Acute massive pulmonary thromboembolism (PTE) is a potentially life-threatening condition requiring urgent management to decrease mortality. However, in the peripheral setting, managing the emergency can be challenging. We report a case of massive PTE presenting with cardiopulmonary arrest, successfully managed with advanced cardiac life support, early initiation of anticoagulants (heparin), and thrombolytics. This case report explores the successful management of a critically ill patient presenting with massive pulmonary embolism in a peripheral healthcare facility, emphasizing the importance

Taking care of offspring suffering a long-term illness such as schizophrenia is one of the more stressful life experiences. Care conditions may act as a protective factor in the health of the caregiver. The present study assesses heart rate (HR), blood pressure (BP), and mood responses to psychosocial stress in 16 mothers receiving specialised support for the care of their offspring (CARE+) and in 11 mothers caring for their offspring without support (CARE-). The CARE- group take care of less functional and more symptomatic offspring; and display higher basal, but lower HR, responses after str

BACKGROUND: Treatment-limiting decisions (TLDs) in neurointensive care are frequently reported with no distinction between withholding and withdrawal of treatment. We investigated the proportion of patients subjected to withholding and withdrawal of life-sustaining treatment during neurointensive care, and the association with mortality.
METHODS: This retrospective observational cohort study included all adult (≥ 18 years) neurocritically ill patients admitted to the neurointensive care unit (neuro-ICU), Rigshospitalet, Denmark, for a primary CNS injury during the period from July 2019 to Feb

IMPORTANCE: Children with medical complexity (CMC) frequently experience recurrent deterioration requiring life-sustaining treatment (LST) decisions in the pediatric intensive care unit. Parents serve as primary caregivers and decision makers; however, how accumulated caregiving experiences shape thresholds for acceptable LST during crises remains insufficiently understood.
OBJECTIVES: To explore how parents of CMC interpret their longitudinal caregiving experiences and how these interpretations shape parents' orientations toward LST during episodes of clinical deterioration.

OBJECTIVE: To validate the content of Primary Care Network Assessment Scale for Palliative Care Patients (ARCP) and to assess its clinical utility in patients with advanced cancer.
METHODS: This methodological study was conducted in two phases. Phase 1 involved content validation by ten social work experts using the Content Validity Index (CVI). Phase 2 evaluated clinical utility in a cross-sectional study of 148 patients with advanced cancer receiving palliative care. Data included sociodemographic and clinical variables, global quality of life (EORTC QLQ-C15-PAL) and perceived social suppor

One of the most difficult decisions that doctors and parents must make is the decision to withdraw life-sustaining treatment. Doctors find it easier to withdraw treatments in situations where withdrawal will be rapidly fatal rather than in situations in which treatment withdrawal will lead to a prolonged dying process. Mechanical ventilation is usually such a treatment. Withdrawal of ventilation generally leads to the patient's rapid demise. Doctors may tell parents that death will occur quickly after a ventilator is withdrawn. But what happens when the doctors are wrong and a patient survives

INTRODUCTION: Decisions to withdraw or withhold curative or life-sustaining treatment can have a huge impact on the symptoms which the palliative-care team has to control. Palliative-care patients and their relatives may also turn to palliative-care physicians and nurses for advice regarding these treatments. We wanted to assess Indian palliative-care nurses and physicians' attitudes towards withholding and withdrawal of curative or life-sustaining treatment.
METHOD: From May to September 2008, we interviewed 14 physicians and 13 nurses working in different palliative-care programmes in New D

Modern military medicine is an ever advancing field, capable of remarkable feats. We present the case of a 16-year-old Iraqi female with a single gunshot wound to the pelvis with extensive rectal injury who survived despite the odds. Using "damage control" techniques, this patient survived despite a reported 30% mortality associated with such wounds. During the course of her stay, she was cared for by a multidisciplinary team whose integrated efforts led to survival and complete closure of 2 of 3 wounds prior to discharge. The case demonstrates the success of modern military medical measures a

Nutrition in palliative care and at the end of life should be one of the goals for improving quality of life. It is important to address issues of food and feeding at this time to assist in the management of troublesome symptoms as well as to enhance the remaining life. While this paper focuses upon the nutritional aspects of cancer in palliative care, the sentiments are applicable to other serious chronic illnesses such as advanced cardiac failure, chronic obstructive pulmonary disease and dementia. Cancer and its treatments exert a major impact upon physical and psychological reserves and at

The provision of palliative care has been seen to have a significant impact on the interprofessional health care team. Being emotionally present to support patients and families at their most stressful points in life can result in compassion satisfaction or compassion fatigue. Providing palliative care through the COVID-19 pandemic added another layer of complexity to this professional caregiving experience. Although efforts have been made by employers to support health care providers in the practices of self-care and mindfulness, the true experience of care providers was not known. This study

The Universal Declaration of Human Rights recognizes the inherent dignity, the equal and unalienable rights to be universally protected for all humans irrespective of race, color, gender, language, religion, political or other opinion, national or social origin, property, birth or other status. Though this includes the right to dignity-conserving care for terminally ill unauthorized immigrants, access to quality end-of-life care eludes them. Most of the estimated 11.3 million unauthorized immigrants either entered the country without the knowledge of the U.S. Immigration and Customs Enforcemen

BACKGROUND: Pain is a frequently reported symptom by patients approaching the end of life and well-established that patients and carers hold fears relating to opioids, and experience side effects related to their use. The management of medicines is intrinsic to achieving effective pain relief. The concept of self-management support whilst well characterised in the context of chronic illness has not been elaborated with respect to end of life care.
AIM: To identify patient, carer and professional views on the concept of self-management support at end of life, specifically in relation to analge

BACKGROUND: The growth in guidance development projects has focused attention on the methods used in developing the guideline. For a guideline to be sound it should be linked on the basis of scientific evidence to the very health outcome that the guideline is designed to promote.
METHODS: Structuring a health intervention as an influence diagram, a decision model (1) allows for the identification of the relevant benefits, harms, and costs that may result from an intervention; (2) provides an explicit link between the intervention and these outcomes, a crucial prerequisite for the development

BACKGROUND: A growing number of older people are living in single households. They form a disadvantaged group within society as regards staying at home, most likely towards the end-of-life. It is mainly non-kin-carers who try to fulfil older people's desire for a home death, but very little is known about the challenges they face during their involvement.
AIM: Getting insight into the engagement of non-kin-carers in the support for older people living alone, and a better understanding of the challenges they have to manage in end-of-life care.
DESIGN: Exploratory qualitative design; perspecti

Advanced breast cancer (ABC) represents a significant global health challenge, not only due to its high mortality rates but also because of the profound physical, emotional, and social burden it imposes on patients. Support services - encompassing psychological, social and peer support, complementary and integrative therapies, wellness and lifestyle support, genetic counseling, survivorship programs, palliative and end-of-life care - are essential to addressing these challenges and ensuring that individuals with ABC receive holistic, person-centered care throughout their disease journey. This

BACKGROUND: Families, friends, and communities play a vital role in supporting individuals facing declining health, caregiving duties, loss, or grief, especially with the growing desire to die at home. The general public can significantly impact end-of-life care and offer essential support mechanisms. This review aimed to explore and identify key educational components related to palliative and end-of-life care for citizens, volunteers, and the general public.
METHODS: A mixed-method systematic review was conducted, incorporating four electronic databases (MEDLINE, PsycINFO, CINAHL, and the C

Identifying and meeting patients' spiritual needs is integral to nursing in end of life care. However, spirituality, spiritual care and assessment models are still not fully embedded across the UK. While nurses often collaborate with chaplains or other spiritual care professionals to support patients, ambiguity and misunderstanding between spirituality and religion can cause challenges in delivering effective, person-centred care. This article explores the role of spirituality in nursing practice and introduces structured reflections and practical tools to support spiritual care in end of life

Tuberculosis (TB) continues to be a major public health challenge in India, contributing significantly to mortality, morbidity, and long-term disability. Although TB is considered a curable disease, people with TB frequently experience substantial physical, psychological, social, and spiritual suffering throughout the disease trajectory. Drug-resistant tuberculosis (DR-TB), recurrent TB, HIV-associated TB, severe extrapulmonary disease, and post-tuberculosis lung disease (PTLD) are associated with particularly high symptom burden and impaired quality of life. Traditionally, palliative care has
