[Role of molecular biology in understanding the formation of congenital cardiomyopathies in humans].

New insights into the genetics of congenital heart diseases in human beings have been obtained these past years. The identification of genes for heart defects have led to a new clinical approach of these malformations in children and their families. These progresses have been made by means of positional cloning as well as through the analysis of mouse models. These findings uncovered a new complexity in the development of cardiac defects and led to revise the different classifications for congenital heart defects. Pediatric cardiologists have also improved their efficiency in defining cardiac

Acromegalic cardiomyopathy: a neglected cause of cardiomyopathy.

Acromegaly represents a rare endocrine condition characterized by an excessive secretion of the growth hormone (GH) and the insulin-like growth factor-1 (IGF-1), mostly subsequent to a pituitary adenoma. Acromegaly affects 40-60 people per million without significant dif-ference between men and women, with an average age of onset of 44. The slow progression of the disease leads to a delay in diagnosis of 4 to 10 years from the onset of the hormonal imbalance. The increase of the GH and IFG-1 levels is associated with cardiac, respiratory, metabolic and rheumatic disorders. The cardiac involvem

Cardiomyopathy and thyrotoxicosis: tachycardiomyopathy or thyrotoxic cardiomyopathy?

Ischaemic heart disease is the most common cause of heart failure, but less common causes such as hyperthyroidism should be kept in mind. As a potential reversible cause of cardiomyopathy hyperthyroidism should be excluded in every new patient, young and old, especially in the absence of coronary artery disease. Inspired by an intriguing clinical case we address the question whether hyperthyroidism-related cardiomyopathy is due to tachycardiomyopathy or to thyrotoxic cardiomyopathy.

Clinical and Genetic Characteristics of Hereditary Arrhythmias and Cardiomyopathies in the Middle East.

Inherited arrhythmias and cardiomyopathies are leading causes of sudden cardiac death in young individuals. Although global registries and genetic databases have advanced diagnosis and prevention globally, data from the Middle East (ME) remain limited and fragmented. This review synthesizes clinical and genetic evidence on long QT syndrome, catecholaminergic polymorphic ventricular tachycardia, Brugada syndrome, arrhythmogenic right ventricular cardiomyopathy, hypertrophic cardiomyopathy, and dilated cardiomyopathy across the ME. A defining feature of ME cohorts is the high prevalence of consa…

When Substrate Meets Device: ICD Implantation and Therapy across the Spectrum of Cardiomyopathies.

INTRODUCTION: Implantable cardioverter-defibrillators (ICDs) are the cornerstone of sudden cardiac death prevention in cardiomyopathies, but disease-specific structural and electrophysiological substrates profoundly affect device performance and outcomes. Understanding these distinctions is critical for tailoring therapy and improving patient safety. AREAS COVERED: This review examines current evidence and practical experience regarding ICD implantation, programming, and management in hypertrophic cardiomyopathy (HCM), arrhythmogenic right ventricular cardiomyopathy (ARVC), and cardiac amyloi…

Low Albumin, Heavy Hearts: Role of Hypoalbuminemia in Infiltrative Cardiomyopathies: A Narrative Review.

Infiltrative cardiomyopathies, a subset of restrictive cardiomyopathies, are rare but clinically essential disorders often lead to heart failure, which is a major global health issue. Although interest in these conditions is growing, they remain poorly understood due to their low occurrence, vague clinical signs, and frequent underreporting. Advances in diagnosis have been made through multimodal imaging techniques-including cardiac magnetic resonance, nuclear imaging, and computed tomography-as well as genetic testing and new biomarkers for amyloidosis. However, a universal biomarker that app…

[Precision medicine in the diagnostics and treatment of cardiomyopathies : State of the art].

Cardiomyopathies (CMP) comprise a group of heterogeneous heart muscle diseases that have molecular genetic causes and cannot be adequately explained by other cardiovascular diseases. The diagnosis and treatment of CMPs have made significant progress in recent decades, which is reflected in novel specific cardiomyopathy guidelines of the European Society of Cardiology (ESC) published in 2023. A patient-centered approach combines multimodal diagnostics, such as echocardiography, magnetic resonance imaging, genetic testing and biopsy to enable precise etiological classification and thus personali…

The contribution of genetics to the understanding and management of cardiomyopathies: Part 2.

Cardiomyopathies may present as a manifestation of various inherited syndromes. Recognizing the rarity and diagnostic challenges of syndromic and metabolic cardiomyopathies is crucial, as their identification holds significant implications for targeted treatment and enables the use of specific risk stratification tools. Genetics has assumed a pivotal role in clarifying the pathophysiology of cardiomyopathies, facilitating molecular diagnosis, and enabling effective family screening. The advent of next-generation sequencing has revolutionized genetic testing, enabling cost-effective, high-throu…

Intrapericardial injection of hydrogels with ASC and their secretome to treat dilated cardiomyopathies.

Doxorubicin-induced cardiomyopathy (DOX-IC) is a significant and common complication in patients undergoing chemotherapy, leading to cardiac remodeling and reduced heart function. We hypothesized that the intrapericardial injection of hydrogels derived from the cardiac decellularized extracellular matrix (dECM) loaded with adipose tissue-derived stromal cells (ASC) and their secretome dampens or reverses the progression of DOX-IC. DOX-IC was induced in Wistar male rats through ten weekly intra-peritoneal injections of doxorubicin (cumulative dose: 18 mg/kg). We performed intrapericardial treat

Endoplasmic reticulum stress in cardiomyopathies: from the unfolded protein response to therapeutic opportunities.

The endoplasmic reticulum (ER), a central organelle responsible for maintaining protein homeostasis, calcium balance, and lipid metabolism, is essential for cardiovascular integrity. Functional disruption-referred to as endoplasmic reticulum stress (ERS)-has been recognized as a major pathogenic driver across diverse cardiovascular disorders. Under pathological conditions such as hypoxia, nutrient deprivation, or infection, sustained ERS activates the unfolded protein response (UPR). While initially adaptive, prolonged or excessive ERS initiates apoptotic cascades, severely impairing cardiomyo
