Stemless components lead to improved radiographic restoration of humeral head anatomy compared with short-stemmed components in total shoulder arthroplasty.

BACKGROUND: Restoring the native center of rotation (COR) in total shoulder arthroplasty (TSA) has been shown to improve postsurgical function, subjective outcomes, and implant longevity. The primary purpose of this study was to compare postoperative radiographic restoration of the humeral COR between short-stem and stemless humeral implants by evaluating the mean COR shift between the 2 techniques. Secondary outcomes evaluated were comparisons of COR shift outliers, humeral head implant thickness and diameter, direction of COR shift, and neck-shaft angle (NSA). METHODS: This study was a mult

Arthroscopic Reduction and Fixation of Impacted Humeral Head Reverse Hill-Sachs Fracture With Absorbable Anchors for Acute Traumatic Posterior Shoulder Dislocation.

A minimally invasive technique is presented for the treatment of acute traumatic posterior shoulder dislocation with a reverse Hill-Sachs lesion. Arthroscopic reduction was combined with internal fixation using 1 absorbable anchor. Under high-definition arthroscopic visualization, precise anatomical reduction of the impacted anteromedial humeral head compression fracture was achieved. Absorbable anchors were inserted into the subchondral bone defect beneath the reduced fragment. Initial stability was provided, and congruity of the articular surface is maintained. Compared with conventional app

Glenoid wear after humeral head replacement with using a single implant in patients with cuff tear arthropathy more than five years.

BACKGROUND: The aim of this study was to investigate glenoid wear in patients with humeral head replacement with a smaller humeral head, who were observed for >5 years, and to examine the relationship between radiographic changes and clinical outcome. METHODS: In this study, 41 shoulders of 41 patients (13 men, 28 women) were followed up for >5 years. The mean patient age at the time of surgery was 70.5 years. The average follow-up period at short-term was 26.0 months, and intermediate-term was 66.5 months. We classified glenoid wear into four grades (i.e., Grade 0 to Grade 3). Age, sex, clin

Complex humeral head fractures treated with blocked threaded wires: maintenance of the reduction and clinical results with two different fixation constructs.

BACKGROUND: Locking plates are the gold standard for treatment of 3-part humeral head fractures, although major complications range from 9% to 36%. Percutaneous techniques may allow vascular supply preservation, maintenance of fracture hematoma, scarce blood loss. Many configurations with Kirschner wires can be performed, generating confusion on result interpretation. We studied the correlation between system configuration, stability, and clinical results in patients with 3-part humeral head fractures treated with the same fixation system but with 2 different biomechanical constructs. An association between the inferior humeral head osteophyte and teres minor fatty infiltration: evidence for axillary nerve entrapment in glenohumeral osteoarthritis.

BACKGROUND: Glenohumeral osteoarthritis often results in inferior humeral osteophytes. Anatomic studies suggest that the axillary neurovascular bundle is in close proximity to the glenohumeral capsule. We therefore hypothesize that an inferior humeral osteophyte of sufficient magnitude could encroach on the axillary nerve and result in measurable fatty infiltration of the teres minor muscle. MATERIALS AND METHODS: Preoperative magnetic resonance imaging studies of 91 consecutive arthritic shoulders were retrospectively reviewed. Two cohorts were established based on the presence of a humeral

Osteonecrosis of the humeral head under corticosteroid therapy for non-medical reason in a 14-year-old female patient: A case report.

Osteonecrosis of the humeral head is a debilitating condition caused by disruption of blood supply to the bone, leading to bone death and potential collapse. Corticosteroid therapy, widely used for its anti-inflammatory effects, is a major risk factor for non-traumatic osteonecrosis, particularly in the shoulder. We present the case of a 14-year-old overweight girl (83 kg, 171 cm, BMI 28.38 kg/m²) admitted for evaluation of a suspected tumor in the upper end of the left humerus, with a history of prolonged corticosteroid use not for medical reasons but aimed at weight gain. Histopathological a

Technical note: reduction of a posteriorly dislocated humeral head via the anterior deltopectoral approach in a posterior fracture dislocation of the proximal humerus.

We aim to describe an approach for reducing the posteriorly dislocated humeral head through the rotator interval via a deltopectoral approach that is frequently utilized for internal fixation of proximal humerus fractures and fracture dislocations. The sheath of the long head of biceps (LHB) and the rotator interval capsule are opened till the glenoid; this enables access to the glenohumeral joint via the rotator interval. A long-handle Cobb elevator is introduced through the rotator interval and, under intraoperative imaging, advanced posteromedially to the dislocated humeral head. After ensu

Ethical Challenges in Managing a Displaced Neer III Humeral Head Fracture in an Undocumented Immigrant With Early Alzheimer's Disease: A Case Report.

This report describes an 82-year-old undocumented Peruvian female immigrant presenting to the ED with right elbow pain and limited shoulder mobility, diagnosed with a displaced Neer III right humeral head fracture sustained two days prior. She had not clearly communicated shoulder pain to her family for two days post-injury, possibly due to early Alzheimer's disease, which may have impaired her ability to recognize or articulate the severity of her injury, illustrating the clinical-ethical intersection. Radiographic imaging confirmed the fracture with medial displacement, necessitating surgica

The Interrater and Intrarater Reliability of the Humeral Head Ossification System and the Proximal Femur Maturity Index Assessments for Patients with Adolescent Idiopathic Scoliosis.

BACKGROUND: Skeletal maturity can evaluate the growth and development potential of children and provide a guide for the management of adolescent idiopathic scoliosis (AIS). Recent studies have demonstrated the advantages of the Humeral Head Ossification System (HHOS) and the Proximal Femur Maturity Index (PFMI), based on standard scoliosis films, in the management of AIS patients. We further assessed the HHOS and the PFMI method's reliability in the interrater and intrarater. METHODS: The data from 38 patients, including the humeral head and proximal femur on standard scoliosis films, were di

The relevance of neutral arm positioning for true ap-view X-ray to provide true projection of the humeral head shaft angle.

INTRODUCTION: Textbooks commonly recommend using the true anterior-posterior (ap)-view with the patient's arm in a sling and therefore in internal rotation (IR) for radiologic diagnostic assessment of the proximal humerus after trauma. However, IR or external rotation (ER) may affect the projection of the head shaft angle (HSA) and therefore bias the diagnostic conclusion significantly. We hypothesized that neutral rotation (NR) of the arm is mandatory for true ap-view to provide true projection of the HSA. MATERIALS AND METHODS: A simplified geometrical model of the proximal humerus was used

How does a varus deformity of the humeral head affect elevation forces and shoulder function? A biomechanical study with human shoulder specimens.

OBJECTIVES: A biomechanical study was performed to test the hypothesis that a varus deformity of the humeral head decreases supraspinatus (SSP) efficiency and increases deltoid elevation forces in human specimens. METHODS: Twenty-four fresh-frozen human shoulder specimens were prepared by preserving the rotator cuff and deltoid. A defined, medial closed-wedge osteotomy was performed and lateral locked plate applied to simulate a varus deformity of 45° in Group I (n = 8) and 20° in Group II (n = 8). The control group (n = 8) was not osteotomized. The effect of the deformities on arm elevation

Comparison between osteonecrosis of the humeral and femoral heads - epidemiological analysis of the surgical trend using the nationwide claims database of the republic of Korea.

BACKGROUNDS: The humeral head is the second most common site of osteonecrosis, after the femoral head. However, compared to osteonecrosis of the femoral head (ONFH), epidemiological information on osteonecrosis of the humeral head (ONHH) is scarce. We hypothesised that different biomechanical properties of the shoulder from the hip joint might present different epidemiological characteristics of ONHH from those of the ONFH. To evaluate epidemiological differences, we compared trends in the surgical treatment of ONHH and ONFH using the nationwide medical claims database of the Republic of Korea

Bone mineral density, mechanical properties, and trabecular orientation of cancellous bone within humeral heads affected by advanced shoulder arthropathy.

The mechanical properties of cancellous bone in the humeral head are increasingly interesting due to the increased popularity of stemless prosthetic fixation in the cancellous bone of the metaphysis. Age or pathology-related systemic osteoporosis, inactivity, or pathology of the shoulder joint may influence the primary bonding of implants that rely on good cancellous bone quality. We assessed the bone mineral density (BMD) and anisotropy using micro-computed tomography (micro-CT) (0.04 mm voxel size) and correlated the results with indentation load/displacement response. Resected parts of hume

Postoperative infection caused by Acinetobacter baumannii misdiagnosed as a free-living amoeba species in a humeral head hemiarthroplasty patient: a case report.

BACKGROUND: Acinetobacter baumannii is ubiquitous, facultative intracellular, and opportunistic bacterial pathogen. Its unique abilities allow it to survive in a diverse range of environments, including health care settings, leading to nosocomial infections. And its exceptional ability to develop resistance to multiple antibiotics leaves few drug options for treatment. It has been recognized as a leading cause of nosocomial pneumonia and bacteremia over the world. CASE PRESENTATION: In this case, a 73-year-old woman presented with a Neer Group VI proximal humeral fracture. Six hours after a s

Survey study suggests that reverse total shoulder arthroplasty is becoming the treatment of choice for four-part fractures of the humeral head in the elderly.

PURPOSE: The role of reverse total shoulder arthroplasty (RTSA) for three and four-part proximal humerus fractures is evolving. However, there does not appear to be a clear consensus amongst surgeons. The purpose of this study is to further define the standard of care, assessing surgeon preference and treatment considerations for management of such fractures. METHODS: Orthopaedic surgeons were surveyed on their training, practice setting, and experience regarding management of four-part proximal humerus fractures. The survey also presented five representative cases to assess treatment prefere

Functional malcentering of the humeral head and asymmetric long-term stress on the glenoid: potential reasons for glenoid loosening in total shoulder arthroplasty.

We tested the hypothesis that functional malcentering of the humeral head during arm elevation exists in patients with glenohumeral osteoarthritis and influences long-term glenoid loading. Twenty-eight healthy volunteers and 10 patients with primary osteoarthritis, 10 with cuff-arthropathy, and 1 with dysplastic glenoid were examined. Open magnetic resonance imaging and 3-dimensional (3D) digital postprocessing techniques were applied in various arm positions. Osteoabsorptiometry was used to determine 3D subchondral mineralization patterns of the glenoid as an indicator of integral long-term s

Management of Bilateral Humeral Head Avascular Necrosis in Sickle Cell Disease Using Surgical Intervention With Adjuvant Hyperbaric Oxygen Therapy: A Case Report.

BACKGROUND Avascular necrosis (AVN) is a disabling musculoskeletal complication of sickle cell disease (SCD), resulting from recurrent vaso-occlusive episodes and chronic microvascular ischemia. Although weight-bearing joints such as the femoral head are most commonly affected, humeral head involvement can lead to significant pain and functional limitation. Management of AVN in SCD remains challenging, and interest has grown in adjunctive therapies such as hyperbaric oxygen therapy (HBOT), which can enhance tissue oxygenation, promote angiogenesis, and support bone remodeling. Evidence regardi
