Surgical correction of cryptotia with the square flap method: a preliminary report.

Cryptotia is the fourth most common congenital auricular deformity, and it is more prevalent among Asians. A number of techniques for its correction have been introduced, and relatively favourable results have been achieved. We describe our experience with a technique for its correction using the square flap method designed on the temporal scalp and skin over the auricle. From 2001 to 2006 a total of 19 examples in 12 patients were treated. Seven patients had bilateral, and five had unilateral, cryptotia. Traction was applied to the upper part of the auricle to draw it away from the scalp, and

A small vestibular schwannoma with preoperative facial palsy treated via retrolabyrinthine approach.

Facial nerve paralysis caused by a vestibular schwannoma is a rare but critical symptom; however, few reports have focused on preoperative facial nerve palsy from the viewpoint of treatment. In addition, its mechanism is not fully understood. We report a rare case of preoperative facial nerve palsy that improved after the surgical resection of a small vestibular schwannoma with hearing preservation. A small vestibular schwannoma within the internal auditory canal caused acute deafness and facial paralysis in a 51-year-old woman. Conservative treatment was initially administered, but severe hea

MEFA-Unet: Multi-scale feature extraction and fusion attentional unet for segmenting short process of incus in otologic microsurgical scenarios.

BACKGROUND: The application of convolutional neural networks (CNNs) in microsurgery has been limited. Current CNNs struggle to capture diverse semantic details across various scales and receptive fields, and they also face challenges in establishing meaningful connections between features detected by different receptive fields. Thus, they fail to deal with small objects and maintain accurate boundaries, especially in some complex microsurgical scenarios. METHODS: We propose a multi-scale feature extraction and fusion attentional Unet (MEFA-Unet) to address these limitations. This model employ

Online training for hearing implant surgery : A new approach to otological training.

OBJECTIVE: Education in microsurgery of the ear includes staged training to allow for mastering of the complex microsurgical procedures, particularly in the context of middle ear reconstruction and cochlear implantation. Traditional surgical training includes temporal bone preparations by cadaver dissection and supervised operating room practice. As these on-site trainings are limited, there is a need to broaden education facilities in an on-line format. Therefore, a first basic on-line training for otosurgery was developed. MATERIALS AND METHODS: The system consists of an artificial temporal

Video Learning of Surgical Procedures: A Randomized Comparison of Microscopic, 2- and 3-Dimensional Endoscopic Ear Surgery Techniques.

BACKGROUND: Video learning of surgical procedures helps trainees gain an initial understanding of the complex anatomy and the surgical procedure. Because no comparative studies have yet examined which microsurgical approach to the middle ear is most suitable for video learning, the authors investigated objective and subjective outcomes for medical trainees observing microscopic, 2-dimensional (2D) endoscopic, and 3- dimensional (3D) endoscopic ear surgeries. METHODS: Sixty-two medical students (min. 3rd year) from the University Hospital of Bern watched three standardized videos of a type I t

Curved adjustable fibre-optic diode laser in microscopic cholesteatoma surgery: description of use and review of the relevant literature.

OBJECTIVE: The use of lasers in cholesteatoma surgery is common and well accepted. The most commonly used laser fibres are straight and non-adjustable; these have several limitations. This paper describes the use of an alternative laser fibre. METHOD: This 'How I Do It' paper describes and illustrates the use of an alternative curved adjustable fibre-optic diode laser in microscopic cholesteatoma surgery. RESULTS: The curved, adjustable laser fibre allows accurate and atraumatic disease removal when the use of a straight laser fibre may be less effective or accurate. It reduces potential dam

Saline injection to determine the volume required for personalised patulous Eustachian tube augmentation with long-standing material.

OBJECTIVE: There is currently no general consensus on patulous Eustachian tube management. Injection of autologous fat, cartilage or hydroxylapatite has been described for Eustachian tube occlusion, with promising results. However, complete resolution of symptoms is not achieved in all cases. This could be connected to the amount of material injected into the surroundings of the Eustachian tube, as this greatly differs among existing studies. Identifying the appropriate volume of injected material could be challenging because anatomical conditions vary among patients, and there is always a ris

[The endoscopic anatomy of the middle ear (a dissection study)].

The objective of the present work was to study the specific endoscopic anatomical features of the middle ear using the dissected temporal bones with the intact tympanic membrane. The 18 cm long endoscopes 4 mm in diameter with a visual angle from 0 to 45 degrees in the combination with some other microinstruments, such as ear pincers, needles, curettes, elevators, and suction tubes, were used during the examination. It was shown that endomeato-transtympanic endosopy provides a panoramic view of almost all structures of the middle ear. After the resection of the posterior bone edge of 'annulus

Streamlined bilateral otologic surgery: how I do it.

Bilateral simultaneous otologic surgery is being performed more commonly among otologists. The benefits of performing bilateral simultaneous cochlear implants in the pediatric population, in particular, have become increasingly recognized as the safety and efficacy of this operation have been recognized in the literature. Here we present a streamlined method of performing bilateral simultaneous otologic surgery that emphasizes midline placement of facial nerve electrodes and a method of sterile preparation and draping that affords direct exposure to both ears at one time, without the need to t
