Humeral greater tuberosity osteolysis as a complication of intraosseous calcification migration: natural history depicted by imaging.

Migration of calcification within the bone leading to greater tuberosity osteolysis is a peculiar complication of the calcifying tendinitis of the rotator cuff. The case of a 38-year-old woman complaining of right shoulder pain, which had been going on for one year, is hereby described. The evolution of the infraspinatus tendon calcifying tendinitis leading to osteolysis of the greater tuberosity of the humerus is depicted by imaging and, particularly, by the MR and CT features changing over time. In this paper we focus on the importance of both MR and CT exams in the diagnostic process of the

Editorial Commentary: Superior Capsular Reconstruction Works Biomechanically but Should Be Used Selectively for Genuinely Irreparable Tears.

Superior capsular reconstruction (SCR) has undergone extensive biomechanical and clinical study. Systematic review of biomechanical studies has shown that SCR functions to depress the humeral head and improve the glenohumeral register, which provides proof of concept. Clinical studies have shown the short-term effectiveness of SCR, but this is influenced by graft type and thickness, as well as surgical technique. These studies are mostly flawed because follow-up is brief, postoperative imaging is often lacking, and the results may not be generalizable. Multiple systematic reviews pooling the r

Increased T2 signal intensity in the distal clavicle does not justify acromioclavicular resection arthroplasty during rotator cuff repair.

INTRODUCTION: Acromioclavicular (AC) arthropathy can contribute to shoulder pain; it can be treated surgically by distal clavicle resection (DCR). The aim of this study was to determine whether increased T2 signal intensity in the clavicle on MRI is an argument in favour of AC resection arthroplasty by DCR during rotator cuff repair. METHODS: The MRI images from 107 shoulders in 107 patients were analysed. We looked for statistical relationships and correlations between shoulders with T2 hyperintensity (HI+) and those without (HI-) before surgery and then in shoulders with T2 hyperintensity (

Editorial Commentary: Choose Wisely: Rotator Cuff All-Suture Anchors.

The defining feature of a surgeon's professional practice is clinical decision making coupled with surgical execution. Surgeons' decisions are continually being scrutinized, and clinicians are trained to consult and interpret the peer-reviewed literature in formulating a treatment plan. An important decision in the surgical episode of care is which implant(s) will be used, and surgeons should feel comfortable defending their implant choices. Understanding the finer details of a bench study is important in formulating a clinical opinion of a biomechanical investigation of surgical implants. The

Arthroscopic isolated long head of biceps tenotomy in patients with degenerative rotator cuff tears: mid-term clinical results and prognostic factors.

INTRODUCTION: The long head of biceps tendon is frequently involved in degenerative rotator cuff tears. Therefore, this study explored the clinical results of an isolated biceps tenotomy and identified prognostic factors for improvement in pain and function. MATERIALS AND METHODS: Between 2008 and 2017, an arthroscopic isolated biceps tenotomy was performed on 64 patients with a degenerative rotator cuff tear (> 65 years). Primary outcome was patient-perceived improvement in pain and function. Potential prognostic factors for improvement in pain and function were identified. RESULTS: A perce

Biomechanical Effects of Fiber Patch Augmentation on Rotator Cuff Repairs.

Rotator cuff repairs are a common orthopedic procedure that have a relatively high failure rate when tendon quality is poor. New biotechnology exists that can make tendons with poor quality more amenable to repair. This study examined the biomechanical effects of augmenting a rotator cuff repair with an absorbable fiber patch. Six human cadaveric supraspinatus tendons were prepared into 1-cm wide strips and then repaired to the rotator cuff footprint using a titanium anchor and a single mattress suture. Each shoulder underwent repair with and without a fiber patch. The specimens were subjected

Finite element analysis of the rotator cuff: A systematic review.

BACKGROUND: Finite element modeling serves as a promising tool for investigating underlying rotator cuff biomechanics and pathology. However, there are currently no concrete guidelines for reporting in finite element model studies. This has compromised the reliability, validity, and reproducibility of literature due to omission of pertinent items within publications. Recently a Finite Element Model Grading Procedure has been proposed as a reporting guideline for model developers. The aim of this study was to conduct a systematic review of rotator cuff focused finite element models and characte

Editorial Commentary: Single Versus Double-Row Abductor Tendon Repair: Is the Juice Worth the Squeeze?

Anatomical understanding of the peritrochanteric space and abductor insertions has been well documented in the literature. Extrapolating this knowledge to perform a "successful" abductor tendon repair has led to controversy over the best method to achieve a durable, stable repair and improve patient outcomes. I feel that the debate in the hip over single- versus double-row fixation is only beginning and we are in for a ride to uncover the best method, as we have with the shoulder over the last 15 years. What is best? Single or double row?

[Indications and techniques of arthroscopic anterior and lateral acromioplasty].

OBJECTIVE: Arthroscopic anterior acromioplasty (AAAP) for treatment of symptomatic subacromial spur. Arthroscopic lateral acromioplasty (ALAP) to reduce a pathological critical shoulder angle (CSA) and prevent rotator cuff re-tear after reconstruction. INDICATIONS: AAAP is indicated for acromial impingement due to an anterolateral acromial spur with or without bursa-sided rotator cuff lesion. ALAP is indicated concomitant to arthroscopic rotator cuff repair if the CSA is pathologically increased. RELATIVE CONTRAINDICATIONS: Irreparable rotator cuff tear with acetabularization of the acromion

Editorial Commentary: Superior Capsule Reconstruction: Grafts for Superior Capsular Reconstruction Must Be Thick and Stiff.

To achieve a good functional outcome, including minimal graft tear, after superior capsule reconstruction, surgeons should carefully compare the biomechanical properties of all graft materials under consideration. In particular, graft stiffness affects postoperative stability in the glenohumeral joint, acromiohumeral distance, and graft healing. In my experience, a sufficiently stiff fascia lata graft is 6- to 8-mm thick and includes suture reinforcement and/or an augmentation patch. I believe that augmenting acellular dermal allografts in a similar way will decrease the incidence of graft elo

Arthroscopic rotator cuff repair using a single-row of triple-loaded suture anchors with a modified suture configuration.

Achieving an adequate restoration of the muscle-tendon-bone unit and the anatomical footprint is essential for a successful outcome in open and arthroscopic rotator cuff repair. The described suture grasping technique using triple-loaded suture anchors might combine high initial fixation strength with good footprint coverage. It describes two mattress' stitches medial at the articular margin of the tendon. Additionally, a third mattress stitch is performed laterally to increase footprint contact and avoid dog-ear deformity. The triple-mattress repair is easy to perform and might be a good alte

Arthroscopic approach to massive rotator cuff tears.

The treatment of a patient with a painful massive rotator cuff tear can be challenging. Massive tears exceed 5 cm in length and involve partial or complete tearing of three tendons. The arthroscopic approach provides a minimally invasive technique to evaluate the tear, mobilize and repair tendons, and improve symptoms without disruption of the deltoid. The arthroscopic technique can create a low-tension repair. The reduction of postoperative motion provides the best environment for tendon-to-bone reattachment. Procedures that have reduced rates of adhesion formation allow for this protection w

US of the shoulder: rotator cuff and non-rotator cuff disorders.

Ultrasonography (US) has been shown to be an effective imaging modality in the evaluation of both rotator cuff and non-rotator cuff disorders, usually serving in a complementary role to magnetic resonance imaging of the shoulder. US technique for shoulder examination depends on patient positioning, scanning protocol for every tendon and anatomic part, and dynamic imaging. The primary US signs for rotator cuff supraspinatus tendon tears are tendon nonvisualization for complete tears, focal tendon defect for full-thickness tears, a hypoechoic defect of the articular side of the tendon for an art

Treatment of rotator cuff impingement.

The author performed 497 shoulder decompression procedures between 1978 and 1989. Of these procedures, 252 (51%) had tears of the rotator cuff that were repaired concomitantly. The results were successful in over 90% of the cases. It is suggested that articular-surface tears or intratendinous tears are the possible cause of some of the failures of the open procedure. The findings also suggest that the use of arthroscopy or magnetic resonance imaging may be indicated in selected cases. However, for the appropriate patient, open shoulder decompression continues to be an excellent treatment.

Baseball and softball injuries.

Baseball and softball injuries can be a result of both acute and overuse injuries. Soft tissue injuries include contusions, abrasions, and lacerations. Return to play is allowed when risk of further injury is minimized. Common shoulder injuries include those to the rotator cuff, biceps tendon, and glenoid labrum. Elbow injuries are common in baseball and softball and include medial epicondylitis, ulnar collateral ligament injury, and osteochondritis dissecans. Typically conservative treatment with relative rest, medication, and a rehabilitation program will allow return to play. Surgical inter
