A Fatal Case of Fulminant Panton-Valentine Leukocidin-Positive Methicillin-Resistant Staphylococcus aureus Necrotising Pneumonia in a Previously Healthy Adolescent: A Case Report.

Panton-Valentine leukocidin (PVL)-producing Staphylococcus aureus can cause severe necrotising pneumonia in previously healthy individuals. We report the case of a 17-year-old previously well adolescent who presented with a short prodrome of influenza-like symptoms and rapidly deteriorated into severe respiratory failure and septic shock. Despite early escalation of care, broad-spectrum antimicrobial therapy, and intensive supportive management, the patient developed bilateral pneumothoraces, refractory hypoxaemia, and multiorgan dysfunction. Blood cultures and respiratory samples confirmed me

Necrotizing pneumonia and septic shock caused by Burkholderia cepacia in an immunocompetent adult.

We report the case of an immunocompetent adult presenting with acute febrile illness during a dengue outbreak in the Peruvian Amazon. The patient developed rapidly progressive respiratory compromise and hemodynamic instability, requiring admission to the intensive care unit for management of septic shock, necrotizing pneumonia, and multiorgan dysfunction. Despite full supportive care, he experienced refractory cardiorespiratory failure and died within 24 h of admission. Blood cultures later identified Burkholderia cepacia, and the association of bacteremia with necrotizing pulmonary involvemen

Multimodal Management of Coexisting Atlantoaxial Subluxation and Spinal Stenosis in an Older Adult: A Case Report and Literature Review.

This case report investigates the diagnostic and therapeutic challenges caused by the coexistence of atlantoaxial subluxation and spinal stenosis in a 70-year-old male patient with chronic progressive numbness in both hands. A detailed assessment showed that the patient's symptoms were primarily caused by spinal stenosis, not atlantoaxial subluxation. Considering the patient's age and preference for nonsurgical treatment, a conservative chiropractic care plan was implemented, significantly improving his symptoms and quality of life. This case highlights the potential benefit of conservative ch

Classification of Presbyopia by Severity.

There are close to two billion individuals globally living with presbyopia. In spite of its ubiquitous and progressive nature, there is no widely accepted, formal guideline or consensus statement on the classification of presbyopia by degree of severity. A panel of leading eye care professionals representing both optometrists and ophthalmologists convened virtually to discuss and document their combined assessments from the body of literature and clinical practice expertise in this commentary. In light of emerging therapies, classifying presbyopia by mild, moderate, or advanced severity may he

Vitamin E for antipsychotic-induced tardive dyskinesia.

BACKGROUND: Antipsychotic (neuroleptic) medication is used extensively to treat people with chronic mental illnesses. Its use, however, is associated with adverse effects, including movement disorders such as tardive dyskinesia (TD) - a problem often seen as repetitive involuntary movements around the mouth and face. Vitamin E has been proposed as a treatment to prevent or decrease TD. OBJECTIVES: The primary objective was to determine the clinical effects of vitamin E in people with schizophrenia or other chronic mental illness who had developed antipsychotic-induced TD.The secondary objecti

An integrated care pathway improves quality of life in Primary Biliary Cirrhosis.

BACKGROUND: Clinical management of the chronic autoimmune liver disease, Primary Biliary Cirrhosis (PBC) involves addressing the underlying liver disease and a range of symptoms independent of liver disease severity. We have formally explored how these two perspectives of chronic disease management can be combined into a clinic consultation and impact upon quality of life (QOL) in PBC. AIMS: To develop and implement the first Integrated Care Pathway (ICP) for the management of liver disease progression and symptom management in PBC. METHODS: Process mapping of current practice by a multidisc

Reexpansion Pulmonary Edema Following Palliative Thoracentesis in a Home Care Setting.

BACKGROUND: In Germany, specialist palliative home care provides care for patients in the home setting. This also includes interventions such as thoracentesis. However, complications such as reexpansion pulmonary edema (REPE), a rare but potentially life-threatening condition, require prompt recognition and management. CASE PRESENTATION: An 86-year-old woman with metastatic breast cancer under palliative home care developed progressive dyspnea. Despite pharmacological treatment, she required right-sided thoracentesis, yielding 1350 mL of pleural fluid. Within one hour, she experienced acute r

Harm reduction nursing and the path toward developing best practice: Lessons from caring for people with xylazine-associated wounds in Philadelphia, PA.

Xylazine-associated wounds are a distinct, novel clinical entity characterized by co-occurrence with substance use, progressive necrosis of skin, muscle, tendon, and bone, and slow healing. In Philadelphia, the specter of limb loss, stigma, and shame has hung over hospital-based care for xylazine-associated wounds among people who use drugs (PWUD) and kept many people away from engaging in care. Continued engagement in harm reduction wound care nursing, however, offers an opportunity for PWUD to address their wounds and their fears with members of the medical world. In the absence of establish

Global disparities in arrhythmia care: Mind the gap.

Delivery of comprehensive arrhythmia care requires the simultaneous presence of many resources. These include complex hospital infrastructure, expensive implantable equipment, and expert personnel. In many low- and middle-income countries (LMICs), at least 1 of these components is often missing, resulting in a gap between the demand for arrhythmia care and the capacity to supply care. In addition to this treatment gap, there exists a training gap, as many clinicians in LMICs have limited access to formal training in cardiac electrophysiology. Given the progressive increase in the burden of car

Early Calciphylaxis Mimicking Infection in a Patient With End-Stage Renal Disease and Sarcoidosis: A Diagnostic and Wound Management Challenge.

Calciphylaxis is a rare, life-threatening disorder primarily affecting patients with end-stage renal disease (ESRD) on dialysis. Early lesions can mimic infection, leading to delays in diagnosis and wound-directed care. We describe an atypical early presentation of calciphylaxis in a patient with ESRD and sarcoidosis, initially misdiagnosed as cellulitis. Despite early sodium thiosulfate therapy and metabolic control, the patient developed progressive necrotic wounds. This case underscores that calciphylaxis can progress despite controlled mineral metabolism and standard therapy, particularly

Central diabetes insipidus following a sports-related concussion: a case report.

A 24-year-old female swimmer presented to a sports medicine clinic with complaints of frequent urination and increased thirst. The patient admitted to progressive worsening of her symptoms over a 4-year period since suffering a concussion. A water deprivation test, antidiuretic hormone level, and diamino-8-D-arginine vasopressin challenge were completed, and the patient was diagnosed with persistent central diabetes insipidus. As concussion awareness increases, health care professionals will be faced with treatment of post-concussive patients more often. The aim of this case report is to incre

An unusual presentation of cardiomyopathy in a patient with microcephaly-cardiomyopathy syndrome.

We report the case of a 15-year-old male patient with microcephaly, dilated cardiomyopathy, mental retardation, secondary hypopituitarism, and minor dysmorphic features: downward- slanting palpebral fissures, narrow palate, small and low-set ears, fifth finger clinodactyly, sandal gaps on both feet, and anal stenosis. He was admitted to the pediatric intensive care unit with signs of progressive cardiac failure. Lethal outcome occurred 25 days after admission. Most clinical features of the patient were similar to those of the four previously reported patients with microcephaly-dilated cardiomy

Difficulties of delayed treatment of craniosynostosis in a patient with Crouzon, increased intracranial pressure, and papilledema.

Crouzon syndrome is an autosomal dominant disorder characterized by cranial synostosis, hypertelorism, orbital proptosis, parrot-beaked nose, short upper lip, hypoplastic maxilla, and a relative mandibular prognathism, without extremity involvement. Surgical intervention should occur at the onset of progressive craniosynostosis to treat or to prevent intracranial hypertension and visual impairment. Throughout developing countries, early treatment is often not a viable option. Often, the only option for treatment is through humanitarian missions. Appropriate preselection of surgical patients is

[Potentially inappropriate medication in older adults with palliative care at home].

INTRODUCTION: Older adults with advanced chronic diseases and palliative care needs are more exposed to polypharmacy and use of potentially inappropriate medication, which generates a high risk of adverse events and impaired quality of life. The objective of this study was to describe the frequency of potentially inappropriate medication use among older adults with palliative care needs receiving home care services after hospital discharge. METHODS: Observational cross-sectional study of pharmacy dispensing and electronic health records, of older adults in a home care system and with palliati

Adjunctive hyperbaric oxygen therapy for chronic diabetic foot ulcer unresponsive to standard care: A case report.

BACKGROUND: Diabetic foot ulcers (DFUs) are a serious complication of diabetes mellitus and are often challenging to treat, particularly in patients who fail to respond to standard wound care. Adjunctive therapies such as hyperbaric oxygen therapy (HBOT) have shown potential in promoting wound healing in chronic cases. CASE PRESENTATION: A 66-year-old male with a 10-year history of type 2 diabetes mellitus presented with a chronic, non-healing DFU on the right foot. Despite oral antibiotic therapy and conventional wound management, the ulcer demonstrated progressive necrosis, persistent infec
