Hypoglossal nerve palsy secondary to cervical spine involvement in rheumatoid arthritis: clinical and radiological features.

Isolated hypoglossal nerve (HN) palsy has been reported in a variety of disorders involving the cervical spine and/or skull base, however, unilateral HN palsy caused by rheumatoid arthritis (RA) has rarely been reported. We report herein an uncommon case of isolated HN palsy secondary to RA cervical spine involvement: pannus formation at the C1-C2 articulation, atlanto-axial subluxation, as well as, erosion of the right occipital condyle, lateral mass and anterior arch of C1. Pulse therapy with methylprednisolone followed by maintenance therapy with prednisone resulted in dramatic improvement.…

Auricular chondritis associated with systemic joint and cartilage inflammation in a cat.

An adult, Japanese domestic cat presented with bilateral swelling, distortion, and erythema of ears and deformation of the limbs. Biopsy of the pinnae confirmed auricular chondritis. These lesions and the cat's general condition subsequently deteriorated, and the cat died. At necropsy, chondral changes were present in the pinnae, costae, larynx, trachea, and limbs. Histopathologically, these chondral tissues showed marked deformation with lymphocytic inflammation. The limb joint inflammation was associated with chondral erosion, deformation of subchondral bones with pannus, and thinning of can…

Rabbit model of septic arthritis.

Eighty-five rabbits were injected in one knee with Staphylococcus aureus in order to study the time-related changes in untreated septic arthritis up to 3 months. In the synovial membrane a severe release of lysosomal enzymes was observed. The activity was mainly located in and around lining cells and leucocytes in the pannus demonstrating increasing destructive characteristics. This resulted in marginal erosion and undermining of the cartilage border visible from Day 5 continuing gradually to total joint destruction after 5 weeks. The glycosaminoglycan depletion was observed at the surface of …

Chloroquine treatment and bone changes in rheumatoid arthritis.

The effect of chloroquine on bone and articular cartilage was studied in 28 rheumatoid arthritis patients. After the administration of chloroquine for a minimum of 6 months, a metatarsal bone specimen was taken at operation. The control material consisted of 24 operated patients treated in the same way, except that they had not received chloroquine. Histological investigation, in which pairs of slides were compared in a blind test, revealed that the changes in the articular cartilage in those given chloroquine were less marked. Moreover, a diminished tendency to pannus formation and a more vig…

Endoscopic endonasal odontoidectomy with C1 anterior arch preservation for spinal-cord compression in the elderly.

The endoscopic endonasal corridor for odontoidectomy (EEO) is a valuable route in expert's hand to manage craniovertebral junction (CVJ) pathologies of the midline. The possibility to preserve part of the C1 anterior arch along to an adequate preoperative planning, makes this option a viable choice especially in elderly patients with multiple comorbidities who cannot sustain a second procedure closed in time. Indeed, the current case shows the results of EEO in the presence of ventral compression due to degenerative non-rheumatoid retro-odontoid pannus in an 80-year-old male. Due to the absenc…

Altered immunoregulation in rheumatoid arthritis: the role of regulatory T cells and proinflammatory Th17 cells and therapeutic implications.

In recent years several studies investigated the role of T lymphocyte subpopulations in the pathogenesis of rheumatoid arthritis (RA). Pathogenic Th17 cells mediate pannus growth, osteoclastogenesis, and synovial neoangiogenesis; hence they are key players in the development of the disease. On the other hand, regulatory T (Treg) cells are a T cell subset whose peculiar function is to suppress autoreactive lymphocytes. The imbalance between Th17 and Treg cells has been identified as a crucial event in the pathogenesis of RA. In addition, the effects of currently employed RA therapeutic strategi…

Hypoglossal nerve palsy from cervical spine involvement in rheumatoid arthritis: 3 case reports.

Rheumatoid arthritis (RA) involvement of the cervical spine is a well-known but perhaps underappreciated phenomenon. Neurologic complications of this involvement include pain, myelopathy, and cranial nerve (CN) palsies. However, hypoglossal nerve palsy (CN XII) is rarely diagnosed. Mechanical nerve injury, either from vertical odontoid subluxation or pannus formation, is the suspected mechanism. We present 2 cases of hypoglossal nerve palsy attributed to cervical spine involvement of RA and 1 case of postoperative tongue weakness after cervical fusion in a patient with long-standing RA. These …

Severe persistent inclusion conjunctivitis in a young child.

A 14-month-old girl had inclusion conjunctivitis although there had been no signs or symptoms that required medical attention in her first year. By 2 1/2 years of age, the child had extensive pannus and corneal scarring that severely reduced vision. The infection was caused by a genital strain of Chlamydia trachomatis, TRIC type E, that was probably acquired at birth. Although genitally transmitted chlamydial strains normally cause a self-limiting inclusion conjunctivitis in areas where trachoma is not endemic, this case illustrates that they may occasionally cause severe trachoma. Possibly lo…

Extensive superior limbic keratoconjunctivitis in Graves' disease: case report and mini-review of the literature.

BACKGROUND: Superior limbic keratoconjunctivitis (SLK) is characterized as an inflammation of the superior bulbar conjunctiva with predomina … month. Total remission was then observed for 1 year. CONCLUSION: Extensive keratitis and vascular pannus in SLK is rarely reported. This form could be treated with topical steroids. Tapering treatment remains essential for long-term success.

MRI features of tuberculosis of peripheral joints.

The aim of this article is to present the magnetic resonance imaging (MRI) features of peripheral tubercular arthritis. The clinical presentation of peripheral tubercular arthritis is variable and simulates other chronic inflammatory arthritic disorders. MRI is a highly sensitive technique which demonstrates fine anatomical details and identifies the early changes of arthritis, which are not visible on radiographs. The MRI features of tubercular arthritis include synovitis, effusion, central and peripheral erosions, active and chronic pannus, abscess, bone chips and hypo-intense synovium. Thes…

Presence of human 65 kD heat shock protein (hsp) in inflamed joints and subcutaneous nodules of RA patients.

Monoclonal antibodies to the human homologue of the bacterial 65 kD heat shock protein (hsp) were used to investigate the tissue distribution of endogenous hsp 65 in normal versus rheumatoid synovial tissue, in subcutaneous nodules of patients with rheumatoid arthritis (RA) and in several instances of non-rheumatoid inflammation. A strong reactivity of the anti-hsp antibody was found in the cartilage-pannus junction in rheumatoid joints and in rheumatoid nodules, but not in normal joints or in normal or inflamed kidney or liver (irreversible graft rejection, chronic glomerulonephritis or prima…

Magnetic resonance imaging of palindromic rheumatism.

A 44-year-old man with intermittent asymmetric migratory oligoarthritis lasting the recent decade was admitted to our hospital. Considerable specific biomarkers for rheumatoid arthritis such as anti-agalactosyl IgG antibody are all negative. He was diagnosed as palindromic rheumatism (PR). Although hand X-rays showed no remarkable findings, hand magnetic resonance imaging (MRI) detected pannus and bone erosion. PR is defined as the disease characterized by short-lasting attacks of acute oligoarthritis, without radiographic changes. To our knowledge, the findings of MRI for PR have not been pre…

High tension abdominoplasty 2.0.

Traditionally, the primary goal of any abdominoplasty has always been to excise the central lower abdominal excess skin or pannus and plicate the abdominal fascia through a suprapubic incision. Unfortunately, this traditional abdominoplasty may often fall short of this goal: a scar that may ride too high; persistent skin and lipodystrophy at the pubis, thighs, flanks, and hips; and unfortunately a consistent incidence of midline skin necrosis or wound dehiscence. High lateral tension abdominoplasty addresses these shortfalls. It may be defined as a more complete treatment of the lower trunk ae…

[Role of adhesion molecules in the pathogenesis of collagen diseases].

Adhesion molecules play important roles and involve in many aspects of cell-cell or cell-extracellular matrix interactions in collagen diseases. In the present article, we describe the role of adhesion molecules in the pathogenesis of collagen diseases. Adhesion molecules on endothelial cells participate in leukocyte recruitment in collagen diseases. Adhesion molecules on RA synoviocytes which interact with MNCs and destroy chondrocytes by pannus formation. In PSS, adhesion molecules on fibroblasts play important roles to form fibrosis by interacting with MNCs or collagen fibrils. We further m…

Superficial stromal keratitis in the dog.

Superficial stromal keratitis or pannus is a syndrome of corneal, conjunctival and third eyelid inflammation. Superficial stromal keratitis mainly presents as a subepithelial corneal infiltration of vascular connective tissue, and usually arises from the lateral (temporal) limbal area. In some dogs perilimbal hyperaemia and third eyelid blepharitis can be present without corneal involvement. The most commonly affected breed of dog is the German Shepherd. Most cases of superficial stromal keratitis can be controlled with topical corticosteroids, and only rarely is cryosurgery or superficial ker…

Leprous synovitis. A study of fifty cases.

A study of 50 synovial biopsies of proved lepromatous patients with arthritis was carried out. Out of these 50 cases, 14 cases were suffering from lepra reaction and the histopathological study of the synovium in these 14 cases revealed the presence of only vasculitis and lymphocytic infiltration. In the remaining 36 cases, not associated with lepra reaction, the synovial lining showed hyperplasia and villous hypertrophy, and the synovial tissue showed congestion, pannus formation, the presence of macrophage granulomas and, in some cases, even intact acid-fast bacilli. Since there was no other…

Synovial Fluid as a Crucial Component of the Joint Microenvironment in Rheumatoid Arthritis.

Rheumatoid arthritis (RA) is a systemic autoimmune disease closely associated with synovial tissue proliferation, pannus formation in small joints such as the hands, wrists, and feet, cartilage destruction, and systemic complications such as pulmonary, cardiovascular, neurological, and skeletal muscle lesions, glucocorticoid-induced osteoporosis and infections. The importance of confirming the diagnosis and determining local activity is given to the study of synovial fluid. A deep understanding of the pathological process in the joint in RA, characterized by changes in autoreactive CD4+ T cell…

The bridge of the gut-joint axis: Gut microbial metabolites in rheumatoid arthritis.

Rheumatoid arthritis (RA) is an autoimmune disease characterized by joint destruction, synovitis, and pannus formation. Gut microbiota dysbiosis may exert direct pathogenic effects on gut homeostasis. It may trigger the host's innate immune system and activate the "gut-joint axis", which exacerbates the RA. However, although the importance of the gut microbiota in the development and progression of RA is widely recognized, the mechanisms regulating the interactions between the gut microbiota and the host immune system remain incompletely defined. In this review, we discuss the role of gut micr…

Intermittent malfunction and regurgitation of a mitral prosthetic valve due to entrapment by a residual subvalvular apparatus.

Intermittent malfunction is a rare but potentially serious complication of prosthetic heart valve (PHV) replacement. It may be observed in both mitral and aortic valve prosthesis and may lead to either paroxysmal obstruction or regurgitation, depending on the valve location and on the phase of the cardiac cycle during which the leaflet is entrapped. The most frequently encountered causes of intermittent valve malfunction are pannus formation, prosthetic valve thrombosis, vegetations, and remnants of subvalvular apparatus such as chordae tendineae and suture material. Intermittent severe regurg…
