Bilateral occlusion of the common carotid arteries of urethane-anesthetized rats evoked a pressor response of 14 +/- 1 mm Hg. Injection into the lateral cerebral ventricle of neostigmine (0.2-1.0 microgram) or physostigmine (10-15 microgram) caused a dose-dependent increase in basal blood pressure and in the magnitude of the carotid artery occlusion (CAO) pressor reflex. Neostigmine (1 microgram) and physostigmine (15 microgram) caused nearly maximal and approximately equal degrees of cholinesterase inhibition in several brain regions. The recovery of the cardiovascular parameters and of brain…

Four patients were admitted with respiratory muscle paralysis following an elapid snake bite. Two were treated with antivenine, repeated doses of neostigmine and artificial ventilation; the other two with artificial ventilation only. There was no clinically significant difference in recovery. It is suggested that one should avoid the use of antivenine and neostigmine in the management of elapid snake bite once muscle paralysis has become established.

Background/Objectives: Perioperative neurocognitive disorders (PND), including postoperative delirium and cognitive dysfunction (POCD), represent significant complications in elderly surgical patients undergoing general anesthesia. The choice of neuromuscular blockade reversal agent may influence POCD risk through different mechanisms and side effects. This systematic review and meta-analysis evaluated the comparative effect of neostigmine versus sugammadex on POCD incidence in elderly patients. Methods: A systematic search of PubMed, Web of Science, Scopus, and Google Scholar was conducted fr…

PURPOSE: To describe the systemic benefits of sugammadex as a reversal agent for neuromuscular blockade.
SUMMARY: Sugammadex is the preferred reversal agent for neuromuscular blockade as recommended in guidelines from the American Society of Anesthesiology. This recommendation is primarily based on a faster time to recovery from neuromuscular blockade as compared to that seen with neostigmine. Systemic adverse effects of neostigmine are often mitigated by the administration of glycopyrrolate. However, glycopyrrolate is an antimuscarinic agent that may be associated with its own adverse effect…

Muscarinic receptors involved in cholinergic neurotransmission were studied in isolated innervated guinea-pig tracheas using preganglionic (nerve) and postganglionic (field) stimulation, after blocking sympathetic effects with bethanidine (5 microM). Neostigmine (10 nM) significantly increased responses to nerve and field stimulation. The M1 antagonist pirenzepine (0.1-100 nM) selectively reduced tracheal responses to nerve stimulation in control and in neostigmine-treated tissues. The M2 antagonist gallamine (0.1-100 microM) significantly increased tracheal responses to nerve and field stimul…

Background: Studies have proposed that the routine use of the modified gamma-cyclodextrin, sugammadex, could provide perioperative time savings. However, these investigations have been limited to small group analyses. The purpose of this study was to test the effectiveness of sugammadex on perioperative times when compared to neostigmine under general clinical practice conditions following rocuronium-induced neuromuscular blockade for laparoscopic cholecystectomy. Methods: Following institutional review board approval, data from 1,611 consecutive surgical records for laparoscopic cholecystecto…

BACKGROUND: Non-depolarizing neuromuscular blockade can be reversed with neostigmine/glycopyrrolate or sugammadex. We test the hypothesis that sugammadex is associated with earlier postoperative recovery of bowel function (first bowel movement, BM).
METHODS: In adult patients undergoing craniotomy from 2016 to 2019, we identified time of first postoperative BM after receiving neostigmine/glycopyrrolate or sugammadex to reverse neuromuscular blockade. Logistic and proportional hazard regression, with and without inverse probability of treatment weighting (IPTW), were used to assess whether sug…

A soldier presented in Jan 2002 with features of proximal myopathy and diplopia. Clinically he had features of myasthenia gravis, which was confirmed by significantly positive neostigmine test, decremental response on electrophysiological study and raised acetylcholine receptor antibody titres. He also tested positive for HIV during evaluation of a cervical lymph node detected incidentally. He responded well to neostigmine and has remained asymptomatic on follow up.

Paralytic ileus is a major concern in the acute phase of spinal cord injury. Classical treatment with neostigmine is often ineffective. Continuous intravenous (i.v.) lidocaine infusion has been previously proposed intra and post-operatively in order to decrease the duration of post-operative ileus after abdominal surgery. We report the cases of seven patients suffering from complete paralytic spinal cord injury-related ileus with colectasy resistant to neostigmine, who were treated by i.v. lidocaine infusion.

Cholinergic drugs can modulate anaphylactic shock and change lymphocyte functions. Plasma proteins modulate effects of muscarinic antagonists during anaphylactic shock. The present investigation was carried out to study the antianaphylactic activity of methacine (antagonist at muscarinic receptors) in combination with neostigmine (anticholinesterase drug). However, it is not known whether plasma proteins-albumin, C-reactive protein (CRP) and immunoglobulin G (IgG) - modify the effects of cholinergic drugs like methacine, serotonin (5-HT) level in the lymphoid organs and quantity of antibody-fo…

The effect of galanthamine, an anticholinesterase having a tertiary ammonium group, on a constant infusion of pancuronium was studied in the rat tibialis anterior-sciatic nerve preparation. The results were compared with those previously obtained with neostigmine, pyridostigmine, and 4-aminopyridine. The antagonizing activity of galanthamine is similar to that of 4-aminopyridine and less than that of the other 2 anticholinesterases. The duration of galanthamine antagonism is similar to that of neostigmine and pyridostigmine but less than that of 4-aminopyridine.

The roles of hypothalamic neuron mechanisms in regulation of blood glucose were studied in rats. Microinjection of HgCl2 and neostigmine into the third ventricle under anesthesia caused marked hyperglycemia. To characterize the effective neurons in the hypothalamus, tissue contents of neurotransmitters and extracellular levels of metabolites in the ventromedial part of the hypothalamus were measured by microwave irradiation and in vivo brain microdialysis. HgCl2 increased both the content and the extracellular levels of choline. Neostigmine increased the acetylcholine content. In both instance…

INTRODUCTION: Crotamine is a basic, low-molecular-weight peptide that, at low concentrations, improves neurotransmission in isolated neuromuscular preparations by modulating sodium channels. In this study, we compared the effects of crotamine and neostigmine on neuromuscular transmission in myasthenic rats.
METHODS: We used a conventional electromyographic technique in in-situ neuromuscular preparations and a 4-week treadmill program.
RESULTS: During the in-situ electromyographic recording, neostigmine (17 μg/kg) caused short-term facilitation, whereas crotamine induced progressive and susta…
