Intramedullary osteoblastoma of the femur: a case report.

INTRODUCTION: Osteoblastoma is a rare primary bone tumour, and the lesions are typically intracortical affecting long-bone metaphysis and the posterior spine. The histological hallmark is characteristic networks of trabeculated woven bone, usually lined by osteoblasts within a vascularised stroma. This case report details an unusual case of an intramedullary osteoblastoma of the femur. CASE PRESENTATION: A 22-year-old male presented with an eight-month history of right knee pain on a background of a previous distal tibia non-ossifying fibroma. The patient underwent cross-sectional imaging, an

Solitary osteochondroma of the proximal femur causing sciatic nerve compression.

UNLABELLED: Osteochondromas (OC) are the most common benign tumors of the bones. They are most frequently found near the knee joint and are typically asymptomatic. The presence of OC near the hip joint is rare. In addition, OC is an extremely uncommon cause of sciatica. We present a rare case of OC of the proximal femur. The patient presented with left hip pain and manifestations of sciatic nerve compression. Surgical excision was performed. The diagnosis was confirmed postoperatively by histopathology. SIMILAR CASES PUBLISHED: 1.

[Bone liposclerosing myxofibrous tumor. Case presentation and literature review].

The bone liposclerosing myxofibrous tumor (LSMFT) was initially described by Ragsdale in 1986 as a polymorphic fibroosseous bone lesion with a mix of histologic elements that include lipoma, fibroxanthoma, myxoma, myxofibroma, fat necrosis, ischemic ossification, areas of fibrous dysplasia, and infrequent presence of cartilage or cystic changes. The most frequently reported location is the intertrochanteric area of the femur. Radiologically it is a lytic, geographic lesion, with well-defined margins and usually sclerotic. In some cases findings include mineralization inside the lesion or a cer

Osteoid osteoma of the femur in a 7-month-old infant treated with radiofrequency ablation.

Osteoid osteoma occurs most commonly in children, adolescents, and young adults between the ages of 5 and 30 years. In the preschool age group, it is quite uncommon, accounting for only 3-8% of all osteoid osteoma cases. We report a case of osteoid osteoma in a 7-month-old infant, who presented with decreased use of the right lower extremity due to pain. Magnetic resonance imaging (MRI) showed an atypical appearance. A biopsy of the lesion, with histopathological examination, confirmed the diagnosis of osteoid osteoma. Radiofrequency ablation (RFA) of the nidus under computed tomography (CT) g

Fungal infection by Paracoccidioides brasiliensis mimicking bone tumor.

Paracoccidioides brasiliensis infection causes a systemic mycosis originally described in Latin America but with current reports of worldwide distribution. The clinical presentation of paracoccidiodomycosis as an isolated long-bone lesion in children is quite unusual. This article describes a 10-year-old male with a lytic femoral bone lesion caused by P. brasiliensis infection that was first suspected of being of neoplasic etiology. The text also emphasizes the importance of including endemic fungal infections in the differential diagnosis of bone lesions.

Bone cancer pain.

BACKGROUND: Bone cancer pain is very common, and patients with this type of pain may be difficult to treat. Development of an experimental model for studying this condition is critical to advancing an understanding of the mechanisms that cause pain in patients with malignant disease. METHODS: A murine model of bone cancer was studied. Combined analysis of the extent of tumor-induced bone destruction, pain, and neurochemical characterization of the peripheral and central nervous systems was performed to investigate bone cancer pain. Disease-induced bone destruction was assessed by radiographs

Periosteal osteosarcoma.

An analysis of 6 patients with periosteal osteosarcoma treated by the authors along with a review of 55 patients reported in the literature demonstrates that periosteal osteosarcoma is distinctly different from conventional osteosarcoma or periosteal chondrosarcoma. Periosteal osteosarcoma is a less aggressive tumor than conventional osteosarcoma. It is a relatively well-differentiated chondroblastic osteosarcoma occurring on the surface of the long bones of the extremities. Three patients demonstrated frank medullary invasion of tumor, two grossly and one microsurgically. Patients treated wit

Chondroblastoma: a clinical and radiological study of 104 cases.

The clinical and radiographic findings in 104 patients with chondroblastoma are presented. Pain was an almost constant presenting complaint, often accompanied in the case of para-articular lesions by impaired function of an adjacent joint. The majority (80%) were in long bones with a mean age of presentation of 16 years. The characteristic radiological image of these lesions was an eccentric radiolucency, having a sharply defined sclerotic margin and containing areas of calcification in approximately a third of cases. They were always related to a growth plate. Nearly half were confined to the

Osteogenic sarcoma: the past, present, and future.

The term osteogenic sarcoma (osteosarcoma) is applied to malignant bone-forming tumors, identifiable by the matrix produced, though the histologic pattern may differ greatly. Ths more cellular (osteolytic) forms of the tumor have the poorer prognosis. Other prognostic factors are 1) site of the primary tumor; 2) duration of symptoms; and 3) extent of disease and tumor size. The site of tumor origin is the metaphyseal side of the epiphyseal line. The histogenesis of the tumors accounts for this distribution. Following a diagnostic biopsy, amputation of the extremities remains the treatment of c

The burden of metastatic disease of the femur on the Medicare system.

BackgroundIn the United States, over 1,650,000 new cases of cancer are being diagnosed yearly with almost 50 % of them being the top five bone-seeking cancers. Since cancer risk increases with age, this suggests that orthopedic oncology services may be a strain on the Medicare system. The femur is the most common site of long bone metastases. Prophylactic fixation techniques prevent pathologic fractures, reduce morbidities, and enhance the quality of life of patients with femoral metastases. This study aims to assess the rate of metastatic disease to the skeleton and evaluate the use and finan

Comparison of the clinical outcomes of intramedullary nailing between impending and completed pathological fractures caused by metastatic femoral tumors from solid cancers.

BACKGROUND: We examined the treatment outcomes following intramedullary nailing in patients with metastatic femoral tumors, excluding those from hematological malignancies. METHODS: We retrospectively evaluated treatment outcomes following intramedullary nailing between patients who underwent preventative surgery compared with those who had surgery following pathological fracture. Patients who underwent preventative surgery (Mirels' score ≥8) were allocated to the impending fracture group (n = 11) and those who underwent surgery after pathological fracture were allocated to the completed frac

Combined Laparoscopic and Open Surgical Approach for Well Differentiated Trans-compartmental Liposarcoma Between Pelvic and Femoral Regions.

BACKGROUND: Soft tissue sarcomas are rare neoplasms, accounting for only 1% of all solid tumors in adults. Liposarcomas represent the most common histological subtype,1 with aggressive local invasiveness and poor prognosis.2 When feasible, resection achieving negative surgical margins remains the only potentially curative treatment.3 Advances in laparoscopic surgical techniques have broadened the indications for minimally invasive oncologic procedures, particularly in cases involving retroperitoneal structures.4 These developments support the consideration of laparoscopy in selected retroperit

Clinical outcomes of cement gun-assisted intramedullary nailing for femoral bone metastases: a retrospective case series.

BACKGROUND: Surgical stabilization for femoral bone metastases is commonly performed to prevent or treat pathological fractures; however, local tumor progression may result in implant failure and reoperation. Cement gun-assisted intramedullary nailing (CG-IMN) allows extensive polymethylmethacrylate (PMMA) filling within the medullary canal, providing enhanced mechanical stability and facilitating early weight-bearing. This study aimed to evaluate the clinical outcomes of CG-IMN for femoral bone metastases. METHODS: We retrospectively reviewed 11 consecutive patients who underwent CG-IMN for

Unreamed femoral nail with spiral blade in subtrochanteric fractures: experience of 55 cases.

OBJECTIVE: To evaluate the use of the unreamed femoral nail with spiral blade (UFN-SB) in the treatment of subtrochanteric femoral fractures. DESIGN: A retrospective review of a consecutive series of 55 fractures. Fourteen patients had metastatic disease (four had prophylactic nailing). RESULTS: In five fractures, the UFN-SB failed: there was migration in three cases and breakage of the spiral blade in two cases, with breakage of the nail in two cases. Revision surgery was necessary in four cases. Five out of seven complications related to the spiral blade were seen in patients with a Seinsh

Uncommon clinical manifestations in a case of thyroid carcinoma.

A case of multiple, functional bone metastases from a previously operated follicular thyroid carcinoma is reported in an apparently euthyroid patient. Additional uncommon clinical manifestations encountered in this case were: 1) mandibular metastasis mimicking an intraoral arteriovenous malformation, 2) extensive, destructive, and expansive intraspinal metastases in the thoracic vertebral column protruding into the posterior mediastinum and causing severe spinal lesions, and 3) the precipitation of a fatal thyroid crisis following radioactive iodine treatment.

Clinical outcome of percutaneous osteoplasty for pain caused by metastatic bone tumors in the pelvis and femur.

We examined the pain-relieving effects in five patients who underwent percutaneous osteoplasty (POP) for pain caused by metastatic bone tumors in the pelvis and femur. Pain intensity, improvement of walking, and complications associated with POP were evaluated. Pain was measured using a numerical rating scale (NRS), with scores ranging from 0 (no pain) to 10 (worst pain imaginable). The patients were four men and one woman (mean age, 65 years). POP was performed in the pelvis in three patients and in the femur in two patients. Three of the subjects had already received radiation therapy at the
