[High flow nasal cannula oxygen therapy in the treatment of acute bronchiolitis in neonates].

OBJECTIVE: To determine whether the availability of heated humidified high-flow nasal cannula (HFNC) therapy was associated with a decrease in need for mechanical ventilation in neonates hospitalised with acute bronchiolitis. METHODS: A combined retrospective and prospective (ambispective) cohort study was performed in a type II-B Neonatal Unit, including hospitalised neonates with acute bronchiolitis after the introduction of HFNC (HFNC-period; October 2011-April 2015). They were compared with a historical cohort prior to the availability of this technique (pre-HFNC; January 2008-May 2011). …

Are high flow nasal cannulae noisier than bubble CPAP for preterm infants?

BACKGROUND: Noise exposure in the neonatal intensive care unit is believed to be a risk factor for hearing loss in preterm neonates. Continuous positive airway pressure (CPAP) devices exceed recommended noise levels. High flow nasal cannulae (HFNC) are an increasingly popular alternative to CPAP for treating preterm infants, but there are no in vivo studies assessing noise production by HFNC. OBJECTIVE: To study whether HFNC are noisier than bubble CPAP (BCPAP) for preterm infants. METHODS: An observational study of preterm infants receiving HFNC or BCPAP. Noise levels within the external au…

High-flow nasal cannulae for respiratory support of preterm infants: a review of the evidence.

BACKGROUND: High-flow nasal cannulae (HFNC) are gaining in popularity as a form of non-invasive respiratory support for preterm infants in neonatal intensive care units around the world. They are proposed as an alternative to nasal continuous positive airway pressure (NCPAP) in a variety of clinical situations, including post-extubation support, primary therapy from birth and 'weaning' from NCPAP. OBJECTIVES: To present and discuss the available evidence for the use of HFNC in the preterm population. METHODS: An internet-based literature search for relevant, original research articles (both …

Effect of nasal cannula oxygen administration on oxygen concentration at facial and adjacent landmarks.

An oxygen-enriched atmosphere enhances the potential for operating-room fires. We thus determined oxygen concentrations at various facial landmarks during oxygen administration via nasal cannulae. Thirteen supine volunteers were draped similarly to patients undergoing a cervical-node biopsy. Oxygen was delivered in random order through nasal cannulae at rates of 2, 4, and 6 l x min(-1). Oxygen concentration was measured at pre-determined facial landmarks and also distal to the drape at non-facial sites. At a flow of 2 l x min(-1), oxygen concentrations exceeded 23% only within a few centimetre…

Placement of a long term tracheal cannula in a Thoroughbred racehorse with bilateral laryngeal dysfunction.

A Thoroughbred horse with bilateral laryngeal dysfunction was treated by placement of a long term tracheal cannula in order to restore full athletic function. The horse initially presented with right-sided arytenoid dysfunction that was considered to be due to a congenital malformation of the laryngeal cartilage. This was corrected by a right-sided laryngoplasty and ventriculectomy. The horse re-presented 1 year later with idiopathic left laryngeal hemiplegia. The decision was made to place a long-term tracheal cannula due to the low success rate and high complication rate associated with othe…

Delivered oxygen concentrations using low-flow and high-flow nasal cannulas.

INTRODUCTION: Nasal cannulas are commonly used to deliver oxygen in acute and chronic care settings; however, there are few data available on delivered fraction of inspired oxygen (F(IO(2))). The purposes of this study were to determine the delivered F(IO(2)) on human subjects using low-flow and high-flow nasal cannulas, and to determine the effects of mouth-closed and mouth-open breathing on F(IO(2)). METHODS: We measured the pharyngeal F(IO(2)) delivered by adult nasal cannulas at 1-6 L/min and high-flow nasal cannulas at 6-15 L/min consecutively in 10 normal subjects. Oxygen was initiated …

Technical Note: Improved technique for fitting pigs with steered ileocecal valve cannulas.

Collection of ileal digesta to evaluate AA digestibilities has become increasingly important in swine nutrition research. Steered ileocecal valve cannulation of pigs permits total collection of ileal digesta, while still allowing normal digesta flow during noncollection periods. This technique was modified and used with 64 crossbred barrows in five trials. Our procedural changes included preoperative i.v. administration of a broad-spectrum antibiotic and nonsteroidal antiinflammatory drug, sharp incision through the muscle layers of the laparotomy wound, use of a heparinized saline lavage solu…

Active venovenous bypass of the retrohepatic vena cava utilizing two atrial cannulae.

Injuries to the hepatic veins and retrohepatic vena cava have a high mortality due to uncontrolled hemorrhage. Successful repair may necessitate interruption of flow through the retrohepatic vena cava. Active bypass of the area is then needed to provide adequate venous return. Published methods for active bypass require cannulation of axillary and femoral veins in addition to clamping of the vena cava above and below the liver, often with limited exposure and significant risk. This report describes active bypass of the retrohepatic vena cava utilizing two right atrial cannulae. The simplicity …

Effect of cannula length on aortic arch flow: protection of the atheromatous aortic arch.

Atheromatous disease in the transverse aortic arch is associated with an increased incidence of perioperative stroke. In addition, tissue erosion in the aortic arch is caused by the high-velocity jet emerging from an aortic cannula during cardiopulmonary bypass (CPB), termed the "sandblast effect". To quantify this phenomenon, flow in the aortic arch was measured intraoperatively by epiaortic ultrasonography in 18 patients undergoing CPB. All were cannulated in the ascending aorta, 10 with a short (1.5 cm) cannula and 8 with a long (7.0 cm) cannula. The peak forward aortic flow velocities (mea…

[Scanning electron microscopy examination of 87 pre-injection blunted retrobulbar cannulas].

UNLABELLED: The use of blunt cannulas for retrobulbar injection (retro) is recommended to reduce the risk of retrobulbar hemorrhage and globe perforation. Retro cannulas with blunt tips--so-called Atkinson needles--are commercially available. They can also be produced by being blunted on a sterile surface immediately before injection. METHODS: In a prospective study, all retro cannulas (0.5 x 40 mm, Steriseal) used over 8 days in our hospital were collected after being blunted on a sterile Petri dish, followed by injection. Subsequently, scanning electron microscopic examination was performed…

Determination of cerebrospinal fluid gentamicin in the beagle using an indwelling cerebral ventricular cannula.

This study compared the concentrations of gentamicin in the blood and cerebrospinal fluid (CSF) of young beagles. Indwelling cannulas were surgically implanted into the external jugular vein and the left lateral cerebral ventricle of two 5-month-old beagles. Gentamicin was given intravenously (10 mg/kg) and CSF and blood samples were taken. This was repeated in 1 dog three times over 42 days. CSF collected 2 weeks after cannula implantation was cytologically normal. Significant levels of gentamicin were detected in the CSF shortly after injection in the absence of inflammation. Levels persiste…

Intravascular Retrieval of a Kinked Inferior Vena Cava Cannula Complicating Venovenous Extracorporeal Life Support.

Cannula malpositioning is one of several complications recognized during the insertion of cannulas for extracorporeal life support that can lead to loss of cannula function or more serious injury due to vessel perforation. In the absence of vessel perforation, repositioning the cannula is often sufficient to restore function. We report a case of a kinked femoral drainage cannula with folding and retroflexion in the inferior vena cava (IVC) that was successfully repositioned with intravascular intervention. A 41-year-old woman was placed on venovenous extracorporeal life support (VV ECLS) for n…

A Cannula-Based Sequential Injection Protocol for Non-surgical Rhinoplasty in East Asian Patients.

INTRODUCTION: Non‑surgical rhinoplasty is increasingly popular for nasal augmentation in East Asian patients. However, a standardized, anatomy‑driven protocol for the sequential and layered correction of the nasal tip, dorsum, and glabellar complex-key aesthetic units in the Asian nose-is still lacking. MATERIALS AND METHODS: In this dual‑center retrospective study, 528 East Asian patients (490 females, 38 males; mean age 27.7 years) underwent non‑surgical rhinoplasty between 2020 and 2024. A high‑G' hyaluronic acid filler was injected via cannula in a predefined sequence: first, the nasal ti…

Impact of Multistage Cannula on Recirculation in Various Venovenous Extracorporeal Membrane Oxygenation Cannulation Strategies.

We aimed to compare recirculation of venovenous extracorporeal membrane oxygenation (V-V ECMO) administered with various cannula placements for femoro-femoral (F-F) and femoro-jugular (F-J) configurations in patients with respiratory failure. This single-center retrospective study included 58 consecutive patients who underwent V-V ECMO between January 2021 and December 2023. Ultrasonic dilution technology was used to quantify the flow of recirculation. The drain cannula placements were categorized into three positions: right atrium and superior and inferior venae cavae. The primary outcome was…

A cannula connecting strategy fostering vascular anastomosis based on mechanical support and biochemical modulation.

Anastomosis is essential in cardiovascular surgery. However, traditional hand-sewn techniques are technically demanding, and existing sutureless methods often result in complications such as thrombosis and delayed healing due to poor mechanical compliance and insufficient endothelialization. Herein, we propose a cannula connection strategy that combines mechanical and biochemical support via a vascular-shaped polyetheretherketone (PEEK) connector. The PEEK substrate is coated with polydopamine (PDA) and chemically grafted with S-nitroso-N-acetylpenicillamine (SNAP) to enable sustained nitric o…
