Eligibility of out-of-hospital cardiac arrest patients for extracorporeal cardiopulmonary resuscitation in the United States: A geographic information system model.

BACKGROUND: Recent evidence suggest that extracorporeal cardiopulmonary resuscitation (ECPR) may improve survival rates for nontraumatic out-of-hospital cardiac arrest (OHCA). Eligibility criteria for ECPR are often based on patient age, clinical variables, and facility capabilities. Expanding access to ECPR across the U.S. requires a better understanding of how these factors interact with transport time to ECPR centers. METHODS: We constructed a Geographic Information System (GIS) model to estimate the number of ECPR candidates in the U.S. We utilized a Resuscitation Outcome Consortium (ROC)

fNIRS-based neurofeedback targeting the right dorsolateral prefrontal cortex (dlPFC) in children at familial risk for substance use disorder and/or severe mental illness: a proof-of-concept study from eastern India.

BACKGROUND: Children who have first-degree relatives diagnosed with substance use disorders and/or severe mental illnesses show altered prefrontal cortical development and an elevated risk of executive function impairment. fNIRS-based neurofeedback (NF) is a portable, child-tolerant technique for the modulation of cortical hemodynamics, with preliminary evidence of dlPFC-dependent cognitive benefits in adult populations. However, evidence evaluating its efficacy in pediatric at-risk cohorts from the Indian subcontinent is currently lacking. METHODS: This single-arm, uncontrolled proof-of-conc

Impact of community-initiated Kangaroo Mother Care on survival of low birth weight infants: study protocol for a randomized controlled trial.

BACKGROUND: Around 70% neonatal deaths occur in low birth weight (LBW) babies. Globally, 15% of babies are born with LBW. Kangaroo Mother Care (KMC) appears to be an effective way to reduce mortality and morbidity among LBW babies. KMC comprises of early and continuous skin-to-skin contact between mother and baby as well as exclusive breastfeeding. Evidence derived from hospital-based studies shows that KMC results in a 40% relative reduction in mortality, a 58% relative reduction in the risk of nosocomial infections or sepsis, shorter hospital stay, and a lower risk of lower respiratory tract

South Asian women with diabetes: Psychosocial challenges and management: Consensus statement.

Diabetes is the ninth leading cause of death in women globally. In South Asians mortality in women with diabetes stands second highest. There is a marked gender discrimination which is faced by women across South Asia esp in access to services and support for diabetes, resulting in high rates of morbidity and mortality in women with diabetes. The most important risk factor identified for the diabetes epidemic is obesity along with genetic susceptibility. Lack of health care, social and cultural disparity, discrimination at work, disparity in marriage, restricted medical facilities are prevalen

Shifting perspectives: diphenhydramine usage in anaphylaxis and urticaria across US emergency departments.

BACKGROUND: In recent years, the medical community's view on diphenhydramine for urticaria and anaphylaxis has shifted due to its side effects and the efficacy of second-generation antihistamines. The AAAAI's 2020 anaphylaxis guidelines discourage the use of first-generation antihistamines in the acute phase or to prevent biphasic reactions. OBJECTIVE: Our study aimed to evaluate the usage of diphenhydramine in US emergency departments (EDs) for treating anaphylaxis and urticaria between 2019 and 2021, while also investigating the potential impact of the 2020 AAAAI guidelines on its utilizati

HIV self-testing lived experiences of female sex workers in the Garden City, Eastern Nigeria.

BACKGROUND: HIV self-testing (HIVST) provides a private and convenient alternative to traditional testing, addressing barriers such as stigma, fear of discrimination, and accessibility challenges. Female sex workers (FSWs) face a disproportionate burden of HIV and limited access to facility-based testing, making HIVST a critical tool for advancing Joint United Nations Programme on HIV/AIDS (UNAIDS) 2030 targets. OBJECTIVES: To explore the lived experiences of FSWs with HIVST and identify strategies to enhance its uptake and accessibility in Nigeria. DESIGN: This was a qualitative study utili

Comparative Analysis of Instrumental Variables on the Assignment of Buprenorphine/Naloxone or Methadone for the Treatment of Opioid Use Disorder.

BACKGROUND: Instrumental variable (IV) analysis provides an alternative set of identification assumptions in the presence of uncontrolled confounding when attempting to estimate causal effects. Our objective was to evaluate the suitability of measures of prescriber preference and calendar time as potential IVs to evaluate the comparative effectiveness of buprenorphine/naloxone versus methadone for treatment of opioid use disorder (OUD). METHODS: Using linked population-level health administrative data, we constructed five IVs: prescribing preference at the individual, facility, and region lev

The use of hospital-based nurses for the surveillance of potential disease outbreaks.

OBJECTIVE: To study a novel surveillance system introduced in Mpumalanga Province, a rural area in the north-east of South Africa, in an attempt to address deficiencies in the system of notification for infectious conditions that have the potential for causing outbreaks. METHODS: Hospital-based infection control nurses in all of Mpumalanga's 32 public and private hospitals were trained to recognize, report, and respond to nine clinical syndromes that require immediate action. Sustainability of the system was assured through a schedule of regular training and networking, and by providing feedb

Evaluating Simulation-Based Tobacco Treatment Scenarios for Providers Delivering Treatment for People Living With Mental Illnesses.

BACKGROUND: People living with mental illnesses (PMI) experience elevated tobacco use and related morbidity and mortality. Despite the availability of effective and safe tobacco treatments along with evidence that PMI are motivated and able to quit successfully, few Mental and behavioral healthcare providers (MHPs) engage PMI in such treatment. MHPs may lack the confidence or skills to engage their clients in tobacco treatment. Currently, there are limited training modalities to prepare MHPs in delivering tobacco treatment for PMI. However, animated scenario-based simulated encounters can brid

Personalised neoantigen vaccines for precision oncology in Southeast Asia: clinical opportunities, infrastructure challenges, and policy perspectives.

Personalised neoantigen vaccines are patient-specific immunotherapies designed to target tumour-restricted antigens arising from genomic and transcriptomic alterations, including single-nucleotide variants, insertions and deletions, frameshifts, gene fusions and aberrant splicing. Early clinical studies have established manufacturing feasibility and immunogenicity, while randomised phase 2b evidence in resected melanoma has provided a signal of clinical activity that requires confirmation in larger trials. Clinical development nevertheless remains concentrated in high-income academic centres,

Family caregiver education programmes for safe home care: a scoping review.

BACKGROUND: Population ageing and the rise in chronic diseases are driving a shift from residential models to home care where family carers play a key role. Although education programmes have shown benefits, limited attention has been paid to how family caregivers can be effectively trained to prevent unintentional mistakes in home care and to how they should be prepared to respond to them related to the care they provide. OBJECTIVES: This scoping review aims to examine education programmes that enhance family caregivers' knowledge and skills to promote safe home care, prevent unintentional m

Project ACHIEVE - using implementation research to guide the evaluation of transitional care effectiveness.

BACKGROUND: Poorly managed hospital discharges and care transitions between health care facilities can cause poor outcomes for both patients and their caregivers. Unfortunately, the usual approach to health care delivery does not support continuity and coordination across the settings of hospital, doctors' offices, home or nursing homes. Though complex efforts with multiple components can improve patient outcomes and reduce 30-day readmissions, research has not identified which components are necessary. Also we do not know how delivery of core components may need to be adjusted based on patien

Saturation sampling for spatial variation in multiple air pollutants across an inversion-prone metropolitan area of complex terrain.

BACKGROUND: Characterizing intra-urban variation in air quality is important for epidemiological investigation of health outcomes and disparities. To date, however, few studies have been designed to capture spatial variation during select hours of the day, or to examine the roles of meteorology and complex terrain in shaping intra-urban exposure gradients. METHODS: We designed a spatial saturation monitoring study to target local air pollution sources, and to understand the role of topography and temperature inversions on fine-scale pollution variation by systematically allocating sampling lo
