Augmented Reality and 3D Printing as an Aid to Surgical Planning in Modern Microtia Reconstruction.

Since Tanzer's seminal work introducing costal cartilage for ear reconstruction, subsequent advances by Brent, Nagata, and Firmin have refined surgical techniques and aesthetic outcomes. However, fundamental aspects of the procedure-such as planning incisions, achieving accurate ear positioning, and guiding chest wall incisions for rib harvest-have seen limited innovation. The current standard practice of using projection paper or transparent templates to replicate the contralateral ear position remains imprecise. Similarly, autologous cartilage harvest can be particularly challenging in patie

Total ear reconstruction in postburn deformity.

The external ear enjoys a special place in society all over the world. It is meant to be flaunted and adorned. Ear piercing is routine, with a range of jewelry pieces concentrating in enhancing its natural beauty. Persons with deformed ears have to limit their range of hair styles. There is a definite need to reconstruct the deformed ear of both sexes. To achieve desirable results in ear reconstruction is a difficult task. Although cartilage fabrication is an important step in ear reconstruction in postburn deformity of the ear, the final outcome is mainly decided by the quality and quantity o

Tissue engineering: revolution and challenge in auricular cartilage reconstruction.

External ear reconstruction for congenital deformity such as microtia or following trauma remains one of the greatest challenges for reconstructive plastic surgeons. The problems faced in reconstructing the intricate ear framework are highly complex. A durable, inert material that is resistant to scar contracture is required. To date, no material, autologous or prosthetic, is available that perfectly mimics the shapely elastic cartilage found in the ear. Current procedure involves autologous costal cartilage that is sculpted to create a framework for the overlying soft tissues. However, this i

An 8-year-old boy with oculo-auriculo-vertebral syndrome presented to the authors' clinic for multidisciplinary management of his condition. His congenital ear deformity included bilateral protrusion as well as atypical Microtia affecting the left side. He underwent bilateral otoplasty combined with left canaloplasty at the first stage to address prominent ears and canal atresia and. The patient underwent subsequent reconstruction of his constricted left ear with costochondral cartilage graft to address elsewhere. He presented to the authors' clinic 2 weeks postoperatively with wound dehiscenc

[Reconstruction of the burned ear].

BACKGROUND: The external ear is a complex, delicate structure and treatment of burned ears should not be neglected. We will detail the different scenarios a surgeon can encounter and present a therapeutic algorithm. PATIENTS AND METHODS: From 1984 to 2011, we operated 134 patients for ear deformities secondary to burns, of which 90 cases were unilateral (67%) and 44 cases bilateral (32%), accounting for 178 ear in total. Our two-stage technique is based on an autologous cartilage framework, covered by mastoid skin, superficial temporal fascia (STF) or indirect skin expansion, and later by ele

[Application of cartilage in tympanoplasty].

Cartilage has undergone rapid development in ear surgery since Heermann used the grid cartilage technique to repair large perforations of the tympanic membrane. Cartilage has been widely used in tympanoplasty, ossicular chain reconstruction, reconstruction of the lateral wall of the upper tympanic cavity and the posterior wall of the external auditory canal due to its advantages of convenient sampling, high stability, good elasticity, low metabolic rate, easy survival and strong plasticity. This paper reviews the use of cartilage in tympanoplasty and discusses the possibility of placing cartil

Dual Option Microtia Clinic: A Comparison of Outcomes in Microtia Reconstruction Using Autologous Rib or Porous Polyethylene Implant.

Introduction: Auricular reconstruction techniques most frequently utilize either autologous costal cartilage or alloplastic porous polyethylene (PPE) implant. Objectives: To compare the aesthetic outcomes, number of surgeries, and complications in children who underwent microtia reconstruction with either rib or PPE implant by blinded photograph review. Methods: This retrospective cohort study included consecutive pediatric patients who underwent auricular reconstruction with either autologous cartilage or PPE between November 2017 and February 2022. Blinded observers rated each postoperative

"Pseudocyst of the auricle", othematoma and otoseroma: three faces of the same coin?

Cystic swellings of the choncha of the ear without serious inflammation are routine findings for otolaryngologists. They are frequently diagnosed as othematoma or otoseroma and may be caused by traceable traumas or microtraumas. "Pseudocyst of the auricle" is defined as intracartilaginous cavity lacking epithelial lining. Thus, according to previous reports "pseudocysts" are supposed to occur due to chondromalacia within the cartilage. We recently observed four cases of "pseudocyst of the auricle" characterized by non-inflammatory, merely painless swellings on the anthelix part of the ears wit

Use of a plastic eraser for ear reconstruction training.

BACKGROUND: Microtia reconstruction is a challenging procedure, especially in developing nations. The most complex part is learning how to fabricate a framework from costal cartilage. We herein propose a training regimen for ear reconstruction with the use of a plastic eraser. MATERIALS AND METHODS: The texture of a plastic eraser made from polyvinyl chloride is similar to that of human costal cartilage. The first step of the training is carving out the sixth through eighth rib cartilages from a block of plastic eraser. The second step is a fabrication of the framework from plastic rib cartil

Pinnaplasty: reshaping ears to improve hearing aid retention.

The hearing aid is extremely important to the deaf. A small number have difficulty in retaining the device because the ear is prominent or cup-shaped. This report describes 11 children whose ear shape was modified to improve hearing aid retention and one adult in whom an over set back ear was released to allow fitment of a postaural device. In eight of the 11 children treated, conservative measures such as double-sided tape and retention bands (Huggies) had been tried previously without success. The creation of an antihelical fold in a misshapen ear lacking such a fold provides a reinforcing s

Perichondritis of the auricle.

A search of the literature reveals reports of only 191 cases of auricular perichondritis. The present study includes 15 further cases caused by pseudomonas and proteus. Stroud's excision technique was used in four cases and resulted in marked deformity, repeated debridement, and protracted treatment. Tubal drainage was used in the remaining ears with very good esthetic results. It has been demonstrated in experimental animals and in humans that new cartilage forms in the subperichondrial auricular space in the second postoperative week, being maximum in the fourth week. In the present study tu

Benefits of Combined Surgical Techniques in Otoplasty.

OBJECTIVE: Prominent ear deformity is one of the aesthetic problems that should be treated due to the psychosocial problems it causes. We aimed to present our results in which we used the combination of Mustarde suture to create an antihelix, conchal resection, and concomastoid suture combination to reduce conchal excess in the light of literature. MATERIALS AND METHODS: A total of 28 cases in which the same otoplasty method was used in 2 separate centers between 2014 and 2020 were included in the study. Elliptic skin excision, antihelix reconstruction with Mustarde suture, and in cases with

Pseudomonas aeruginosa necrotizing chondritis complicating high helical ear piercing case report: clinical and public health perspectives.

BACKGROUND: Auricular or high helical ear piercing is an increasingly widespread fashion trend that is associated with an increased risk of potentially serious post-piercing complications such as auricular perichondritis. CASE REPORT: An 11-year-old girl developed severe auricular perichondritis following piercing of the upper helical cartilage of her ear at a hairdressing salon. Four days post piercing, she returned to the same salon for a haircut during which the pierced site was manipulated. She presented to her family physician and was treated unsuccessfully with oral cephalexin. She was

Cartilage exposure following autologous microtia reconstruction: An algorithmic treatment approach✰.

BACKGROUND: Successful reconstruction of microtia involves fabrication of a cartilaginous framework and provision of thin, durable, soft tissue cover. Vascular compromise of this skin envelope can lead to exposure of the underlying cartilage, resulting in cartilage resorption and distortion of the final form of the ear construct. We describe our algorithm for management of this complication. METHODS: All patients who underwent autologous ear reconstruction by a single surgeon (NWB) from April 2006 to September 2012 were retrospectively reviewed to identify any that developed exposure of the u

A method of stapedectomy in the fenestrated ear.

Two consequences of a previous fenestration militate against successful stapledectomy. They are the lessened depth of the middle ear over the stapes, and the missing incus. Twelve years' experience of using the stapedectomy for fenestrated ears in 62 patients has given generally disappointing results until, in 1962, technique of using cartilage blocks to increase the depth of the middle ear was devised.

Alkaptonuria: a case report.

Alkaptonuria is a rare, autosomally recessive, metabolic disorder caused by a deficiency in homogentisic acid oxidase. It results in accumulation and deposition of homogentisic acid in cartilage, eyelids, forehead, cheeks, axillae, genital regions, nail beds, buccal mucosa, larynx, tympanic eardrum, and the tendons. We report a 33-year-old woman who presented with alkaptonuria and ochronotic pigment deposited in articular cartilage and cartilage of the ear and sclera.
