Mobile encapsulated adipose tissue (MEAT) of cows and humans: a distinct nonneoplastic entity.

Mobile encapsulated adipose tissue can be found in both the subcutis and peritoneal cavity. The cutaneous lesions are more common and better described; they are usually designated as "mobile encapsulated lipoma" or "nodular-cystic fat necrosis." The clinical name of abacus tumor describes best the small marble-like nodules that often can be moved freely through the subcutaneous tissue planes. Histopathologically, the nodules are composed of a dense fibrous capsule surrounding fat that may show varying degrees of necrosis, calcification, and lipomembranous changes. The peritoneal nodules are th…

Sclerosing mesenteritis, mesenteric panniculitis and mesenteric lipodystrophy: a single entity?

We reviewed 84 cases coded as mesenteric lipodystrophy (ML), mesenteric panniculitis (MP), or retractile mesenteritis and sclerosing mesenteritis (SM), grading fibrosis, inflammation, and fat necrosis, and evaluating clinical subgroups. There was no gender or racial predominance. Patient age range was 23-87 years (average 60). Patients most often presented with abdominal pain or a palpable mass. A history of trauma or surgery was present in four of 84 patients. The most common site of involvement was the small bowel mesentery as a single mass (58 of 84) with an average size of 10 cm, multiple …

Fat Hypertrophy as a Complication of Fat Transfer for Hemifacial Atrophy.

Fat hypertrophy is a less commonly known complication of autologous fat transfer. We present a 32-year-old female with left hemifacial atrophy associated with systemic sclerosis, who was treated with 7 fat transfer procedures to correct the facial asymmetry. A total of 236.5 mL of fat was injected to the hemiface over a 4-year period to achieve good symmetry. A progressively enlarging, painless, soft mass over the left parotid region was noted at 3 months after the final fat transfer procedure. Magnetic resonance imaging showed a markedly enlarged bulk of subcutaneous fat over the left cheek w

Giant post-traumatic liponecrotic granuloma of the gluteal region: A case report with multimodality imaging findings.

Post-traumatic liponecrotic granuloma is a benign soft-tissue lesion resulting from adipose tissue necrosis after blunt trauma. Although post-traumatic fat necrosis is a recognized entity, large encapsulated lesions with a pseudotumoral appearance are rare and may closely mimic soft-tissue neoplasms, creating a diagnostic challenge. In this setting, multimodality imaging is essential for accurate lesion characterization and appropriate patient management. We report the case of a 40-year-old woman presenting with a progressively enlarging left gluteal mass that developed over 5 years following …

Dermal Fat Graft for Preventing Extrusion in Tibial Bone Osteo-Keratoprosthesis Exposure.

PURPOSE: Tibial bone keratoprostheses are used in situations where conventional corneal transplantation is not viable. However, the prognosis is often guarded in cases of ocular surface diseases, such as autoimmune conditions or chemical burns, because of the risk of tissue necrosis and subsequent prosthesis extrusion. In this study, we describe an innovative approach using a dermal fat graft to manage keratoprosthesis (KPro) exposure and avoid extrusion. METHODS: We report the case of a man with a history of bilateral chemical burns caused by caustic soda, who underwent tibial bone keratopro

[Pancreatic panniculitis with neuroendocrine pancreatic carcinoma].

Pancreatic panniculitis is a rare complication occurring in 0.3-3% of patients with pancreatic disease. Clinical features include erythematous painful subcutaneous nodules usually on the lower leg and foot. A 72-year-old man was diagnosed in 2002 with a neuroendocrine carcinoma of the head of the pancreas. In 2003 following surgery and radiation therapy, he developed liver metastases and painful nodules on his legs. Lipase was found to be markedly elevated; amylase and alpha1-antitrypsin were in the normal range. Histopathologic examination of a nodule showed subcutaneous fat necrosis with gho…

Mesenteric panniculitis of the sigmoid colon. Report of two cases.

Mesenteric panniculitis is an extremely rare disease in which the normal fatty architecture of the mesentery is replaced by fibrosis, necrosis, and calcification. Grossly, the mesentery is massively thickened and rubbery, with irregular areas of reddish-brown to pale yellow foci resembling fat necrosis scattered throughout. Microscopically, inflammatory involvement of the fibroadipose tissue shows round cells, foam cells, and giant cells. Macrophages that have ingested fat, also called lipophages, are the hallmark of the diagnosis. One hundred twenty-two cases of mesenteric panniculitis have b…

Intraosseous lipomas: radiologic and pathologic manifestations.

Sixty-one cases of surgically treated solitary intraosseous lipoma were staged into three categories depending on the degree of involution present histologically: stage 1, tumors of viable fat cells; stage 2, transitional cases composed partly of viable fat cells but also demonstrating fat necrosis and calcification; and stage 3, lesions demonstrating necrotic fat, calcification of necrotic fat, variable degrees of cyst formation, and reactive woven bone formation. Each of these stages had radiologic features that could be correlated with the histopathologic findings in the excised tissue. Exa…

Osteomyelitis: A rare complication of pancreatitis and PPP-syndrome.

Pancreatic diseases can be accompanied by periarthritis with bone necrosis and panniculitis (PPP-syndrome). It is postulated that this is caused by systemic activity of pancreatic enzymes leading to microcirculatory disturbances and fat necrosis. The morbidity and mortality of the PPP-syndrome is high. Successful treatment of pancreatitis can lead to resolution of accompanying panniculitis and periarthritis without adverse sequelae, but weeks or months after pancreatitis, asymptomatic necrosis of the bone may become symptomatic by fracturing spontaneously. In this report, we also describe oste…

Visceral fat lipolysis by pancreatic lipases worsens heart failure.

Heart failure can be worse when associated with obesity, elevated serum pancreatic enzymes, elevated non-esterified fatty acids (NEFAs), or acute pancreatitis (AP). To understand this, here we study doxorubicin-induced heart failure, experimental AP, or pancreatic lipase-induced visceral fat necrosis in lean, genetically obese (ob/ob), or dual ob/ob pancreatic triglyceride lipase (PNLIP)-knockout mice. NEFA generation and resulting cardiac injury are measured. We note that ob/ob mice develop fat necrosis containing PNLIP and phospholipase A2. This generates excess NEFAs that worsen cardiac inj…

Fatty lesions in intra-articular loose bodies: a histopathological study of non-primary synovial chondromatosis cases.

Intra-articular loose bodies (ILBs) are not uncommon conditions in patients with various joint diseases. Their morphological alterations have been investigated in detail, but little attention has been paid to their fatty lesions. In this study, we examined fatty bone marrow, fat cells without bone marrow structures (extramedullary fat cells), and their necrotic changes in 55 ILBs surgically removed from 42 patients, excluding primary synovial chondromatosis cases. The presence of viable re-vascularized vessels with or without enchondral ossification could discriminate 19 re-attached ILBs from

[Adiponecrosis subcutanea neonatorum in congenital pneumonia].

In the course of a connatal pneumonia, a 7-day-old female newborn developed symmetrical subcutaneous nodules on her back, shoulders, and upper arms. These skin lesions were accompanied by hypercalcemia. Histological examination confirmed the putative clinical diagnosis of subcutaneous fat necrosis of the newborn. We discuss the differential diagnoses, therapeutic strategies, and prognosis of this uncommon disorder of the fat tissue.

A firm plaque on the back of a newborn.

We report a case of subcutaneous fat necrosis of the newborn (SCFN), a rare disorder in term or post-term neonates. Although it is often associated with hematological abnormalities such as anemia and hypercalcemia, SCFN in this patient presented with hyperbilirubinemia. The course of SCFN is generally benign and self-limiting, though may be associated with complications secondary to hypercalcemia.

Extra-hepatic biliary-ductal necrosis in acute pancreatitis: a rare complication.

An 8-year-old male presented with an acute abdomen. Exploration revealed bilious ascites, oedematous pancreatitis with areas of necrosis, and omentum showing patches of fat necrosis. The common bile duct and almost all of the common hepatic duct were gangrenous and had sloughed, with bile leaking from the junctional stump of the right and left hepatic ducts. The patient was managed successfully by Roux-en-Y hepaticojejunostomy. This is a rare case showing necrosis and sloughing of the extrahepatic bile ducts in acute pancreatitis.
