Individualized Perioperative Blood Pressure and Fluid Therapy in Esophagectomy: A Prospective, Single-blind Randomized Controlled Trial.

BACKGROUND: Esophagectomy is a key treatment for esophageal cancer but carries a high risk of postoperative complications, some of which are potentially preventable through optimized hemodynamic management. Goal-directed fluid therapy individualizes cardiac output targets but often applies fixed blood pressure thresholds and is discontinued before major postoperative fluid shifts occur. Extending goal-directed fluid therapy into the postoperative period with individualized blood pressure thresholds may address these limitations. METHODS: In this single-center, prospective, blinded, randomized…

Impact of fluid removal rate on patients receiving continuous renal replacement therapy for acute kidney injury.

BACKGROUND: Fluid accumulation in severe acute kidney injury (AKI) has been associated with dialysis dependence and mortality. Less is known about the impact of fluid removal and its optimal rate with continuous dialysis. METHODS: We conducted a single-center retrospective cohort study amongst adult patients admitted to the intensive care unit at the University of Chicago with AKI treated with continuous veno-venous hemodialysis (CVVHD) from April 1, 2016 to March 31, 2020. We collected baseline demographics, daily fluid balances, and patient outcomes. Weight adjusted fluid removal rates were

No increased risk of spinal cerebrospinal fluid leak after spinal manipulative therapy: A retrospective cohort study.

BACKGROUND: Spinal cerebrospinal fluid (CSF) leaks, a rare but debilitating condition, have been described following spinal manipulative therapy (SMT) in case reports. However, the nature of the potential association between SMT and CSF leak is uncertain, and symptoms such as neck pain or headache may reflect preexisting leaks rather than effects of SMT. OBJECTIVE: To test the hypothesis of no significant association between SMT and spinal CSF leak, comparing chiropractic SMT recipients to matched controls undergoing physical therapist-supervised therapeutic exercises (PTE). METHODS: We sear

Neurosymptomatic Cerebrospinal Fluid Escape Is a Central Nervous System-Focused Form of Antiretroviral Therapy Treatment Failure.

BACKGROUND: During antiretroviral therapy, people with HIV-1 (PWH) occasionally present with new symptoms and signs of central nervous system (CNS) injury accompanied by higher HIV-1 RNA levels in cerebrospinal fluid (CSF) than in blood, a condition defined as neurosymptomatic CSF escape (NSE). PWH meeting these general criteria but with plasma HIV-1 RNA levels >500 copies/mL have typically been assumed to be experiencing systemic treatment failure without special consideration for HIV-1 dynamics in the CNS. Thus, people with CNS-focused treatment failure may receive suboptimal treatment, targ

FLUID OVERLOAD MODIFIES HEMODYNAMIC IMPACT OF CONTINUOUS RENAL REPLACEMENT THERAPY: EVIDENCE OF A COVERT CARDIORENAL SYNDROME?

Background: Fluid overload (FO) in critically ill children correlates with higher morbidity and mortality rates. Continuous renal replacement therapy (CRRT) is commonly employed to manage FO. In adults, both FO and CRRT adversely affect myocardial function. It remains unclear if children experience similar cardiovascular effects. Methods: Observational single-center study on children (<18 years) receiving CRRT at Texas Children's Hospital from 11/2019 to 3/2021. Excluded were those with end-stage renal disease, pacemakers, extracorporeal membrane oxygenation, ventricular assist devices, aphere

Human amniotic fluid stem cell therapy can help regain bladder function in type 2 diabetic rats.

BACKGROUND: Diabetes mellitus (DM) is a serious and growing global health burden. It is estimated that 80% of diabetic patients have micturition problems such as poor emptying, urinary incontinence, urgency, and urgency incontinence. Patients with diabetic bladder dysfunction are often resistant to currently available therapies. It is necessary to develop new and effective treatment methods. AIM: To examine the therapeutic effect of human amniotic fluid stem cells (hAFSCs) therapy on bladder dysfunction in a type 2 diabetic rat model. METHODS: Sixty female Sprague-Dawley rats were divided in

Transient accumulation of subretinal fluid after half-fluence photodynamic therapy in neovascular age-related macular degeneration.

BACKGROUND: Photodynamic therapy (PDT) is known to occlude choroidal neovascularisation selectively, and there have been several reports on its adverse effects on the normal choroid and retinal pigment epithelium, resulting in decreased vision. METHODS: This retrospective interventional case series aimed to investigate the changes in visual acuity and retinal thickness in the immediate post-treatment period after half-fluence PDT, administered alone or with anti-vascular endothelial growth factor and steroids, in 29 eyes (26 patients) with neovascular age-related macular degeneration. The pat

Assessment of dispersion of airborne particles of oral/nasal fluid by high flow nasal cannula therapy.

BACKGROUND: Nasal High Flow (NHF) therapy delivers flows of heated humidified gases up to 60 LPM (litres per minute) via a nasal cannula. Particles of oral/nasal fluid released by patients undergoing NHF therapy may pose a cross-infection risk, which is a potential concern for treating COVID-19 patients. METHODS: Liquid particles within the exhaled breath of healthy participants were measured with two protocols: (1) high speed camera imaging and counting exhaled particles under high magnification (6 participants) and (2) measuring the deposition of a chemical marker (riboflavin-5-monophosphat

Amniotic fluid and breast milk: a rationale for breast milk stem cell therapy in neonatal diseases.

Amniotic fluid and breast milk play important roles in structural development throughout fetal growth and infancy. Given their significance in physical maturation, many studies have investigated the therapeutic and protective roles of amniotic fluid and breast milk in neonatal diseases. Of particular interest to researchers are stem cells found in the two fluids. These stem cells have been investigated due to their ability to self-replicate, differentiate, reduce tissue damage, and their expression of pluripotent markers. While amniotic fluid stem cells have received some attention regarding t

Transplantation of amniotic fluid-derived neural stem cells as a potential novel therapy for Hirschsprung's disease.

BACKGROUND/PURPOSE: We have previously shown that embryonic enteric neural stem cells (NSCs) isolated from the intestine colonize aganglionic intestine upon transplantation, but posttransplantation cell survival limits efficacy. The aims of this study were to investigate whether transplantation of amniotic fluid (AF)-derived NSCs could improve survival of the engrafted cells and promote functional recovery of the diseased colon. METHODS: AF cells were induced into NSCs with neurogenic medium, and further differentiated into neurons and glial cells. Ednrb knockout mice received an intestinal i

Effects of antimicrobial photodynamic therapy on transforming growth factor-β1 levels in the gingival crevicular fluid.

BACKGROUND AND OBJECTIVE: Antimicrobial photodynamic therapy (aPDT) has been proposed as an adjunctive therapy to scaling and root planing (SRP). The transforming growth factor-β1 (TGF-β) has been considered as an anti-inflammatory cytokine, and its levels in the gingival crevicular fluid (GCF) could monitor the periodontal repair. This study evaluated the adjunct effect of aPDT compared with SRP, analyzing the TGF-β levels in GCF after nonsurgical and surgical therapy in chronic periodontitis patients. METHODS: Fifteen patients, presenting bilaterally lower molars with class III furcation le

Choroidal Proteins Involved in Cerebrospinal Fluid Production may be Potential Drug Targets for Alzheimer's Disease Therapy.

Alzheimer's disease is known to be the most common form of dementia in the elderly. It is clinically characterized by impairment of cognitive functions, as well as changes in personality, behavioral disturbances and an impaired ability to perform activities of daily living. To date, there are no effective ways to cure or reverse the disease. Genetic studies of early-onset familial Alzheimer's disease cases revealed causative mutations in the genes encoding β-amyloid precursor protein and the γ-secretase-complex components presenilin-1 and presenilin-2, supporting an important role of β-amyloid

[Neovascular age-related macular degeneration under anti-angiogenic therapy : Subretinal fluid is a relevant prognostic parameter].

BACKGROUND: Due to its progressive nature neovascular age-related macular degeneration (nAMD) is the most common cause of irreversible vision loss and blindness in people aged 60 or older. Optical coherence tomography (OCT) plays an important role in monitoring this exudative disease. However, prognostic factors for visual rehabilitation under anti-angiogenic therapy are still rare. It was the aim of this study to identify prognostic parameters based on OCT. METHODS: In this study 17 patients with nAMD were included and treated with 3 intravitreal ranibizumab injections. Stratus-OCT and Cirru
