Sotalol, a noncardioselective beta-adrenergic blocking agent without partial agonist activity, has class-3 antiarrhythmic effects. Twenty-nine patients with supraventricular tachyarrhythmias were treated on 31 occasions to determine the effective therapeutic dosage and optimal method of administration. Fifty percent of patients with paroxysmal supraventricular tachycardia, 33 percent of those with atrial flutter, and 20 percent with atrial fibrillation were restored to sinus rhythm by bolus administration; and when sotalol was given by continuous infusion, sinus rhythm was obtained in 100 perc

Initial treatment of atrial fibrillation often involves pharmacological therapy to control ventricular response. While verapamil is usually safe and effective when used for this purpose, we report a proarrhythmic response. In this report a 30-year-old female presented with palpitations associated with atrial fibrillation and a ventricular response of 145 beats/min. Soon after she was given 5 mg of intravenous verapamil her ECG documented a regular wide QRS tachycardia at 290 beats/min. After 7 seconds the rhythm returned to an irregularly irregular narrow QRS tachycardia at 125-150 beats/min.

BACKGROUND: This multicenter study evaluated experience with implantable cardioverter defibrillators (ICD) as a bridge to orthotopic heart transplantation (OHT) in children.
METHODS: The application of ICD therapy continues to expand in pediatric populations, due in part to improved technology and new indications, including the prevention of sudden death while awaiting OHT.
METHODS: We performed a retrospective review of ICD databases at 9 pediatric transplant centers.
RESULTS: Twenty-eight patients (16 males) underwent implantation or had a preexisting ICD while awaiting OHT between 1990 a

Radiofrequency catheter ablation has become the treatment of choice for paroxysmal supraventricular tachycardia involving dual atrioventricular nodes or an accessory pathway. For reentry confined to the atrium where the arrhythmia itself or the ventricular response cannot be controlled with drugs, catheter ablation of the His bundle is a treatment option, but requires implantation of a permanent pacemaker and does not restore normal rhythm. In the atria, important anatomic obstacles, such as the great veins and the ostium of the coronary sinus, interrupt the normal arrangement of myocardial fi

A 15 year old girl who underwent surgical correction of ventricular septal defect and patent ductus arteriosus ligation in childhood presented with atrial tachycardia of crista terminalis origin and counterclockwise atrial flutter. She also had associated interruption of inferior vena cava which continued as azygous vein and left superior vena cava which drained via coronary sinus into the right atrium. She underwent radiofrequency ablation of both the tachycardias via internal jugular vein and azygous vein approach using 3D electroanatomical mapping system.

A 14-year-old boy with repaired transposition of the great arteries and ventricular septal defect presented with atrial flutter and severe congestive heart failure. Despite successful cardioversion and optimal medical therapy, the patient deteriorated and was supported with extracorporeal membrane oxygenation (ECMO). Two days after initiating ECMO support, we implanted the DeBakey VAD Child ventricular assist device (MicroMed Technology, Inc., Houston, TX) under the Humanitarian Device Exemption program. Later, he was able to pursue normal daily activities including physical rehabilitation and

Amiodarone is used in the treatment of previously drug-resistant supraventricular and ventricular arrhythmias. We report our experience with amiodarone in 8 patients. Five patients had paroxysmal atrial flutter, one had paroxysmal atrial fibrillation, one had supraventricular tachycardia, and one ventricular tachycardia. Considerable improvement, both objectively and subjectively, was observed in all patients. Side effects were as follows: all patients had corneal microdeposits, one developed left bundle branch block which resolved on stopping amiodarone, and one reported constipation and abdo

AIMS: Catheter ablation is a treatment of first choice for many arrhythmias. Data in geriatric patients are still limited, mostly focusing on single arrhythmia types. The aim of the study was to investigate characteristics of contemporary ablation therapy in the very elderly, focusing on patient characteristics, arrhythmia spectrum, and outcome in a large cohort.
METHODS AND RESULTS: We studied 131 consecutive patients aged 80 and older (mean age 83 +/- 3) undergoing ablation for any indication from 1998 until 2004. Sixty-eight patients (52%) had structural heart disease. Most common indicati

BACKGROUND: The long-term success of intracardiac repair of tetralogy of Fallot is hampered by the occurrence of arrhythmias. Numerous studies have stressed the potential role of ventricular arrhythmias. However, the importance of other arrhythmias in the morbidity of these patients appears to be underestimated. Furthermore, most follow-up studies have been limited to children or adolescents, whereas many patients have reached adulthood after earlier repair of tetralogy of Fallot. The aim of the present study was to determine the incidence of atrial fibrillation, atrial flutter, and other supr

Cardiomyopathy is a heterogeneous group of diseases of heart muscle accompanied with impaired cardiac function. Tachycardia-induced cardiomyopathy (TIC) is caused by prolonged tachycardia leading to dilatation and systolic dysfunction with clinical manifestation of heart failure. This state is reversible after normalization of heart rate. The diagnosis is usually made retrospectively after normalization of heart rate and recovery of left ventricular function (LVF). More than 100 years after the first documented case (described in 1913 in a young patient with atrial fibrillation and symptoms of

Although rare, sudden cardiac death does occur in British military personnel. In the majority of cases, the cause is considered to be a malignant ventricular tachyarrhythmia, which can be precipitated by a number of underlying pathologies. Conversely, a tachyarrhythmia may have a more benign and treatable cause, yet the initial clinical symptoms may be similar, making differentiation difficult. This is an overview of the mechanisms underlying the initiation and propagation of arrhythmias and the various pathological conditions that predispose to arrhythmia genesis, classified according to whic

Landiolol, an ultra-short-acting β1-selective blocker, is more effective in controlling heart rate compared with digoxin in patients with atrial tachyarrhythmias and left ventricular dysfunction. However, the effect of atrial tachyarrhythmia type on the effectiveness of landiolol remains unknown. Hence, this study aimed to evaluate the effectiveness of landiolol in patients with atrial fibrillation (AF) and atrial tachycardia (AT), not limited to those with heart failure with a reduced ejection fraction. To this end, we evaluated the efficacy and safety of landiolol in managing tachycardiac AF

A 55-year-old lady with non-ischemic cardiomyopathy (NICM) was referred for multiple implantable cardioverter defibrillator (ICD) shocks. Stored electrograms (EGM) revealed atrial flutter (AFL) with A > V. Morphology match was good and RR-intervals were irregular. Despite all these, the dual-chamber-ICD (Abbott medical) classified this as ventricular tachycardia (VT-2) via V > A algorithm where it did not analyze morphology/stability and delivered therapy. Anti-tachycardia-pacing (ATP) was delivered which induced a true VT (rate in VF-zone) and immediate shock was delivered. It was hence appro

We describe the case of 2-year-old baby with compound heterozygosity for paternal and maternal alleles mutation of α-subunit of the cardiac sodium channel (SCN5A), sinus node dysfunction, atrial flutter recurrences, and drug induced long-QT syndrome. In this setting, we chose at first to perform linear ablation of cavotricuspid isthmus resulting in a bidirectional isthmus block. As a second step, we decided to implant a miniaturized loop recorder that, with a minimally invasive procedure, permits us to follow the development of the disease in order to define the future strategy. After 8 months

BACKGROUND: Pharmacological management of arrhythmias is not curative, is sometimes difficult, and may be associated with significant morbidity.
OBJECTIVE: This article discusses the place of catheter ablation in the treatment of arrhythmias.
DISCUSSION: Supraventricular and typical atrial flutter can be cured with catheter ablation in approximately 95% of cases with very low complication rates. Catheter ablation can successfully restore and maintain sinus rhythm in patients with atrial fibrillation (AF) and should be considered in symptomatic patients not controlled on medical therapy befor

A 42-year-old woman was admitted to our hospital because of anterior chest discomfort on exercise. She had already been diagnosed as having a complete form of Behçet's disease. On auscultation, there was a grade 2/6 diastolic regurgitant murmur along the left sternal border. Echocardiogram and pulse doppler led to the diagnosis of aortic regurgitation and mitral fluttering in diastole. Left ventriculography disclosed an enlarged left ventricular chamber, but the ejection fraction was 60% within the normal range. Aortography confirmed the diagnosis of aortic regurgitation (III.) and dilatation

Vagal stimulation manoeuvres are currently used in clinical practice both for diagnostic and therapeutic purposes. Such manoeuvres are not, however, without risk. The Authors describe the case of a patient in whom, in the past, a Lown-Ganong-Levine syndrome had been diagnosed. During a vagal stimulation, performed because of a suspected paroxysmal supraventricular tachycardia crisis, a sudden increase of ventricular rate, up to 280 b/min, occurred. At the end of this paroxysm, the presence of an atrial flutter was ascertained. The high ventricular rate was thus attributed to an 1:1 A-V conduct
