgically diagnosed as otosclerotic did not undergo surgery (nonsurgery group). Nineteen ears underwent stapedectomy using a wire loop prosthesis (TS group). Thirty-eight ears underwent stapedotomy using a wire piston prosthesis (SFS group). In the nonsurgery ear group, the last air conduction thres

Axillary view of the glenoid articular surface.

The glenoid articular surface is best studied radiographically on a tangential projection with both true anteroposterior and axillary views. Forty-one dry scapulas were studied under fluoroscopy to define the axillary projection that would provide a true scapular lateral view. The superior and inferior articular margins were marked with radiopaque solder wires. A true axillary view was obtained when the projection of the wires superimposed. The projection of the cortical bone of the posterolateral surface of the coracoid was noted to be continuous with the projection of the subchondral cortica

Intraoperative measurement of bone electrical potential: a piece in the puzzle of understanding fracture healing.

INTRODUCTION: Bone electrical potentials change with the force applied. Also, fracture alters the bone electrical potential, so it becomes more electronegative. These potentials have an important role in fracture healing, bone growth and remodelling. Literature data on the influence of fracture operative treatment on bone electrical potentials, and possible consequences of this influence, are sparse. The objective of this study was to establish a method of intraoperative bone potential measurement, and to try to find a correlation between electrical potential and fracture type, osteosynthesis
