Cumulative blood pressure exposure and cognition: the potential mediating role of brain volume.

Elevated blood pressure (BP) has been linked to brain structure changes and cognitive decline. However, few studies have accounted for long-term cumulative BP exposure. We investigated the association between cumulative BP exposure, brain volume, cerebral blood flow (CBF), and cognitive decline. Furthermore, we explored whether alterations in brain volume and CBF mediated the association between cumulative BP and cognitive decline. We included 1012 adult participants from the Kailuan study. Cumulative BP exposure was calculated from 2006 to 2020. Brain MRI scans and the Montreal Cognitive Asse

Renal TRPM3 Channels Regulate Blood Pressure via Tubuloglomerular Feedback and Plasma Volume Control.

BACKGROUND: TRPM3 is a nonselective cation channel activated by heat, osmotic pressure, and neurosteroids. It is highly expressed in sensory neurons, where it integrates thermal, chemical, and inflammatory signals to modulate downstream responses, but is also present in the brain, kidney, and cardiovascular system. This distribution suggests a role in cardiovascular and renal regulation. We hypothesize that TRPM3 channels may play a role in blood pressure (BP) regulation via both vascular and renal mechanisms. Therefore, analysis of the vascular phenotype in Trpm3-KO mice can provide insights

Time evolution of piglet cerebral blood volume after resuscitation from hypoxic-ischemic insult.

Neonatal hypoxic-ischemic encephalopathy (HIE) is a significant cause of neonatal mortality and developmental disabilities. It has been revealed that the temporal behavior of the cerebral blood volume (CBV) carries information on the degree of hypoxia-ischemia. CBV can be estimated by means of near-infrared spectroscopy. The change of CBV after the insult is related to the change of CBV during the insult. In this paper, we consider a mathematical model which governs the time evolution of CBV after the insult. We show that the temporal behavior of CBV can be predicted with the Kalman filter whi…

Regional pulmonary perfusion, blood volume, and their relationship change in experimental early ARDS.

Regional pulmonary perfusion (Q) has been investigated using blood volume (Fb) imaging as an easier-to-measure surrogate. However, it is unclear if changing pulmonary conditions could affect their relationship. We hypothesized that vascular changes in early acute respiratory distress syndrome (ARDS) affect Q and Fb differently. Five sheep were anesthetized and received lung protective mechanical ventilation for 20 h while endotoxin was continuously infused. Using dynamic 18F-FDG and 13NN Positron Emission Tomography (PET), regional Fb and Q were analysed in 30 regions of interest (ROIs) and no…

Testicular arterial blood flow volume in predicting semen improvement following microscopic subinguinal varicocelectomy.

BACKGROUND: Varicocele is a significant but treatable contributor to male infertility. The efficacy of varicocelectomy in improving sperm quality is not consistent, with only 60-80% of patients experiencing improved semen quality. This prospective cohort study aimed to evaluate the effect of microscopic subinguinal varicocelectomy (MSV) on testicular arterial blood flow volume (TABFV) and to determine the value of preoperative TABFV in predicting the outcome of MSV. METHODS: Patients with varicocele who underwent MSV at the same clinical center between July 2020 and April 2023 were enrolled.

Relative Blood Volume Monitoring during Continuous Renal Replacement Therapy: A Prospective Observational Study.

INTRODUCTION: Hematocrit monitoring during continuous renal replacement therapy (CRRT) allows the continuous estimation of relative blood volume (RBV). This may enable early detection of intravascular volume depletion prior to clinical sequelae. We aimed to investigate the feasibility of extended RBV monitoring and its epidemiology during usual CRRT management by clinicians unaware of RBV. Moreover, we studied the association between changes in RBV and net ultrafiltration (NUF) rates. METHODS: In a cohort of adult intensive care unit patients receiving CRRT, we continuously monitored hematocr…

Is minimising waste volume for drawing blood samples in critically ill patients feasible?

INTRODUCTION: Drawing blood samples through a central venous catheter (CVC) is a customary practice in Intensive Care Units (ICUs). It is indicated to discard a volume of waste blood to avoid interference in the results. AIM: To determine whether a lower discard volume for obtaining blood samples from temporary CVCs placed into the internal jugular, femoral or subclavian vein offers valid results. METHOD: A quasi-experimental prospective cross-sectional study for which sixty-five patients of over 18 years of age in ICUs, who had been fitted with a triple lumen central venous catheter, were r

All-cause mortality in relation to changes in relative blood volume during hemodialysis.

BACKGROUND: Relative blood volume (RBV) monitoring is widely used in hemodialysis (HD) patients, yet the association between intradialytic RBV and mortality is unknown. METHODS: Intradialytic RBV was recorded once/min during a 6-month baseline period; all-cause mortality was noted during follow-up. RBV at 1, 2 and 3 h into HD served as a predictor of all-cause mortality during follow-up. We employed Kaplan-Meier analysis, univariate and adjusted Cox proportional hazards models for survival analysis. RESULTS: We studied 842 patients. During follow-up (median 30.8 months), 249 patients (29.6%)…

Effect of Improving Volume Overload on Home Blood Pressure Variability in Hemodialysis Patients.

Blood pressure variability is an independent predictor of adverse clinical events in hemodialysis patients. Volume overload is one of the most important factors affecting blood pressure homeostasis. In the present study, we explored the effects of dry weight reduction on home blood pressure variability in volume overload hemodialysis patients. Hemodialysis patients with volume overload had their dry weight gradually decreased under the guidance of bioimpedance methods, which was represented by calf-bioimpedance ratio (Calf-BR). Home blood pressure was measured on waking up and at bedtime for 1

Chronic kidney disease, cerebral blood flow, and white matter volume in hypertensive adults.

OBJECTIVE: To determine the relation between markers of kidney disease-estimated glomerular filtration rate (eGFR) and urine albumin to creatinine ratio (UACR)-with cerebral blood flow (CBF) and white matter volume (WMV) in hypertensive adults. METHODS: We used baseline data collected from 665 nondiabetic hypertensive adults aged ≥50 years participating in the Systolic Blood Pressure Intervention Trial (SPRINT). We used arterial spin labeling to measure CBF and structural 3T images to segment tissue into normal and abnormal WMV. We used quantile regression to estimate the association between

Adverse effects of small-volume red blood cell transfusions in the neonatal population.

BACKGROUND: Adverse transfusion reactions in the neonatal population are poorly understood and defined. The incidence and pattern of adverse effects due to red blood cell (RBC) transfusion are not well known, and there has been no systematic review of published adverse events. RBC transfusions continue to be linked to the development of morbidities unique to neonates, including chronic lung disease, retinopathy of prematurity, intraventricular haemorrhage and necrotising enterocolitis. Uncertainties about the exact nature of risks alongside benefits of RBC transfusion may contribute to evidenc

Increased blood volume following resolution of acute cardiogenic pulmonary oedema: a retrospective analysis.

BACKGROUND: Acute cardiogenic pulmonary oedema (APO) occurs due to an increase in pulmonary microvascular pressure and massive transvascular fluid filtration into the lungs, causing respiratory insufficiency. OBJECTIVE: To determine whether fluid sequestration in the lungs effectively leads to contraction of the circulating blood volume, leading to relative hypovolaemia, and whether resolution of APO and fluid shift to the vascular compartment restores the circulating volume. METHODS: A retrospective analysis was conducted in the intensive care unit of a university teaching hospital, April -…

Respiration-induced changes in ear photoplethysmography relates to relative blood volume during hemodialysis.

Renal failure patients provide a good model of fluid overload with the process of hemodialysis leading to central hypovolemia. This study ai … hange was observed in non-fluid removal patients. A moderate correlation was found between relative blood volume (RBV) and HF power (median R = 0.64, p 〈 0.05). This study suggests that ear PPG may be a suitable monitor of the systemic circulation and can provide a non-invasive tool to detect blood volume loss.

The sympathetic nervous system and blood volume regulation: lessons from autonomic failure patients.

Patients with autonomic failure provide a unique opportunity to study the role of sympathetic function on the regulation of blood volume. These patients have a reversal of the normal diurnal variation in urine output and have twice as much natriuresis during the night. Autonomic failure patients are also unable to conserve sodium and fail to decrease natriuresis in response to dietary sodium restriction. Whereas normal subjects are able to maintain blood pressure within narrow values throughout a wide range of plasma volumes, blood pressure is linearly correlated to changes in plasma volume in…

Model of depressed myocardium shows orthostatic intolerance with or without reduced blood volume.

INTRODUCTION: The development of orthostatic hypotension (OH) is complex and multi-factorial. Previous simulation work indicates that myocardial contractility depression (MCD) may increase OH when there is a total blood volume decrease. This paper hypothesized that MCD increased OH in both humans with and without decrease in blood volume. METHODS: A model, which was previously used to reproduce cardiovascular response to lower body negative pressure (LBNP), hypovolemia, and MCD, was modified by incorporating the physiologic mechanism of plasma filtration into the interstitium during LBNP. The…
