PURPOSE: Persistent pain in referred areas and voiding dysfunction are characteristic symptoms of chronic abacterial prostatitis. Since referred pain from visceral organs is considered a neurological event, it appeared reasonable to hypothesize that the persistent pain associated with prostatitis might also be explained by neural mechanisms. Neurogenic plasma extravasation and c-fos expression in the spinal cord, after chemical irritation of the rat prostate, was identified as a method to investigate the neurogenic aspect of prostatic inflammation.
MATERIALS AND METHODS: The distribution of p…

BACKGROUND: Subgrouping of migraine patients according to the pain response to manual palpation of the upper cervical spine has been recentl … ation of C1 and C2, indicating that people with a higher impairment are more likely to respond with referred pain during palpation of C2.
CONCLUSION: Statistical analysis confirms that results from the palpation of the cervical segments C1 and C2 in migraine patients can be combined. IRT analysis confirmed the ordinal pattern of the pain response and showed th …

A 74-year-old woman presented with severe right ear pain associated with bleeding of 2 months' duration and vertigo. She was otherwise fit and well apart from arthritis. On examination there was dried blood overlying the right ear drum and very mild inflammation of the canal but no obvious acute infection. The dried blood was removed in subsequent clinic visits. The ear drum was intact and there was no evidence of infection. However, the patient still had persistent pain in the right ear. A diagnosis of referred otalgia was made. Nose, throat, neck, temporo-mandibular joint, oral examination a

OBJECTIVE: To review the disposition and outcome of patients with back pain seen in a neurology clinic, and determine the value of the neurologist's input.
BACKGROUND: Back pain is a common cause of referral to the neurology clinic, but it is not clear how much the neurologist contributes to the care of such patients.
DESIGN/METHODS: We reviewed all new patients and new consultations seen for the following reasons: low back pain; 'sciatica'; 'back and leg pain', and 'spinal stenosis'. Patients were evaluated by a single attending neurologist at a general neurology clinic over a 2-year period

BACKGROUND: The incidence of low back pain, a leading cause of global disability, has increased significantly. Spinal cord stimulation (SCS) is US Food and Drug Administration-approved for treating intractable back pain, particularly in patients with prior surgical interventions.
CASE REPORT: A 43-year-old woman was referred for SCS implantation following 3 prior lumbar spinal surgeries with continued, right-sided radicular back pain. Additional diagnostic imaging was obtained, and a magnetic resonance imaging with and without contrast revealed a recurrent disc despite 3 lumbar microdiscectom

Acute scrotal pain in a child is a urological emergency. Surgeons should be involved early and exclude torsion promptly, often by surgical exploration. This short report compares the time taken by different medical practitioners [Emergency Department (ED) doctors and surgeons] to assess children presenting to the ED with acute scrotal pain. A retrospective observational study was carried out in a district general hospital ED over a 2-year period. A total of 205 children presented with testicular pain, 48 of whom were clinically diagnosed with torsion in the ED. Surgeons reviewed patients with

The patterns of referral for 600 consecutive children and adolescents under the age of 17 years were analysed and the symptoms were compared with the diagnoses. 40% were referred because of foot problems, 20% had mobility problems and 20% had pain in the back or extremities. Out of the 240 patients with foot problems, these could be confirmed in 44%. 30% had merely short muscles and 7% had, in addition, spasticity. 70% out of the 110 patients referred on account of flat foot had too short achilles tendons. 50% out of the 600 patients referred had short achilles tendons and/or hamstring muscles

» Lumbar spine pathology is a known cause of referred pain to the lower extremities and should be investigated as a possible source of knee pain, especially with patients in their sixth decade.» While primary knee pathology is common, spinal pathology should always be considered in older patients presenting with knee pain, especially in atraumatic cases where knee imaging does not correlate with complaints or examination findings.» Lumbar (L) 3-4 pathology is most commonly affected in referred knee pain, with the 2 most common pathologies being spinal stenosis and disc herniation.» If knee rad…

BACKGROUND: Studies evaluating the efficacy of treatments for recurrent abdominal pain (RAP) in children have used a wide range of methods, causing difficulty in the comparison of results. An expert panel on functional gastrointestinal disorders recently made recommendations regarding the standardization of study methods for childhood RAP, but many of their recommendations remain untested or lack supportive evidence.
METHODS: During completion of a pilot study and randomized controlled trial for childhood RAP, baseline data were collected regarding the child and parent reports of abdominal pa

Chest pain accounts for much of the rising numbers of emergency admissions, but in-patient assessment is not necessarily the best way of dealing with these patients. We ran a 'rapid-assessment chest pain clinic' to provide an alternative route of assessment, and audited its outcome. General practitioners referred patients with recent-onset chest pain, increasing chest pain, chest pain at rest, or other chest pain of concern, on the understanding that they would be seen within 24 h. During 8 1/2 months, 334 patients were referred and 317 patients were seen, most of whom had exercise electrocard

Pain around the face and neck is common. This pain may be referred from a different area. Because patients usually present with pain and discomfort in a specific location such as the ear, nose, face, throat, or neck, this article discusses separately the innervation of each region and the most frequent causes of pain in each of these areas.

Sciatica has classically been associated with irritation of the sciatic nerve by the vertebral disc and consequent inflammation. Some authors suggest that active trigger points in the gluteus minimus muscle can refer pain in similar way to sciatica. Trigger point diagnosis is based on Travel and Simons criteria, but referred pain and twitch response are significant confirmatory signs of the diagnostic criteria. Although vasoconstriction in the area of a latent trigger point has been demonstrated, the vasomotor reaction of active trigger points has not been examined. We report the case of a 22-…

This case demonstrates that shoulder pain can be the sole manifestation of a benign gastric ulcer. Thus benign gastric ulcer should be considered in the differential diagnosis of a referred shoulder pain.

OBJECTIVE: To assess the perceived efficacy of pain clinics in the rehabilitation of injured workers among four groups of professionals.
DESIGN: A questionnaire was given to 351 subjects representing four professional groups: physicians, vocational rehabilitation counselors, staff at pain clinics, and employees of a workers compensation program. Subjects rated the effectiveness of pain clinics in eight specific functions, estimated the percentage of workers who return to work after pain clinic treatment, and indicated how soon after injury a worker should be referred to a pain clinic.

PURPOSE/OBJECTIVE: This study investigated the type and recency of the pain experience recalled (i.e., pain referent) by a healthy group and a chronic/recurrent pain group when they responded to the trait version of the Pain Catastrophizing Scale (PCS). We also aimed to disentangle the influence of trait personality factors and state oriented pain-related cognitive processes on PCS scores. Research Method/Design: A cross-sectional online survey was administered. Participants who reported chronic/recurrent pain comprised the pain group (N = 153) and those not reporting a pain condition comprise
