Baseline trajectories of drinking moderate acamprosate and naltrexone effects in the COMBINE study.

BACKGROUND: The COMBINE study evaluated the effects of acamprosate, naltrexone, and the Combined Behavioral Intervention (CBI). In secondary analyses, our goals were to identify trajectories of any drinking prior to randomization, to characterize subjects in these trajectories, and to assess whether prerandomization trajectories predict drinking outcomes and moderate treatment response. METHODS: We analyzed daily indicators of any drinking in 90 days prior to randomization using a trajectory-based approach. General linear models and generalized logistic regression assessed main and interactiv…

A pharmacogenetic model of naltrexone-induced attenuation of alcohol consumption in rhesus monkeys.

BACKGROUND: Variation at the human mu-opioid receptor has been associated with alcohol abuse. The A118G (N40D) polymorphism in humans is functionally mimicked by the C77G (P26R) polymorphism in rhesus monkeys; both show similar in vitro influences on ligand binding and in vivo correlations with physiological measures as well as behavioral measures including predilection towards alcohol consumption. Naltrexone, an antagonist at the receptor, has been used to treat alcoholism in humans and has been reported to show differences in effectiveness depending on genotype. METHODS: Here we describe a …

Effects of naltrexone on electrocutaneous pain in patients with hypertension compared to normotensive individuals.

An opioid mechanism may help explain hypertensive hypoalgesia. A double-blind placebo-controlled design compared the effects of opioid blockade (naltrexone) and placebo on electrocutaneous pain threshold, pain tolerance, and retrospective McGill Pain Questionnaire ratings in 35 unmedicated patients with essential hypertension and 28 normotensive individuals. The hypertensives experienced less pain than normotensives during the assessment of their pain tolerance; however, this manifestation of hypertensive hypoalgesia was not moderated by naltrexone. These findings fail to support the hypothesi…

Influence of naltrexone administration on dehydroepiandrosterone sulfate levels in male and female participants.

Dehydroepiandrosterone sulfate (DHEAS) is an excitatory neurosteroid with anti-glucocorticoid properties. Endogenous opioid system blockade is known to activate the hypothalamic-pituitary-adrenal axis and other hormonal systems. However, the literature is sparse regarding the extent to which this blockade acutely influences DHEAS activity. Further, the stability of DHEAS concentrations across short term laboratory studies is not well established. The current study examined these issues in human participants. Using a double-blind, counterbalanced design, 50mg of naltrexone and placebo were admi…

Pharmacokinetics and pharmacodynamics of carfentanil and naltrexone in female common eland (Taurotragus oryx).

The pharmacokinetic parameters of carfentanil and naltrexone were determined in the common eland (Taurotragus oryx). Six adult females were immobilized with xylazine (0.23 +/- 0.03 mg/kg i.m.) and carfentanil (0.0169 +/- 0.0005 mg/kg i.m.) for a 45-min period, during which time routine health care procedures were performed. Heart and respiration rates and body temperatures were monitored throughout the immobilization period. A single intramuscular injection of naltrexone (1.66 +/- 0.08 mg/kg i.m.) was sufficient for reversal. The eland were intermittently restrained in a hydraulic squeeze chut…

Sequential analysis of the effects of naltrexone on the environmental mediation of self-injurious behavior.

Accumulated evidence shows that biology and the environment can mediate self-injurious behavior (SIB) in persons with mental retardation. Wh … l dependency between staff behavior and self-injury was significantly greater during treatment with naltrexone than during treatment with a placebo. Results are discussed in relation to behavioral mechanisms of action regulating medication effects for self-injury.

Effects of naltrexone and CCK on estrous behavior and food intake in Syrian hamsters.

Food deprivation inhibits estrous behavior in several species of rodents, but little is known about the neurotransmitter systems mediating this phenomenon. We determined whether partial blockade of opioid receptors by continuous infusion of naltrexone and/or acute peripheral injection of cholecystokinin (CCK) administration would overcome the suppressive effects of food deprivation on estrous behavior in Syrian hamsters. Contrary to expectation, naltrexone produced a slight suppression of estrous behavior, and systemic CCK administration had no effect. This dose of naltrexone was sufficient to…

High-performance liquid chromatographic determination of naltrexone in plasma of hemodialysis patients.

A simple, sensitive, selective and reliable reversed-phase high-performance liquid chromatographic (HPLC) method with UV detection is described for the determination of naltrexone in plasma samples. Naltrexone and the internal standard, naloxone, were isolated from plasma either with a liquid-liquid extraction method using ethyl acetate or with a solid-phase extraction method using Sep-Pack C18 cartridge before chromatography. The extracts were dried under a stream of nitrogen and the samples were reconstituted in the mobile phase, then 20 microL were injected on a Waters Symmetry C18 column (…

Naltrexone does not attenuate the acute behavioral effects of ethanol or pentobarbital in humans.

The aim of the present study was to determine whether naltrexone, an opioid antagonist, attenuates the acute subject-rated, performance-impairing and physiological effects of ethanol or pentobarbital. To accomplish this aim, two separate experiments were conducted. In Experiment 1, eight volunteers (one female, seven males) received ethanol (0, 0.5 and 1.0 g/kg) alone and in combination with naltrexone (0, 50 and 100 mg). In Experiment 2, eight different volunteers (five females, three males) received pentobarbital (0, 150 and 300 mg), alone and in combination with naltrexone (0, 50 and 100 mg…

Effects of naltrexone on amphetamine-induced locomotion and rearing: acute and repeated injections.

The present experiment investigated the ability of the opiate receptor antagonist naltrexone to block the increased locomotion and rearing produced acutely by amphetamine as well as the sensitization of these responses produced when this drug is administered repeatedly. Rats in different groups received an injection of amphetamine (1.5 mg/kg, i.p.) or saline preceded 30 min earlier by an injection of naltrexone (0, 0.5, 1.0, 5.0 or 10.0 mg/kg, i.p.). Naltrexone dose-dependently reduced the rearing but had no effect on the locomotion produced by this dose of amphetamine. The locomotion and rear…

Naltrexone plasma levels, clinical response and effect on weight in autistic children.

This research measured naltrexone levels in plasma, to assess the relationship between behavioral response to naltrexone and plasma levels, and the effects of naltrexone on weight in hospitalized autistic children (n = 41). A double-blind, placebo-controlled, parallel groups design with fixed dose was used, with random assignment to naltrexone or placebo. Drug plasma levels were analyzed by gas chromatography/mass spectrometry, and weights were obtained weekly. Naltrexone levels measured in 17 children ranged from 0.12 to 5.60 ng/mL (mean = 0.71, standard error of the mean = 0.32). There was n…
