Extending the Use of Disposable Caging Based on Results of Microbiologic Surface Testing.

Prions are proteinaceous infectious agents that are highly resistant to denaturation. Sterilization of prion-contaminated mouse cages requires chemical agents and increased autoclave temperatures that damage traditional cages, thus increasing facility costs. Disposable cages are a possible alternative that might decrease replacement costs without compromising the environment of the mice. We compared our standard protocol of changing traditional cages and bedding once every 2 wk to an experimental protocol using disposable cages in which only the bedding was changed once every 2 wk over an 8-wk

Reducing levels of medical device contamination through package redesign and opening technique.

OBJECTIVES: The goal of this research was to evaluate how material curl, package structure and handling of pouches containing medical devices affect rates of contact between non-sterile surfaces and sterile devices during aseptic transfer. METHODS: One hundred and thirty-six individuals with practical experience in aseptic technique were recruited. Participants were asked to present the contents of four different pouch designs (a standard, one designed to curl in, another to curl out and one that incorporated a tab) using two transfer techniques. During the first block of trials "standard tec

Persistent residual contamination in endoscope channels; a fluorescence epimicroscopy study.

BACKGROUND AND STUDY AIMS: The increasing demand for endoscopic procedures poses new contamination challenges, given developing antimicrobial resistance worldwide and potential viral or prion diseases in populations at risk. We examined working channels from reusable luminal endoscopes used in recent years. METHODS: Very sensitive fluorescence epimicroscopy was used to examine working channels from 6 decommissioned and 6 factory-new channels, as received, or following spiking and washing in the laboratory. RESULTS: After a single contamination and wash test cycle, new channels retained appro

[Endoscopes in urology: disinfection, sterilization, labeling and tracking. Circulars and decrees. Modes of application and commentary. The Committee of Infectious Diseases of the French Association of Urology. Congressional forum UFA--Paris, November 1996. DGS Circular 20 October 1997].

Administrative texts published in 1995, 1996 and 1997, have reinforced materiovigilance and impose disinfection precautions for endoscopes. The steps of disinfection of non-sterilizable endoscopes are: preliminary treatment, rinsing, actual disinfection, final rinsing, storage (see: Progrès en Urologie, 1997, 7, 505-507). Each procedure from collection of the endoscope until storage must be defined by written standard operating procedures validated by CLIN. The risk of transmission of Creutzfeld-Jakob disease requires autoclaving, which is only possible, at the present time, with the most rece

Widespread Legionella pneumophila contamination of dental stations in a dental school without apparent human infection.

Following isolation of Legionella pneumophila from a special dental station water circuit, used primarily to cool high-speed dental drills which produce fine aerosols, a case finding and environmental survey was undertaken. Widespread colonization of the dental stations was found and the results suggested that amplification of the background levels of L. pneumophila was taking place within the stations. However there was no evidence for transmission causing human infection.

Bacterial Contamination of Veterinary Surgical Microscopes in Referral Ophthalmology Practice and a Survey of Cleaning Protocols-A Pilot Study.

OBJECTIVE: To investigate cleaning protocols and the presence of bacteria on surgical microscopes in multidisciplinary referral centers. METHODS: Bacteriology samples from one surgical microscope were taken at each center (n = 2), from different areas (eyepieces/handles/objective lens) and at different time points (prior to and immediately after cataract surgery) and submitted for routine culture and antimicrobial susceptibility testing. A survey on surgical microscope cleaning protocols was distributed to referral centers in the UK. RESULTS: Seven isolates (three different bacterial species

Iatrogenic infection associated with positive airway pressure therapy: A review of precedent, epidemiology, bioaerosols and risk mitigation.

Currently, evidence-based guidelines about cleaning positive airway pressure devices for maintenance or reprocessing for a new user do not exist. There is no strong evidence of harm caused by contaminated positive airway pressure equipment. Future research opportunities exist to streamline cleaning processes, assure hygiene, and reduce waste.

Optimizing the Glass Bead Sterilization Protocol Focusing on Removal of Organic and Bacterial Intraoperative Contamination.

Validated glass bead sterilization protocols to effectively sterilize rodent surgical instruments after bacterial exposure (for example, cecal contamination) are lacking. To refine current approaches, we added either a multienzyme detergent, neutral pH detergent, or chlorhexidine scrub step before glass bead sterilization of forceps or needle drivers exposed to cecal contents. We exposed sets of forceps and needle drivers to cecal contents, which were then air dried for 3 min. Immediately after, the instruments were wiped several times with a clean, dry paper towel. The contaminated tips were

Risk of disease transmission to patients from "contaminated" surgical instruments and immediate use steam sterilization.

BACKGROUND: There are several sources of pathogens that cause surgical site infections (SSI) to include the patients endogenous microflora and exogenous sources (e.g., air, surfaces, staff, surgical equipment). METHODS: We searched the published English literature (Google, Google Scholar, PubMed) for articles on reprocessing surgical instruments, effectiveness of sterilization methods, microbial load on surgical instruments, frequency of "contaminated" instruments, and the infection risk associated with "contaminated" surgical instruments and immediate use steam sterilization. RESULTS: There

Cost utility analysis of strategies for minimizing risk of duodenoscope-related infections.

BACKGROUND AND AIMS: Transmission of multidrug-resistant organisms by duodenoscopes during ERCP is problematical. The U.S. Food and Drug Administration recently recommended transitioning away from reusable fixed-endcap duodenoscopes to those with innovative device designs that make reprocessing easier, more effective, or unnecessary. Partially disposable (PD) duodenoscopes with disposable endcaps and fully disposable (FD) duodenoscopes are now available. We assessed the relative cost of approaches to minimizing infection transmission, taking into account duodenoscope-transmitted infection cost

Assessment of laparoscopic instrument reprocessing in rural India: a mixed methods study.

BACKGROUND: Laparoscopy is a minimally-invasive surgical procedure that uses long slender instruments that require much smaller incisions than conventional surgery. This leads to faster recovery times, fewer post-surgical wound infections and shorter hospital stays. For these reasons, laparoscopy could be particularly advantageous to patients in low to middle income countries (LMICs). Unfortunately, sterile processing departments in LMIC hospitals are faced with limited access to equipment and trained staff which poses an obstacle to safe surgical care. The reprocessing of laparoscopic devices

A cold water, ultrasonically activated stream efficiently removes proteins and prion-associated amyloid from surgical stainless steel.

BACKGROUND: Sterile service department decontamination procedures for surgical instruments struggle to demonstrate efficient removal of the hardiest infectious contaminants, such as prion proteins. A recently designed novel system, which uses a low pressure ultrasonically activated, cold water stream, has previously demonstrated efficient hard surface cleaning of several biological contaminants. AIM: To test the efficacy of an ultrasonically activated stream for the removal of tissue proteins, including prion-associated amyloid, from surgical stainless steel surfaces. METHODS: Test surfaces

Efficacy of air/water syringe tip sterilization.

BACKGROUND: Dental procedures involve contact between instruments and the patient's tissues, blood or saliva. This study evaluated the efficacy of the standardized sterilization of non-disposable air/water syringe tips and corrosion and contaminant build-up in these tips. METHODS: The bacterial contamination of single-use and multiple-use non-disposable air/water syringe tips after routine use and sterilization was compared to that of single-use disposable tips by microbial culturing on PCA and blood agar plates. The effect of flushing the syringe tips prior to sterilization was also measured

Preparation of modern anesthesia workstations for malignant hyperthermia-susceptible patients: a review of past and present practice.

Patients with malignant hyperthermia experience an exaggerated metabolic response when exposed to volatile anesthetic gases and succinylcholine. The minimum concentration of anesthetic gas needed to trigger a malignant hyperthermia crisis in humans is unknown and may remain so because of the inherent risks associated with studying the complex nature of this rare and lethal genetic disorder. The Malignant Hyperthermia Association of the United States provides specific instructions on purging anesthesia machines of volatile agents to reduce the risk of exposure. However, these recommendations we

A comparative study of sterility levels in continuous ambulatory peritoneal dialysis system.

INTRODUCTION: Peritonitis remains a serious risk associated with continuous ambulatory peritoneal dialysis (CAPD), although better patient education programs and such technological advances as improved automated connecting devices have greatly decreased its incidence over the past 20 years. The automated devices have a good resistance to contamination, but they rely on an external electrical power source and are not easily portable. There has, therefore, been a need for a highly sterile nonelectric manual connecting device to complement the automated devices already in use. Such a manual devic
