"General surgical pearls" for the anterior exposure of vertebral fractures.

Recently interest has been increasing in the anterior surgical approach for spinal cord decompression and bony stabilization of vertebral compression fractures. Our neurosurgical spine service routinely consults us to provide anterior operative exposure and wound closure for all levels of the thoracic and lumbar vertebral spine. Averaging about 30 exposures per year we have developed an excellent operative experience with these vertebral exposures. With no complete general surgery reference on anterior vertebral identified this summary of our "general surgical pearls" that we have learned and/

Neurosurgical Biopsy and Resection for Diagnosis and Treatment of Balamuthia mandrillaris Amebic Encephalitis, United States.

We report a systematic case review of antemortem neurosurgical resections and biopsies and outcomes including new lesions after procedure and survival in Balamuthia mandrillaris granulomatous amebic encephalitis. The investigation was prompted by a 5-year-old patient in the southwestern United States who was treated with nitroxoline, the 2021 Centers for Disease Control and Prevention regimen, and underwent 2 resections; initial resection site recurrence and a new lesion after resection prompted the question whether complete resection versus biopsy is associated with better outcomes. We conduc

The Potential Usefulness of Magnetic Resonance Guided Focused Ultrasound for Obsessive Compulsive Disorders.

Obsessive compulsive disorder is a debilitating condition characterized by recurrent obsessive thoughts and compulsive reactions. A great portion of the obsessive compulsive disorder (OCD) patients are managed successfully with psychiatric treatment such as selective serotonin-reuptake inhibitor and cognitive behavioral psychotherapy, but more than 10% of patients are remained as nonresponder who needs neurosurgical treatments. These patients are potential candidates for the neurosurgical management. There had been various kind of operation, lesioning such as leucotomy or cingulotomy or capsul

Ultra-Late shunt infection in the paediatric population.

BACKGROUND: Referral to neurosurgical services for suspected shunt infection commonly occurs in shunted children who present with fever and sepsis of unknown origin. While early shunt infections are common, ultra-late shunt infections (Beyond 1 year) are exceedingly rare. This study aims to investigate the incidence and causes of ultra-late shunt infection within the paediatric neurosurgical population. METHODS: We conducted a retrospective study of all shunt procedures performed between the years 2010 and 2025. Delayed shunt infections were defined as infections occurring 1 year or later aft

Virtual Reality vs Phantom Model: Benefits and Drawbacks of Simulation Training in Neurosurgery.

BACKGROUND AND OBJECTIVES: Traditional neurosurgical education has relied heavily on the Halstedian "see one, do one, teach one" approach which is increasingly perceived as inefficient in contemporary settings marked by a steady decline in surgical caseload. In recent years, simulation training has emerged as an effective and accessible training alternative. To date, however, there is no standardized criterion pertaining to the quality and implementation of simulators in neurosurgical education and training. This research aims to compare the efficacy of virtual reality (VR) and Phantom-based s

Neurosurgical treatment for Gilles de la Tourette syndrome: the Italian perspective.

Despite the availability of both pharmacological and behavioral therapies for Gilles de la Tourette Syndrome (GTS), a subgroup of patients suffer intractable disease and require treatment through non-conservative means. Since 1955, various neurosurgical procedures have been considered as a potential resort for this severely affected sub-group; this article reviews the neurosurgical treatment for GTS, with in-depth discussion on deep brain stimulation (DBS). Internationally, 39 cases of GTS undergoing DBS treatment have been published. Yet, despite the small numbers of patients assessed in cent…

Neurosurgical intervention in a haemophilic child with a subdural and intracerebral haematoma.

A case of a 3-year-old boy with severe haemophilia A who had a successful neurosurgical drainage of a combined spontaneous left temporal subdural and intra-parenchimal haematoma is reported. Surgical intervention was required because of clinical worsening during conservative treatment with dexamethasone and factor VIII (FVIII) replacement therapy. Continuous FVIII infusion was given before, during and after the procedure. There were no surgical complications and neurological examination remains intact. Neurosurgical interventions may be reserved for special, high-risk cases, as the one present

Association of Neurosurgical Work Relative Value Units with Objective Markers of Operative Complexity.

OBJECTIVE: Relative value units (RVUs) form the backbone of health care service reimbursement calculation in the United States. However, it remains unclear how well RVUs align with objective measures of procedural complexity within neurosurgery. METHODS: The 2018 American College of Surgeons National Surgical Quality Improvement Program database was queried for neurosurgical procedures with >50 patients, using Current Procedural Terminology (CPT) codes. Length of stay (LOS), operative time, mortality, and readmission and reoperation rates were collected for each code and a univariate correlat…

Optimizing neurosurgical operating room schedule: Integrating machine learning models for operative time prediction and schedule design.

BACKGROUND: Estimating operative time with low accuracy and precision leads to suboptimal scheduling of available operating rooms (ORs) and waste of healthcare resources. We aimed to develop a predictive machine learning (ML) model for operative time using our institutional data to improve the scheduling of neurosurgical ORs, minimize waste, and maximize resource utilization. METHODS: We developed a predictive model using our institution's multicenter registry, which included diverse neurosurgical cases across three tertiary hospitals. We applied multiple ML techniques, including linear regre

Comparison of cefazolin administered as a continuous or intermittent infusion for prophylaxis of surgical site infections in adult patients undergoing cardiac or neurologic surgery.

In a propensity-matched cohort of adult cardiac or neurosurgical procedures (n = 1,342), infection was less frequent with continuous infusion (1.8%) versus intermittent cefazolin (2.4%), though the difference was statistically non-significant (-0.6%, 95% CI-2.3 to 1.1; p = 0.57). The 0% infection rate among cardiac cases receiving continuous cefazolin infusion warrants further investigation.

Dual-Biofluid Metabolomic Profiling by MALDI-MS Enables Noninvasive Diagnosis and Prognosis of Central Nervous System Infections.

Central nervous system (CNS) infections represent a common but serious complication following neurosurgical procedures. However, the metabolic alterations induced by infection remain incompletely understood, and noninvasive monitoring strategies remain urgently needed. In this study, we conducted a systematic metabolomic analysis of two biofluids─cerebrospinal fluid (CSF) and urine─obtained from patients undergoing neurosurgical procedures, using matrix-assisted laser desorption/ionization mass spectrometry (MALDI-MS). Metabolomic analysis of CSF revealed significant metabolic reprogramming as…

A surgeon beyond his era: neurosurgical contributions of Eugène-Louis Doyen (1859-1916).

Eugène-Louis Doyen studied medicine in Reims and Paris, further refining his surgical approach through experiences in Heidelberg, Würzburg, and Vienna. Recognized in French surgery for his innovations across thoracic, abdominal, and gynecological disciplines, his critical contributions to early neurosurgery, particularly trigeminal neuralgia treatment, remain significantly underexplored. Combining elements from the Hartley-Krause subtemporal route and the Rose sphenoidal route, he developed an advanced trigeminal ganglionectomy technique to minimize complications. Doyen introduced the electric

Fifteen-minute consultation: assessment of a child with suspected shunt problems.

Cerebrospinal fluid diversion procedures have saved more lives than any other neurosurgical procedure. However, they do have a unique set of complications which a paediatrician in a district general hospital may encounter. Timely recognition and appropriate referral to the neurosurgeon is vital in order to avoid serious consequences and to have a favourable outcome.
