The effects of multitype prompt engineering for large language models in hypertension treatment decisions.

The effects of various prompt engineering on Large Language Models (LLMs) performance in hypertension decision-making are not yet fully understood. We evaluate the impact of different prompt engineering on LLM performance in hypertension treatment decision-making. We conducted a two-stage validation study using 300 de-identified simulated hypertension cases based on real-world clinical scenarios. ChatGPT-4.1 with Guidance-Self-Consistency achieved optimal performance (91.3% accuracy), nearing expert-level competency, while zero-shot prompting yielded worst results (62.7% with DeepSeek-V3). Opt…

Practical Guide to Large Language Models for Information Extraction in Behavioral Health Notes: Tutorial.

BACKGROUND: Mental health clinical notes contain decision-critical information often absent from structured electronic health record fields. Large language models (LLMs) can extract clinically relevant signals from narrative text; however, variability in output format, limited reproducibility, and inconsistent evaluation remain barriers to clinical deployment. Despite rapid advances in LLM-based information extraction, clear and reproducible guidance for interdisciplinary clinical teams is limited. OBJECTIVE: This tutorial aims to present a structured workflow for zero-shot information extrac…

A S.C.O.R.E. framework for evaluating open-ended responses from large language models in healthcare.

Domain-specific evaluation is essential for clinical validation. We propose S.C.O.R.E. (Safety, Consensus & Context, Objectivity, Reproducibility, Explainability), a five-dimensional framework for structured expert evaluation of LLM-generated healthcare responses. S.C.O.R.E. has been validated against quantitative metrics (BLEU, ROUGE, and BERTScore) using three LLMs (GPT-4o, Claude 4 Sonnet, and DeepSeek) across ophthalmology, medication, and anesthesia. While quantitative metrics frequently misclassified clinically appropriate responses as inaccurate, S.C.O.R.E. demonstrated acceptable inter

Few-shot ECG analysis with large vision language models in data-scarce clinical settings.

Electrocardiogram (ECG) interpretation is fundamental for cardiac diagnosis. Machine learning has proven strong performance in ECG analysis but models often face challenges when deployed in real-world settings due to difficulties adapting to new environments. Recent progress, particularly through training on large, diverse datasets across various ECG-related tasks, has led to better model generalization. However, many cardiac conditions, particularly rare ones, remain scarce in existing datasets, leading to diminished model performance in these critical cases. This underrepresentation remains

Performance of large language models as a source of clinical information on bacteriophage therapy.

Bacteriophage therapy is re-emerging as a potential strategy to address antimicrobial resistance, but standardized patient education materials are limited. Large language models (LLMs) are increasingly used for patient-facing medical information. The quality of LLM-generated responses to 20 patient-relevant questions was evaluated by 12 clinicians and research experts in bacteriophage therapy independently rated each response for accuracy, completeness, clarity, and tone/empathy using 5-point Likert scales. Expert suggestions for improvement were recorded. A total of 960 ratings were analyzed.…

Multimodal Dementia Prediction With Large Language Models: Cross-Attention Over Text, Audio, and Image.

BACKGROUND: Alzheimer disease (AD) is a leading cause of dementia, and there is growing interest in scalable approaches for early screening using speech-based tasks. While prior work has demonstrated promising results using either transcript-based language features or acoustic cues, most approaches remain unimodal or rely on simple fusion strategies that do not explicitly consider interactions across modalities. OBJECTIVE: In this study, we propose an attention-based trimodal fusion framework that integrates text, audio, and image representations of the Cookie Theft picture, which serves as t

Large Language Models in Multidisciplinary Decision-Making for Hepatopancreatobiliary Oncology: Retrospective Comparative Feasibility Study.

BACKGROUND: Hepatopancreatobiliary (HPB) malignancies require complex treatment planning that often relies on multidisciplinary team (MDT) discussions. Large language models (LLMs) have recently been explored for clinical decision support, but their performance within real-world multidisciplinary decision environments remains unclear. In particular, the stability of LLM-generated recommendations-that is, whether a model produces the same answer when given the same clinical input-has rarely been examined. OBJECTIVE: This study aimed to evaluate the stability of treatment recommendations genera…
