Myositis and eosinophilia in a patient with sarcoidosis.

We present a patient with muscle manifestations of sarcoidosis associated with raised creatine kinase levels, eosinophilia and interstitial lung disease. She had significant improvement on prednisolone 30 mg per day.

Focal myositis of the temporal muscle.

Focal myositis is a rare inflammatory disease of the skeletal muscle that may involve any part of the body. We present a 19-year-old man with a short history of a painful pseudotumor in the right temporal region. MRI pictures showed a swollen right temporal muscle and muscle biopsy revealed the characteristic histological and immunocytochemical findings of polymyositis.

[A case of chronic multifocal myositis].

A 61-year-old civil engineer began to have slowly progressive muscle atrophy in the right shoulder and the left arm at 56 years of age. Muscle wasting became manifest in the left thigh at 59 years and in the right thigh at 60 years. He had mild difficulty in climbing and descending stairs. On examination, although he had notable muscle atrophy in the right trapezius and proximal muscles in the upper and lower extremities, his muscle strength was relatively well preserved. The muscle atrophy was asymmetrical; the right periscapular region and the left upper and lower extremities were more marke

Resolution of myositis ossificans in a hemophiliac.

Hemophilic myositis ossificans at the site of previous hemorrhage is a well recognised entity. A phenomenon, not previously described, is gradual absorption of a well corticated heterotopic bone over a period of five-and-a-half years. We here report this happening in a patient.

Recent advances in the morphology of myositis.

Myositis in man may be divided into infectious and non-infectious forms. The myopathologist more often deals with the latter forms which comprise dermatomyositis/polymyositis, inclusion body myositis, mixed connective tissue disease/collagenoses, and granulomatous myopathies. Modern morphological techniques as enzyme-histochemistry, electron microscopy, immunohistology, and morphometry are of different value in various forms of myositis, but are often indispensable techniques in up-to-date diagnostic work up of a myositis.

Multiplicity of antibodies in myositis sera.

Serologic studies on 114 patients with polymyositis and dermatomyositis revealed that 89% had either a precipitating antibody to antigens in calf thymus extract or a positive immunofluorescent reaction on HEp-2 cells, a human tissue culture line. Previously, the greatest proportion of polymyositis sera demonstrating positive serologic results (i.e., the proportion of patients' sera forming precipitates with calf thymus extract) was reported to be 60%. Use of the HEp-2 cell as immunofluorescent substrate enabled the detection of antibody in 89 (78%) of the sera, providing the additional probe w…

Performance and potential impact of the IMACS cancer screening guideline in myositis: analysis in a myositis referral centre.

OBJECTIVES: To evaluate the performance, direct costs and radiation exposure of the cancer screening strategy proposed by the International Myositis Assessment and Clinical Studies Group (IMACS) in patients with idiopathic inflammatory myopathies (IIMs) at a myositis referral centre compared with real-world cancer screening. METHODS: This is a retrospective study involving consecutive patients registered in a single-centre prospective IIM database from 2014 to 2024 who had no cancer within 3 years before IIM diagnosis. The patients were stratified into high, moderate or standard risk of cance…

The Overlapping Spectrum of Primary Tubercular Myositis and Immune-Mediated Myositis: Diagnostic Challenges in Three Cases.

Primary tubercular myositis is a rare extra-pulmonary manifestation of tuberculosis (TB). We present three cases of biopsy-proven granulomatous myositis, all of which had an initial clinical picture suggestive of dermatomyositis and serological positivity for a myositis-specific antibody. Case 1 was initially diagnosed as dermatomyositis, but the patient, a 46-year-old woman, deteriorated rapidly after immunosuppression and succumbed to disseminated TB. In Case 2, the patient, a 43-year-old woman, developed subacute proximal weakness after the initiation of anti-tuberculous therapy (ATT) for l…

Focal Myositis Localised in Gastrocnemius Muscle: is it Different from Isolated Gastrocnemius Myositis? A Case Report.

Focal myositis is a rare disease defined by an isolated inflammatory pseudotumour usually restricted to one skeletal muscle. Approximately, 250 cases of focal myositis have been described in the literature, and two recent large cohorts have been used to help in the diagnosis. Isolated gastrocnemius myositis, a rare immune-mediated condition, is a diagnostic entity used by internal medicine clinician in the gastrocnemius myalgia syndrome associated with Crohn's disease (CD). However, focal myositis and isolated gastrocnemius myositis with Crohn's disease share clinical, haematological, patholog…

Myositis with endomysial cell invasion indicates inclusion body myositis even if other criteria are not fulfilled.

The objective of this study was to investigate if patients with endomysial mononuclear cell infiltrates invading non-necrotic fibers have a disease course consistent with inclusion body myositis (IBM), irrespective of other histopathological and clinical characteristics. All patients with a muscle biopsy showing endomysial inflammation with invasion of non-necrotic muscle fibers during the period 1979-2006 in two tertiary neuromuscular referral centers were classified into three groups: 1) patients whose biopsies also showed rimmed vacuoles; 2) patients whose biopsies showed no vacuoles but fu…

Inclusion-body myositis associated with Sjögren's disease: clinical characteristics and comparison with other Sjögren-associated myositis.

OBJECTIVES: To describe the characteristics of patients with Sjögren's disease (SjD) and inclusion-body myositis (IBM), and how they compare to SjD patients with other inflammatory myopathies (IM). METHODS: Patients were retrospectively recruited from 13 French centres and included if they met the ACR/EULAR criteria for SjD and for IM. They were categorized as SjD-IBM if sub-criteria for IBM were met, or as SjD-other IM if not. RESULTS: SjD-IBM patients (n = 22) were mostly females (86%), with a median [Q1; Q3] age of 54 [38.5; 64] years at SjD diagnosis, and 62 [46.5; 70] years at first IBM…

Testing for myositis specific autoantibodies: Comparison between line blot and immunoprecipitation assays in 57 myositis sera.

OBJECTIVE: To analyze the performance of a line blot assay for the identification of autoantibodies in sera of patients affected by myositis, compared with immunoprecipitation (IP) as gold standard. METHODS: 66 sera of patients with myositis (23 polymyositis, 8 anti-synthetase syndromes, 29 dermatomyositis and 6 overlap syndromes) were tested by commercial LB (Euroimmun, Lubeck, Germany); 57 sera were analyzed also by IP of K562 cell extract radiolabeled with (35)S-methionine. Inter-rater agreement was calculated with Cohen's k coefficient. RESULTS: Myositis-specific antibodies (MSA) were de…

In adult onset myositis, the presence of interstitial lung disease and myositis specific/associated antibodies are governed by HLA class II haplotype, rather than by myositis subtype.

The aim of this study was to investigate HLA class II associations in polymyositis (PM) and dermatomyositis (DM), and to determine how these associations influence clinical and serological differences. DNA samples were obtained from 225 UK Caucasian idiopathic inflammatory myopathy patients (PM = 117, DM = 108) and compared with 537 randomly selected UK Caucasian controls. All cases had also been assessed for the presence of related malignancy and interstitial lung disease (ILD), and a number of myositis-specific/myositis-associated antibodies (MSAs/MAAs). Subjects were genotyped for HLA-DRB1,…
