Functional Electrical Stimulation Changes Muscle Oxygenation in Patients with Chronic Obstructive Pulmonary Disease During Moderate-Intensity Exercise: A Secondary Analysis.

We previously showed that functional electrical stimulation during cycle ergometry (FES-cycling) increased oxygen consumption (VO2), indicating that metabolism during exercise was increased. However, the effects on muscle oxygenation have never been studied. The aim of this secondary analysis was to analyse changes in muscle oxygenation during an FES-cycling session. Eight patients with chronic obstructive pulmonary disease who were participating in a pulmonary rehabilitation programme were enrolled. Each participant carried out 30 minutes of cycle ergometry with a constant load at 50% of peak

High failure rate 10.8 years after vastus medialis transfer and lateral release (Green's quadricepsplasty) for recurrent dislocation of the patella.

PURPOSE: In adolescent patients with recurrent patellar dislocation, the Green's quadricepsplasty stabilizes the patella in a combination of a lateral release, a transfer of the medial head of the quadriceps onto the lateral part of the patella and an imbrication of the medial patellar retinaculum and joint capsule. This study aimed to evaluate the long-term performance, considering re-dislocations and functional outcomes. We hypothesized a high failure rate in the long term. METHODS: In this single surgeon and single center study 26 knees in 23 patients (mean age 14.2 years; 4-22 years, 18 f

Congenital dislocation of the knee in a three-year-old-child with Larsen syndrome: Treatment with a hexapod-type external fixator.

Congenital knee dislocation (CDK) is a rare condition, and its treatment is subject to debate. Here, we report on a new treatment for CDK (using a hexapod-type external fixator, HEF) in a three-year-old child with Larsen syndrome and grade III anterior dislocations of both knees. The left knee was treated with serial splints, whereas an HEF was used to treat an irreducible dislocation of the right knee. Two HEF aluminium rings were applied surgically. The procedure involved a distraction of the knee, then tibial posterior translation, and lastly progressive flexion. The clinical and radiologic

The quadriceps active ratio: a dynamic MRI-based assessment of patellar height.

INTRODUCTION: Patella alta (PA) is one of the primary correctable risk factors for patellofemoral instability (PFI). Both an accurate diagnosis of PA and a clinically relevant target for correction are necessary for optimal treatment. An ideal test for PA should relate the position of the patella to the femur rather than tibia, should do so with the quadriceps contracted and the patellar tendon under tension and should have good sensitivity and specificity. None of the currently used radiographic tests PA meet these criteria, most of which are based on the position of the patella relative to t

Protective effect by maximal isometric contractions against maximal eccentric exercise-induced muscle damage of the knee extensors.

This study investigated whether maximal voluntary isometric contractions (MVIC) performed before maximal eccentric contractions (MaxEC) would attenuate muscle damage of the knee extensors. Untrained men were placed to an experimental group that performed 6 sets of 10 MVIC at 90° knee flexion 2 weeks before 6 sets of 10 MaxEC or a control group that performed MaxEC only (n = 13/group). Changes in muscle damage markers were assessed before to 5 days after each exercise. Small but significant changes in maximal voluntary concentric contraction torque, range of motion (ROM) and plasma creatine kin

Influence of patellar position on the knee extensor mechanism in normal and crouched walking.

Patella alta is common in cerebral palsy, especially in patients with crouch gait. Correction of patella alta has been advocated in the treatment of crouch, however the appropriate degree of correction and the implications for knee extensor function remain unclear. Therefore, the goal of this study was to assess the impact of patellar position on quadriceps and patellar tendon forces during normal and crouch gait. To this end, a lower extremity musculoskeletal model with a novel 12 degree of freedom knee joint was used to simulate normal gait in a healthy child, as well as mild (23 deg min kne

Quantitative analysis of the patella following the harvest of a quadriceps tendon autograft with a bone block.

PURPOSE: The objective of this study was to determine parameters associated with patellar fracture after quadriceps tendon autograft harvest. METHODS: Thirteen non-fractured and five fractured patella surface models were created based on patient data obtained from a prospective randomized clinical trial in order to assess geometric parameters and bending stress. Measurements that describe the bone block harvest site geometry were used to calculate three normalized parameters. The relative depth parameter describes the thickness of the bone block harvest site with respect to the thickness of t

Quadriceps tendon repair using hamstring, prolene mesh and autologous conditioned plasma augmentation. A novel technique for repair of chronic quadriceps tendon rupture.

BACKGROUND: Several techniques have been described for the primary repair of quadriceps tendon ruptures but there is a paucity of literature on operative management of chronic/recurrent quadriceps tendon ruptures. We describe a novel technique for the revision of quadriceps tendon ruptures which uses hamstring, prolene mesh and autologous conditioned plasma augmentation. METHODS: Our patient was an independently mobile, active 61 year-old man who sustained staggered, bilateral quadriceps tendon ruptures. He had two failed direct repairs on the left side. The patient was unable to actively ext

Is tibial tuberosity-trochlear groove distance an appropriate measure for the identification of knees with patellar instability?

PURPOSE: Tibial tuberosity-trochlear groove distance (TT-TG) has been regarded as a useful tool for establishing therapeutic choices for patellar instability. Recently, it has been shown that TT-TG negatively correlated with the quadriceps angle, suggesting that if used individually, neither provide a valid measure of instability. This study aimed to compare TT-TG distance between both knees in patients with unilateral instability to assess whether this measurement is a decisive element in the management decisions for patellar instability. METHODS: Sixty-two patients (18 male and 44 female),

The acute transverse strain response of the patellar tendon to quadriceps exercise.

INTRODUCTION: The human patellar tendon is highly adaptive to changes in habitual loading, but little is known about its acute mechanical response to exercise. This research evaluated the immediate transverse strain response of the patellar tendon to a bout of resistive quadriceps exercise. METHODS: Twelve healthy adult males (mean age, 34.0 ± 12.1 yr; height, 1.75 ± 0.09 m; and weight, 76.7 ± 12.3 kg) free of knee pain participated in the research. A 10- to 5-MHz linear array transducer was used to acquire standardized sagittal sonograms of the right patellar tendon immediately before and af

Reconstruction of severe patellar infera syndrome with free vascularised composite tensor fascia lata flap.

Post-traumatic severe patella infera and intra-articular adhesion may lead to a severe knee stiffness. We report a 29-year-old man, a muslim prayer leader, who had a previous knee injury. He presented with knee movement from ten degrees to 30 degrees, patellar infera with a length of patella to length of patellar tendon ratio of 2:5, and severe knee arthrofibrosis. He underwent incision of the patella ligament and open arthrofibrosis release, leaving a tendon gap and skin defect of 5 cm. Reconstruction was successfully done using a free vascularised composite tensor fascia lata flap. He regain

Biomechanics of changes in ACL and PCL material properties or prestrains in flexion under muscle force-implications in ligament reconstruction.

The effects of changes in cruciate ligament material and prestrain on knee joint biomechanics following ligament reconstruction surgery by a tendon are not adequately known. A 3D nonlinear finite element model of the entire knee joint was used to investigate the joint response at different flexion angles under a quadriceps force while varying ACL and PCL initial strains or material properties. The ACL and PCL forces as well as tibiofemoral contact forces/areas substantially increased with greater ACL or PCL initial strains or stiffness. The patellofemoral contact force slightly increased where

The effects of Tai Chi Chuan combined with unstable resistance training on knee muscle strength and dynamic balance in female college students: a randomized controlled study protocol.

BACKGROUND: Insufficient physical activity and prolonged sedentary behavior among college students are associated with reduced lower-limb muscle strength, impaired dynamic balance, and increased susceptibility to sports-related knee injury. Female college students may be especially vulnerable because of sex-specific neuromuscular and biomechanical characteristics. Tai Chi Chuan may improve proprioceptive control and coordinated knee muscle activation, whereas unstable resistance training may rapidly enhance strength and neuromuscular stabilization. Whether a combined programme can produce comp

Effects of a Multicomponent Periodized Program in Kinematic and Muscle Activity Characteristics Related to Anterior Cruciate Ligament Injury Mechanism in College Football Players-A Controlled Trial.

Background: Given the persistent sex-based disparities in anterior cruciate ligament (ACL) injury prevalence and the heightened susceptibility observed during critical stages of development in female academy-level players, it is necessary to enhance the effectiveness of prevention programs, particularly during change of direction (COD). Objectives: This study aims to evaluate whether a multicomponent periodized program modifies three-dimensional knee angular velocity and hamstrings and quadriceps muscle activity during a COD task in under-16 female football players. A secondary objective was t…

Effects of rear-foot instability devices on lower-limb muscle activation during the Bulgarian split squat in male football players.

Unilateral resistance exercises such as the Bulgarian Split Squat (BSS) are commonly used to develop lower-limb strength, postural control, and neuromuscular coordination, depending on training variables (e.g., load and intensity). Although instability training increases muscle activation, few studies have examined the effect of rearfoot instability on neuromuscular responses during BSS. This randomized crossover study investigated the acute effects of three rear-foot instability devices on muscle activation during the ascent and descent phases of the BSS in 23 trained male football players. The influence of altered lower-limb muscle strength on dynamic plantar pressure distribution in participants who underwent anterior cruciate ligament reconstruction.

INTRODUCTION: Deficits in lower-limb muscle strength and altered gait mechanics are common after anterior cruciate ligament reconstruction (ACL). While isokinetic strength testing is widely accepted in return-to-sport assessment, the role of plantar pressure analysis in detecting compensatory gait strategies remains underexplored. METHODS: This study included 10 male patients (30.27 ± 5.59 years; 178.37 ± 6.30 cm; 84.85 ± 10.74 kg) who underwent ACL reconstruction using bone-patellar tendon-bone autografts. Assessments were performed preoperatively and at 3 and 6 months postoperatively, evalu
