A novel F11 mutation in a Korean pediatric patient with recurrent epistaxis.

Congenital FXI deficiency (hemophilia C) is a rare bleeding disorder that has been documented mostly in Ashkenazi Jews. Unlike other hemophilias, bleeding tendency varies considerably among individuals, and FXI deficiency rarely manifests as spontaneous bleeding. FXI deficiency is caused primarily by mutations in the F11 gene. Herein, we report a case of a 10-year-old boy with recurrent nose bleeding due to FXI deficiency who was confirmed to have a novel mutation in F11. A molecular analysis of DNA extracted from peripheral blood collected from the patient [FXI clotting activity (FXI:C): 11%]

Age, Dental State and General Health as Risk Factors for Xerostomia and Hyposalivation.

INTRODUCTION: Xerostomia and hyposalivation are common among adults and may significantly influence both oral health and overall quality of life. OBJECTIVE: The aim of this study was to analyze patient-related risk factors of xerostomia and hyposalivation in a clinical cohort. MATERIALS AND METHODS: The clinical evaluation included measurements of unstimulated and stimulated salivary flow rates. Furthermore, the xerostomia inventory (XI) was employed, and an additional general question on xerostomia was included. For analyzing associations, nonparametric tests, chi-square tests, correlation

Is there a 'Basque' profile regarding autosomal recessive deficiencies of coagulation factors?

When excluding haemophilia and von Willebrand disease, coagulation factors deficiencies constitute rare autosomal recessive disorders (<1 in 500,000) of less precisely defined epidemiology. We have reported herein the distribution of these entities in the French Basque Country, a genetic isolate of very old individualization with peculiar biological specificities. The prevalence of these disorders was markedly high, especially, as already shown, factor XI deficiency. This unusual profile needs to be discussed in the view of population genetics.

Clinical Spectrum and Treatment Outcomes of Rare Bleeding Disorders in Female Patients: A Two-Center Experience in North Pakistan.

Introduction Rare bleeding disorders (RBDs) result from genetic mutations in clotting factors. These RBDs vary in prevalence and are often u … ation between factors X and XI with the ISTH BAT score. There was no correlation found in factor I, factor XI, factor XII, and combined factors V+VIII deficiency. Conclusion Women with RBDs face a spectrum of bleeding challenges, significantly impacting their quality of life and reproductive health. Early diagnosis and personalized treatment strategies are …

[Hereditary disorders of hemostasis in obstetrics 2/2-Anesthesiological aspects of secondary hemostasis].

Hemophilia A and B as well as von Willebrand diseases account for 95-97% of all congenital blood coagulation disorders and the rare factor deficiency conditions (factors I, II, V, VII, X, XI, XIII) account for the remaining 3-5%. The clinical presentation varies considerably, whereby the correlation between factor activity and bleeding phenotype is only insufficiently expressed, especially for factor VII deficiency. Pregnancy represents a special challenge as the physiological alterations of hemostasis can compensate for the underlying defect to different extents. These heterogeneous dynamics

[Isolated prolongation of the PTT: 2-year retrospective study].

The hemorrhagic risk associated with isolated prolongation of the PTT has been evaluated in a 2-year retrospective study. Of the 60 cases thus found, a hemorrhagic risk was present in 15 patients of whom 7 had hemophilia A, 5 von Willebrand's disease and 3 factor XI deficiency. Among the other etiologies not associated with a bleeding tendency, there were 31 proved or suspected cases with inhibitors of the PTT, most of whom were children, 2 factor XII deficiencies, 2 prekallikrein deficiencies and 4 contact phase activations. Isolated prolongation of the PTT is therefore without specificity an…

Milvexian: A Focus on a New Oral Anticoagulant that Targets Factor XIa for Thromboembolism Prophylaxis.

Drugs that target factor XI and/or XIa have been evaluated as alternatives to existing anticoagulants, in light of studies that indicate that a decrease in Factor XI/XIa levels or activity may result in a lower risk of thrombosis without a significant increase in bleeding risk. Milvexian is an investigational small-molecule factor XIa inhibitor that has recently completed phase 2 clinical trials. Preclinical studies were suggestive of its potential to prevent arterial and venous thrombosis. It was well-tolerated in healthy participants, as well as in participants with mild or moderate hepatic …

Advances in Antithrombotic Therapy.

Thrombosis remains a major cause of morbidity and mortality. Consequently, advances in antithrombotic therapy are needed to reduce the disea … ng albeit not fatal bleeding. To overcome this problem, the second advance is the identification of factor XI as a target for new anticoagulants that are potentially safer than those currently available.

Rationale and design of the AXIOMATIC-SSP phase II trial: Antithrombotic treatment with factor XIa inhibition to Optimize Management of Acute Thromboembolic events for Secondary Stroke Prevention.

BACKGROUND: Individuals with ischemic stroke or transient ischemic attack (TIA) have a high early risk of ischemic stroke despite dual antiplatelet therapy. The risk of ischemic stroke, and associated disability, represents a significant unmet clinical need. Genetic variants resulting in reduced factor XI levels are associated with reduced risk for ischemic stroke but are not associated with increased intracranial bleeding. Milvexian is an oral small-molecule inhibitor of FXIa that binds activated factor XI with high affinity and selectivity and may reduce the risk of stroke when added to anti…

Perspectives and Future Directions of Anticoagulant Therapy in Coronary Artery Disease Patients.

Antiplatelet agents are routinely used to treat patients with chronic atherosclerotic coronary artery disease. Treatment with the addition of a low dose of rivaroxaban as dual-pathway inhibition (DPI) decreases ischaemic events at the expense of increased bleeding. At present, the balance between thrombotic and bleeding risks must be carefully weighed up when considering DPI. However, with the introduction of activated coagulation factor XI inhibitors, which have fewer bleeding effects, the use of DPI in patients with atherosclerotic cardiovascular diseases could be extended.

YY1 binding is a gene-intrinsic barrier to Xist-mediated gene silencing.

X chromosome inactivation (XCI) in mammals is mediated by Xist RNA which functions in cis to silence genes on a single X chromosome in XX female cells, thereby equalising levels of X-linked gene expression relative to XY males. XCI progresses over a period of several days, with some X-linked genes silencing faster than others. The chromosomal location of a gene is an important determinant of silencing rate, but uncharacterised gene-intrinsic features also mediate resistance or susceptibility to silencing. In this study, we examine mouse embryonic stem cell lines with an inducible Xist allele (

Management of venous thromboembolism in patients with active cancer.

Venous thromboembolism (VTE) is a major health burden in patients with cancer, causing morbidity, emergency room visits, hospitalizations, and death. Treatment is challenging, as it is necessary to balance the risk of recurrent thrombosis and bleeding associated with anticoagulants. Treatment paradigms are shifting from low-molecular-weight heparin monotherapy. Multiple recent randomized controlled trials have demonstrated the safety and efficacy of direct oral anticoagulants in this setting. Current studies are evaluating factor XI inhibitors as potential treatments for cancer-associated VTE.

Current therapy for rare factor deficiencies.

Haemophilia A and B and von Willebrand disease account for 80-85% of all inherited bleeding disorders. The other 15% are represented by deficiencies of fibrinogen, prothrombin, or factors V, VII, X, XI, or XIII. In addition, acquired factor deficiencies are seen in a variety of conditions ranging from malignancies to autoimmune disorders. The spectrum of symptoms in these conditions varies from severe and life-threatening haemorrhage to a mild bleeding diathesis. The diagnosis depends on demonstration of decreased activity of one of the clotting factors. Due to the rarity of each of the indivi

Does Advancing Technology Improve Outcomes? Comparison of the Da Vinci Standard/S/Si to the Xi Robotic Platforms During Robotic Nephroureterectomy.

INTRODUCTION: With the introduction of laparoscopy and now robotics, more patients are able to reap benefit from minimally invasive techniques during urologic surgery. With these advancing technologies, it is important to evaluate whether the outlay of hospital capital actually improves patient care. To date, there has been little literature regarding the impact of these advances on patient outcomes. In this article, we directly compare perioperative outcomes and hospital costs between the older da Vinci Standard/S/Si platform and the newer Xi robotic platform during nephroureterectomy. METHO

Factor XII deficiency acquired by orthotopic liver transplantation: case report and review of the literature.

Transmission of congenital clotting factor deficiencies after orthotopic liver transplantation is rare. There are published reports of liver donor-to-recipient transmission of protein C deficiency with dysfibrinogenemia, protein S, factor VII and factor XI deficiencies. We report a case of transmission of factor XII deficiency with liver transplantation in a patient with Budd-Chiari syndrome. There was a persistent elevation of the activated partial thromboplastin time (aPTT), but no evidence of bleeding while the patient was maintained on warfarin. The presence of a persistently abnormal aPTT…

The defective prothrombin consumption in Bernard-Soulier syndrome. Hypotheses from 1948 to 1982.

Bernard-Soulier syndrome (BSS) was described in 1948 as a constitutional platelet disorder characterized by giant sized platelets, a prolonged bleeding time, and a defect in prothrombin consumption. The accurate mechanisms of these abnormalities remain unexplained, especially the defect in prothrombin consumption on which we focus in this paper. Several hypotheses are proposed: firstly, a defective reaction between the platelet membrane, where the phospholipid composition is abnormal, and the proteins which initiate thromboplastin generation such as collagen, factor XI; secondly, an abnormal r…

"Familial Multiple Coagulation Factor Deficiencies of FXI and FXII in an Asymptomatic Saudi Woman".

Factor XI deficiency (FXI) is the third most common coagulation factor deficiency after hemophilia A and B, ie, in the hierarchy after factors VIII and IX, taking into account von Willebrand's factor deficiency, as bleeding disorders are higher than in hemophilia C. Factor XII deficiency (FXII) is a congenital condition, inherited in the vast majority of cases in an autosomal recessive manner, more often associated with thromboembolic complications. A combination of both factor deficiencies has been found very rarely, and it can be familial multiple coagulation factor deficiency (FMCFD). This …
