A 33-month-old boy with bilateral corneal disease.

We report the case of a previously healthy 33-month-old boy with vitamin A deficiency (VAD) who presented with bilateral eye swelling, redness, and photophobia of 1 month's duration following multiple misdiagnoses. After confirming VAD with serum vitamin A level testing, he was treated with vitamin A, and his eye complaints improved. However, he continues to have a corneal scar and is currently undergoing amblyopia treatment.

Simultaneous presentation of symptomatic subependymomas in siblings: case reports and review.

Two young adult siblings independently developed similar neurological complaints that included headaches, photophobia, nausea, and intermittent lancinating facial pains. Magnetic resonance imaging revealed fourth ventricular lesions that required surgery in both patients. A pathological review revealed subependymomas with virtually identical histological features. The clinical features and common pathological findings of both patients suggest that familial subependymomas may have a maldevelopmental origin with genetic implications.

Nonhealing corneal defects due to herpes simplex in children.

Two young children with unilateral photophobia, redness, blepharospasm and corneal epithelial defects unresponsive to conservative therapy were seen between December 1982 and June 1983. Herpes simplex virus was established as the causal agent in both cases, only after the children had been examined under general anesthesia and appropriate cultures obtained. The difficulties in diagnosing herpetic keratitis in children and the treatment and long-term prognosis are discussed.

Report: A case report of Meige syndrome-like blepharospasm caused by ingestion of allopurinol.

One case of allopurinol-caused rare adverse reactions was reported. One male 51-year-old patient presented blurred vision, streaming eyes, photophobia and blepharospasm sequentially 1 week after oral administration of allopurinol. Complete remission was obtained after Botulinum toxin was locally injected. Allopurinol may cause Meige syndrome-like blepharospasm, the mechanism of which may be related to the inhibition of dopamine activity by affecting adenosine level in the brain.

A pain in the eye.

A 38-year-old woman presented with chronic left retro-orbital pain and photophobia. Palpation of the left occipital prominence revealed tenderness in the area of the greater occipital nerve and reproduced the eye pain. The diagnosis of cervicogenic headache was confirmed by symptom resolution following left greater occipital nerve blockade. The functional association between the greater occipital nerve and the trigeminal nerve provides a potential mechanism for this case of cervicogenic eye pain.

A Case of Reversible Cerebral Vasoconstriction Syndrome in a Healthy Adult Male.

Reversible cerebral vasoconstriction syndrome (RCVS) represents a potentially under-recognized cause of thunderclap headache in patients presenting to the ED. While a rarely made diagnosis in emergency medicine practice, RCVS may be as common as subarachnoid hemorrhage (SAH). RCVS typically presents as a sudden onset, excruciating headache that may be associated with nausea, vomiting, photophobia, or other features with overlap in the clinical presentation of both SAH and migraine headaches. As a result of historical features overlapping the presentation of SAH, particularly the rapidity of on…

Corticosteroid-induced glaucoma in pediatric patients with hematological malignancies.

BACKGROUND: Corticosteroids, especially dexamethasone, play a critical role in chemotherapy for pediatric hematological malignancies. We previously observed that patients with complaints of headache or photophobia during corticosteroid administration had high intraocular pressure (IOP). PROCEDURE: We measured IOP during corticosteroid administration in 15 patients with acute leukemia or lymphoma undergoing treatment at our institution from January 2016 to December 2018. IOP was measured by an ophthalmologist within seven days of the initiation of standard dose of corticosteroid, which was def…

Light-Related Cutaneous Symptoms of Erythropoietic Protoporphyria and Associations With Light Sensitivity Measurements.

IMPORTANCE: Erythropoietic protoporphyria (EPP) is a rare and underdiagnosed genetic disease characterized by painful sensitivity to light. A better understanding and characterization of its light-induced cutaneous symptoms may aid in the identification of EPP in patients. OBJECTIVES: To describe the cutaneous symptoms of erythropoietic protoporphyria (EPP) and to determine if these symptoms are associated with the degree of light sensitivity. DESIGN, SETTING, AND PARTICIPANTS: This was a cross-sectional study of adolescent and adult (≥15 years) patients with EPP across the US conducted by a

Complex regional pain syndrome and migraine: Clinical relationships and possible common aetiology.

BackgroundMigraine headache and complex regional pain syndrome share mechanisms, such as neuroinflammation, central sensitization and loss of inhibitory pain controls, that could provoke or exacerbate symptoms in both disorders. In the present study, it was hypothesized that headaches would worsen after the onset of complex regional pain syndrome and that limb pain would be more severe in patients with co-morbid headaches than in patients who remained headache-free. Notably, complex regional pain syndrome is associated with ipsilateral cranial symptoms such as photophobia and forehead hyperalg…

Investigation of light-induced lacrimation and pupillary responses in episodic migraine.

The purpose of this pilot study was to investigate the light-induced pupillary and lacrimation responses mediated by intrinsically photosens … on following blue stimulation in both groups. Our results show that participants with self-reported photophobia have normal ipRGC-driven responses, suggesting that photophobia and pupillary function may be mediated by distinct ipRGC circuits. The positive correlation between melanopsin-driven pupillary responses and light-induced lacrimation suggests the affe …
