Metastatic Pheochromocytoma in an Asymptomatic 12-Year-Old With von Hippel-Lindau Disease.

Pheochromocytoma is a rare chromaffin cell tumor that may be associated with a genetic predisposition, such as von Hippel-Lindau (VHL) disease. VHL is an autosomal dominant disorder that is characterized by a predisposition to multiple tumors, including retinal and central nervous system hemangioblastomas, renal cell carcinoma, and pheochromocytomas. The classic presentation of pheochromocytoma is episodic hypertension, headaches, palpitations, and diaphoresis. In the pediatric population, 40% of pheochromocytomas have a hereditary basis. We present a case of metastatic pheochromocytoma in a c

Metastatic pheochromocytoma complicated with Langerhans cell histiocytosis: a case report.

Pheochromocytoma is a neuroendocrine neoplasm that originates from chromaffin cells of the adrenal medulla. Langerhans cell histiocytosis (LCH) is a proliferative disease of histiocyte-like cells, often associated with activating mutations of the mitogen-activated protein kinase (MAPK) pathway. We present a case of a 49-year-old male with a history of pheochromocytoma, which metastasized to the inferior vena cava eight years after left adrenalectomy. At the same time, it was found that the pheochromocytoma in the metastasis was complicated with LCH, a combination that has not been previously r…

[Pheochromocytoma with thrombus in cava].

Pheochromocytoma, a paraganglioma of suprarenal location, is a catecholamine-secreting chromaffin cell tumour. Spread of these tumours to the vena cava is rare and the thrombus only reaches the right atrium in exceptional cases. We present the case of a patient who, without previous symptomatology, presented with a clinical picture of multiorganic dysfunction with primary manifestation of a suprarenal tumour with vascular spread to the right atrium affecting the right suprahepatic vein.

Adrenal gland response to endocrine disrupting chemicals in fishes, amphibians and reptiles: A comparative overview.

The adrenal gland is an essential component of the body stress response; it is formed by two portions: a steroidogenic and a chromaffin tissue. Despite the anatomy of adrenal gland is different among classes of vertebrates, the hormones produced are almost the same. During stress, these hormones contribute to body homeostasis and maintenance of ion balance. The adrenal gland is very sensitive to toxic compounds, many of which behave like endocrine-disruptor chemicals (EDCs). They contribute to alter the endocrine system in wildlife and humans and are considered as possible responsible of the d

[Morphological and histochemical aspects of the argentaffin and argentophilic cells in the gastric mucosa of Scyliorhinus stellaris and Scyliorhinus canicula].

Using histological and histochemical methods for the identification of biogenic amins, phenolic and indolic groups, and of substances with a proteine and carbohydrate nature, it has been possible to identify at least five types of endocrine cells in the gastric mucose of Scyliorhinus stellaris and Scyliorhinus canicula. One of these, a type I cell, localised mainly in the piloric portion, shows morphological and histochemical characteristics which are perfectly comparable with those of the enterochromaffin type found in the gastro-intestinal tract of mammals. On the other hand, the four remain

Role of adrenomedullin and related peptides in the regulation of the hypothalamo-pituitary-adrenal axis.

Adrenomedullin (ADM) is a hypotensive peptide, originally isolated from human pheochromocytomas, and then found to be widely distributed in … osterone production by in situ perfused rat adrenals and human adrenal slices (containing medullary chromaffin cells), again through the activation of CGRP1 receptors. This aldosterone secretagogue effect of ADM is blocked by the beta-adrenoceptor antagonist l-alprenolol, thereby suggesting that it is indirectly mediated by the release of catecholamines by chromaf …

Case Report: ACTH- and CRH-secreting pheochromocytoma as a very rare cause of ectopic Cushing syndrome.

Ectopic adrenocorticotropic hormone (ACTH) syndrome (EAS) represents a rare clinical entity characterized by severe hypercortisolemia. Pheochromocytoma, a neuroendocrine neoplasm originating from chromaffin cells within the adrenal medulla, is itself an uncommon diagnosis. The phenomenon of ectopic hormone production by pheochromocytomas is exceedingly rare, occurring in less than 1% of cases. In most instances, hypercortisolemia associated with pheochromocytoma is attributed to aberrant ACTH secretion. This report presents an exceptional case of EAS caused by a pheochromocytoma exhibiting co-…

[Malignant paraganglioma with vertebral and skull metastasis].

A paraganglioma is a rare tumor composed of chromaffin cells. Malignant paraganglioma is a very rare presentation diagnosed by local recurrence after total resection of the primary mass or findings of distant metastasis. We report a case of a 30-year-old man who had undergone a carotid body tumor resection 10 years before and who presented to us with recurrence of the tumor associated with lymph node, vertebra and skull metastasis. The diagnosis of malignant paraganglioma was based on loco-regional and distant metastasis.

Human growth hormone co-transfection assay to study molecular mechanisms of neurosecretion in PC12 cells.

The recent introduction of transient co-transfection assays, with human growth hormone (hGH) as a marker of transfection, has allowed us to study the function of transfected proteins in neurosecretion using intact PC12 cells and adrenal chromaffin cells [J. Biol Chem. 268 (1993) 10983]. Since this assay requires several non-trivial steps (transfection of the genes, triggering of secretion, and measurement of hGH), care should be taken before attributing any observed effect to the transfected proteins. In this article, we describe a detailed procedure for the hGH transfection assay and show tha…

GIPC2 is an endocrine-specific tumor suppressor gene for both sporadic and hereditary tumors of RET- and SDHB-, but not VHL-associated clusters of pheochromocytoma/paraganglioma.

Pheochromocytoma/paraganglioma (PPGL) is an endocrine tumor of the chromaffin cells in the adrenal medulla or the paraganglia. Currently, about 70% of PPGLs can be explained by germline or somatic mutations in several broadly expressed susceptibility genes including RET, VHL, and SDHB, while for the remaining, mainly sporadic cases, the pathogenesis is still unclear. Even for known susceptible genes, how mutations in these mostly ubiquitous genes result in tissue-specific pathogenesis remains unanswered, and why RET-mutated tumors almost always occur in the adrenal while SDHB-mutated tumors mo…

"The Calm between the Storms:" Perioperative Management of a Patient with Pheochromocytoma in Catecholamine Crisis.

Pheochromocytoma (PCC) is a catecholamine-secreting tumor that originates from chromaffin cells in the adrenal medulla. Anesthetic management of PCC can be challenging due to hemodynamic fluctuations. Thorough preoperative evaluation and optimization, meticulous intraoperative management and good communication between surgeon and anesthetist are crucial for patient safety. This is a case report of successful perioperative management of patient with PCC posted for laparoscopic adrenalectomy; where the patient developed catecholamine crisis in the immfigediate preoperative period, making the per…

Infection and pheochromocytoma.

A urinary tract infection with possible septicemia and endocarditis developed in a 36-year-old man. The illness was complicated by pulmonary embolism, thrombocytopenia, hematemesis, hepatic dysfunction, paralytic ileus and accelerated hypertension. The latter finding suggested pheochromocytoma. Treatment with antibiotics and phenoxybenzamine hydrochloride was associated with notable clinical improvement. A chromaffin cell tumor was surgically removed above the lift kidney. Conclusively, a pheochromocytoma may mimic and be present in association with infection.

Retroperitoneal pheochromocytoma: Unsual presentation and atypical location.

INTRODUCTION AND IMPORTANCE: Pheochromocytomas are rare tumors (0.1-2% of incidence), arising from the chromaffin cells in the sympathoadrenal system. Approximately 85% of the times are localized in the adrenal medulla; therefore, could be placed extra adrenal in 15% of the population. 10-30% of the cases could be asymptomatic. Classic symptoms vary from palpitations, tachycardia, hypertension. CASE PRESENTATION: Case report of a 37-year-old female patient presented with diffuse abdominal pain, with any associated symptoms. Contrast computed tomography was performed; a retroperitoneal mass wa…

Adrenal medulla development and medullary-cortical interactions.

The mammalian adrenal gland is composed of two distinct tissue types in a bidirectional connection, the catecholamine-producing medulla derived from the neural crest and the mesoderm-derived cortex producing steroids. The medulla mainly consists of chromaffin cells derived from multipotent nerve-associated descendants of Schwann cell precursors. Already during adrenal organogenesis, close interactions between cortex and medulla are necessary for proper differentiation and morphogenesis of the gland. Moreover, communication between the cortex and the medulla ensures a regular function of the ad…

Hereditary Paraganglioma in an Omani Family.

Pheochromocytomas are tumors derived from chromaffin cells within the medulla of adrenal glands, whereas paragangliomas are tumors derived from extra-adrenal chromaffin cells of the sympathetic prevertebral and paravertebral ganglia. The growing deployment of genetic testing has shown that approximately 30% of pheochromocytoma and paraganglioma (PPGL) patients carry familial pathogenic germline mutations in known PPGL-susceptibility genes. This prompts genetic screening of their family members and leads to an increase in the detection of asymptomatic PPGLs or non-secreting tumors reported in f…

Cholinergic differentiation factor/leukemia inhibitory factor enhances functional effects of adrenal medulla grafts after hippocampal lesions in rats.

We recently found that adrenal medulla grafts implanted into the hippocampus of rats survived for several months and significantly decreased the deficits produced by hippocampal lesions in the radial maze test [Jousselin-Hossaja et al. (1994) Neuroscience 59, 275-284]. These grafts contained choline acetyltransferase immunopositive chromaffin cells and received cholinergic innervation. In the experiments reported here, adrenal medulla grafts implanted in lesioned hippocampus were treated with cholinergic differentiation factor/leukemia inhibitory factor. In the presence of this factor, the num…

A case of recurrence and metastasis of pheochromocytoma: a case report.

Pheochromocytomas are rare tumors that develop from chromaffin cells of the adrenal medulla or from extra-adrenal paraganglia . These tumors frequently produce catecholamines, and excessive secretion can lead to a variety of symptoms, including episodic hypertension and metabolic disorders. Such complications can significantly impact cardiovascular health as well as kidney and vascular function, and there is notable concern regarding recurrence and metastasis following surgical resection. This article analyzes the case characteristics, diagnosis, and treatment of a patient with pheochromocytom…

Cardiogenic Shock With Reverse Takotsubo Pattern Secondary to Pheochromocytoma: A Case Report.

Pheochromocytoma is a rare catecholamine-secreting neuroendocrine tumor arising from chromaffin cells. Acute catecholamine-mediated cardiomyopathy secondary to pheochromocytoma is rare, but life-threatening. We report a case of a 50-year-old man who presented with chest pain with electrocardiography showing ST elevation in V2-4. He was transferred to cardiac catheterization laboratory for coronary angiography immediately. However, the results showed no evidence of coronary artery occlusions and the left ventriculography revealed hypokinesia of basal part with poor left ventricular ejection fra…
