[Emergency in-hospital anaesthesia assistance].

BACKGROUND: Anesthesiologists and nurse anaesthesists have important roles in the management of in-hospital medical emergencies, and in trauma management. We investigated the extent of these services provided by the department of anaesthesiology at St. Olav University Hospital in Trondheim in the year 2002. MATERIAL AND METHODS: Missions involving emergency medical assistance were identified by a computer search in the department's database. We assessed medical aspects, place, time, and type of intervention. RESULTS: We identified a total of 646 missions, of which 501 occurred in the emergen

Use of the Bullard laryngoscope in the adult patient with epiglottitis: a case report.

This case report involves the anesthetic management of a rare case of adult epiglottitis. It is unique in that there are real photographs of the illness and a previously undocumented use of a specific airway adjunct in acute epiglottitis, the Bullard laryngoscope. Management of the case from start to finish is discussed, as well as the usefulness of the Bullard laryngoscope in a difficult intubation scenario.

Infectious diseases in the operating room.

Patients with infectious diseases have special implications for infection control in the operating room. The increased use and abuse of antibiotics has ushered in a category of resistant organisms. These multiresistant organisms are spread by direct or indirect contact, primarily from the hands of caregivers or contact with contaminated environmental surfaces. Another category of infectious diseases is prions (pronounced pree-ons). Unlike other infectious diseases, human prions diseases are not spread through routine exposures such as direct contact, droplet, and airborne routes. The causative

Anesthesia considerations for patients with narcolepsy.

Narcolepsy is a derangement of the normal sleep-wakefulness rhythms. Originally, narcolepsy was thought to be a form of epilepsy; however, with the development and subsequent refinement of the electroencephalograph, this notion is no longer accepted. The disorder is characterized by inappropriate intrusions of rapid eye movement sleep into the wakeful state and multiple disruptions of the sleep cycles. Narcolepsy usually has its onset anytime between the ages of 10 years and 50 years, with the greatest majority of patients first reporting noticeable symptoms between the ages of 15 and 35 years

Beyond trauma: The unforeseen challenge of Brugada syndrome.

Brugada syndrome, a rare cardiac disorder stemming from mutations in sodium channels, manifests as ventricular arrhythmias, potentially culm … ardiogram pattern of Brugada syndrome can mimic acute myocardial infarction, posing a challenge for anesthetists to recognize this rare cardiac condition in emergency situations. Given its association with ventricular arrhythmias and sudden cardiac death, prompt identification is crucial for appropriate management. This case emphasizes the potential misdiagnos …

A discussion of anesthesiologically relevant aspects of Klinefelter syndrome -a case report.

BACKGROUND: Klinefelter syndrome (KS), usually the 47,XXY karyotype, is the most common sex chromosome anomaly in males. However, many cases … anesthesia was unremarkable. CONCLUSIONS: Despite the unremarkable anesthetic course in our case, anesthetists should be aware of the potential for a difficult airway and cardiovascular and other complications associated with this syndrome. During preoperative examination, attention should be paid to common secondary manifestations of KS to avoid perioperati …

[Maternal deaths in Sweden: Diagnostics and clinical management could be improved].

MM-ARG, the Swedish maternal maternity mortality group within SFOG (Swedish Society of Obstetrics and Gynecology) has, since 2008, surveyed and analysed maternal deaths in Sweden with the aim to find and give feedback on lessons learned to the medical professions.  MM-ARG consists of obstetricians, midwives and anesthetists and the strength of the working model is that the profession itself takes responsibility for the scrutiny.  A summary of 67 known maternal deaths from 2007‒2017 is presented. Direct causes of death are dominated by hypertensive disease/preeclampsia, followed by thromboembol…

Anesthesia Clinical Practice Guidelines for Ehlers-Danlos Syndrome: A Modified Delphi Survey.

Ehlers-Danlos syndrome (EDS) is a family of inherited connective tissue disorders distinguished by joint hyperextensibility, skin hyperelasticity, and increased tissue friability. No standardized anesthesia guidelines currently exist for this population, despite facing increased perioperative risks. To address this gap in literature, a three-round modified Delphi survey was conducted with 15 experts in anesthesia and EDS. Seven preliminary guidelines, developed from a comprehensive literature review, were evaluated and refined across three rounds. Consensus was achieved for all seven guideline

Use of sterile gowns for single-shot spinal anaesthesia: consensus guidelines from the Association of Anaesthetists, Royal College of Anaesthetists, Obstetric Anaesthetists' Association, Regional Anaesthesia UK, College of Anaesthesiologists of Ireland and Australian and New Zealand College of Anaesthetists.

INTRODUCTION: International guideline recommendations vary on the use of sterile gowns during spinal anaesthesia. There is limited evidence of benefit for their routine use and debate about environmental, financial and clinical costs and benefits. Updated, evidence-informed, consensus-based recommendations on the essential infection prevention measures for single-shot spinal anaesthesia are required to balance the need to maintain high standards of infection prevention with proportionate and sustainable practice. METHODS: A comprehensive literature review was undertaken to explore: the risk o

The potential contribution of deficiencies in handoffs between anaesthetists to the fatal outcome following ventral slot surgery in a dog.

Increasingly complex cases in veterinary anaesthesia challenge anaesthetists, especially when patient care is transferred (handed off) between team members. Standardized and structured patient handoffs are used in human medicine to improve patient safety. A male neutered Dachshund, aged 9 years, with a history of cervical pain presented for magnetic resonance imaging. The medical history included mitral valve insufficiency (grade B1) and intra-anaesthetic bradycardia and apnoea at a different clinic. Induction of anaesthesia was performed by a fourth year anaesthesia resident and the dog hande

An assessment of introducers used for airway management.

Different introducers are available to assist with tracheal intubation. Subtle differences in the design of introducers can have a marked effect on safety and performance. The Difficult Airway Society's Airway Device Evaluation Project Team proposal states that devices should only be purchased for which there is at least a case-control study on patients assessing airway devices. However, resources are not currently available to carry out a case-control study on all introducers available on the market. This study comprised a laboratory and manikin-based investigation to identify introducers tha

Pre-hospital emergency anaesthesia in trauma patients treated by anaesthesiologist and nurse anaesthetist staffed critical care teams.

BACKGROUND: Pre-hospital tracheal intubation in trauma patients has recently been questioned. However, not only the trauma and patient characteristics but also airway provider competence differ between systems making simplified statements difficult. METHOD: The study is a subgroup analysis of trauma patients included in the PHAST study. PHAST was a prospective, observational, multicentre study on pre-hospital advanced airway management by anaesthesiologist and nurse anaesthetist manned pre-hospital critical care teams in the Nordic countries May 2015-November 2016. Endpoints include intubatio

Perioperative anaphylaxis: diagnostic challenges and management.

PURPOSE OF REVIEW: Perioperative hypersensitivity reactions can pose diagnostic and management challenges for the anaesthetist. Difficulties in diagnosing hypersensitivity reactions in the perioperative setting are highlighted and recommendations from recent guidelines on the acute management of life-threatening anaphylaxis are presented. RECENT FINDINGS: Anaesthetists play a key role in investigating perioperative hypersensitivity reactions. During a suspected perioperative hypersensitivity event, a serum tryptase level should be measured to help with subsequent allergy investigation. Moreov

Resuscitative Endovascular Balloon Occlusion of the Aorta (REBOA) as an Option for Uncontrolled Hemorrhagic Shock: Current Best Practices and Anesthetic Implications.

Noncompressible torso hemorrhage is reported to be a leading cause of potentially preventable mortality in both civilian trauma victims and military combat casualties. This hemorrhage may come from venous, arterial, or additional combined sources in the chest, abdomen, pelvis, axilla, or groin regions. Aortic occlusion as an adjunct to strategies for trauma damage control can decrease the amount of bleeding distal to the occluded site and provide a time-sensitive opportunity for resuscitation and definitive hemorrhage control. Recently, resuscitative endovascular balloon occlusion of the aorta

Complications of nasotracheal intubation.

For a two-month period, 71 nasotracheal intubations done in the emergency department or inpatient units at Detroit General Hospital were studied. Sixty-five of 71 intubations, or 91%, were successful. Mild or moderate complications occurred in 15 of 71 patients, and consisted of mild or moderate bleeding. Moderate to severe complications occurred in seven of 71 patients, and consisted of severe bleeding and retropharyngeal perforation. Retropharyngeal perforation may be a more common complication of nasotracheal intubation than realized. The major technical error is failure to appreciate that

Tracheal tube misplacement in the thoracic cavity: A case report.

BACKGROUND: Penetrating neck injuries require prompt recognition, diagnosis and management of critical airways. This case demonstrates an emergent situation that a "medical negligence" was avoided with the aid of end-tidal carbon dioxide (ETCO2) waveform. CASE SUMMARY: We report a case of malposition of the endotracheal tube into the right hemithoracic cavity for cervical knife trauma, resulting in pneumothorax. Tube placement was not confirmed during emergency airway management, and the patient was directly transferred to the emergency operation room. Assisted by ETCO2 and imaging examinatio
