INTRODUCTION: Risks associated with drill plunging are well recognised in clavicle osteosynthesis. To date no studies have described plunge depth associated with clavicle osteosynthesis.
PRIMARY AIM: To determine whether plunge depth associated with clavicle osteosynthesis is great enough to penetrate neurovascular structures and whether surgical experience reduces the risk of neurovascular injury METHOD: Cadaveric clavicles were pressed into spongy phenolic foam to allow measurement of drill bit penetration beyond the far cortex (plunge depth). 15 surgeons grouped according to experience wer

OBJECTIVE: The development of osteoarthritis after intra-articular fractures has been described for decades, although the exact mechanical and cellular changes that occur remain poorly understood. There are several animal models to study this phenomenon, but they are mechanistically different from physiologic fractures in several important ways. This article describes a novel model that recreates the kinematics present in high-energy trauma and intra-articular fractures.
METHODS: We designed a "drop tower" for the creation of intercondylar femoral fractures in rats and tested it on cadaveric

Arteriovenous fistula formation after a closed extremity fracture is rare. We present the case of an 11-year-old boy who developed an arteriovenous fistula between the anterior tibial artery and popliteal vein after closed fractures of the proximal tibia and fibula. The fractures were treated by closed reduction and casting. A fistula was diagnosed 12 weeks after the injury. It was treated by embolisation with coils. Subsequent angiography and ultrasonography confirmed patency of the popliteal vein and anterior and posterior tibial and peroneal arteries, with no residual shunting through the f

We have investigated whether early anatomical open reduction and internal fixation (ORIF) reduces the incidence of complications of fracture of the femoral neck in children, including avascular necrosis, compared with closed reduction and internal fixation (CRIF). We retrospectively reviewed 27 such fractures (15 type-II and 12 type-III displaced fractures) in children younger than 16 years of age seen in our hospital between February 1989 and March 2007. We divided the patients into three groups according to the quality of the reduction (anatomical, acceptable, and unacceptable) and the clini

Pure tibiotalar dislocations without associated fracture are rare. The authors report a case of an unusual closed tibiotalar dislocation without fracture, involving proximal ascension of the talus. Despite immediate closed reduction, a tibiofibular diastasis remained, therefore two syndesmosis screws were inserted, followed by six weeks of plaster casting. After 5 years of follow-up, functional results were excellent, MRI showed slight arthritic degenerative changes.

We retrospectively reviewed the results of closed treatment in 7 patients with acute locked posterior fracture-dislocations of the shoulder. Shoulders were reduced with patients under general anesthesia and were splinted in neutral rotation for 6 weeks. Follow-up averaged 46 months (range, 24-88 months). All shoulders were reduced within 14 days. Humeral head defects ranged from 18% to 32% of the articular surface, and all patients had documented posterior instability under anesthesia after relocation. Of the 7 patients, 6 had a stable shoulder. The mean American Shoulder and Elbow Surgeons sh

We reviewed 42 patients (mean age 37.7+/-14.2 years) with closed fracture dislocations of Lisfranc's joint treated with percutaneous screw fixation. Mean follow-up was 58.4+/-17.3 months. The aim was to compare dislocations in which a perfect anatomical reduction had been reached with dislocations in which reduction was only near anatomical. The mean American Orthopaedic Foot and Ankle Society score for all patients was 81.0+/-13.5. There were no significant differences in outcome scores between patients with perfect anatomical reduction and patients with near anatomical reduction. However, pa

During the period 1986-1991 a total of 3159 patients with diagnosis of either subcapital, intertrochanteric or subtrochanteric fracture were registered in the County of Southern Jutland in order to make a prognosis for the next twenty years. Contrary to other investigations an exponential increase in fractures was not seen, but only an increase related to the rising number of elderly people in the population.

Axial force applied during epiphyseal distraction has been measured close to skeletal maturity in patients having leg lengthening, in a rabbit model, and in vitro from an amputation specimen. In the patient study, both slow distraction rates and low constant distraction loads were applied. For all the distraction regimens, it was not possible to lengthen the limb significantly without evidence of fracture as demonstrated by a sudden decrease in distraction force. Growth plate failure was observed from 600 to 800 N, these levels being lower than those recorded from the in vitro tests. In the an

PURPOSE: No study in literature clearly gives advantage of one method of surgical fixation of fracture over the other in metacarpal and phalangeal bones of hand comparing wider outcome measures.
MATERIALS AND METHODS: A randomized controlled trial between closed reduction and percutaneous Kirschner wire fixation or open reduction and internal fixation with mini fragment plates was conducted. A total of 32 patients with 16 in each group were taken in tertiary-level teaching hospital from 2014 to 2016. Four patients lost to follow-up apart from 32 studied. Inclusion criteria were age group 16-6

PURPOSE: Ankle fractures represent about 10% of all adult fractures, with increasing incidence. Dislocated ankle fractures often require delayed open reduction and internal fixation due to swelling, necessitating temporary stabilization using a cast or an external fixator. This study aims to assess risk factors for insufficient preliminary reduction immobilized by a cast, focusing on medial clearspace and posterior malleolus fragment size, to identify fractures that would benefit from initial stabilization with an external fixator.
METHODS: Patients treated for dislocated ankle fractures at o

BACKGROUND: Ankle fractures are common injuries associated with significant morbidity. A significant proportion of closed ankle fractures are displaced or unstable, requiring reduction. While early near-anatomic reduction is commonly performed, it is not known whether this affects long-term outcomes. This study aims to identify the association of reduction quality with outcomes in patients with displaced, closed ankle fractures. We hypothesize that patients with anatomic reductions will have fewer complications than patients who have suboptimal reductions.

BACKGROUND: Pediatric supracondylar humerus fractures (SCHFs) are among the most common childhood injuries, often requiring surgical fixation. Closed reduction with percutaneous pinning was the preferred initial management due to its minimally invasive nature. This study aimed to evaluate demographic, clinical, and radiographic predictors of failed closed reduction in Gartland type III and flexion-type pediatric SCHFs, emphasizing the role of preliminary reduction performed in the emergency department. We hypothesized that preliminary reduction in the emergency setting would reduce the risk of

Extra-octave fractures of the fifth proximal phalanx are Salter-Harris type II physeal injuries resulting in ulnar deviation and dorsal angulation. If inadequately reduced, these fractures may result in cosmetic deformity, malalignment, and functional impairment. Traditional reduction methods, such as the Jahss maneuver, can be technically demanding, forceful, and often require sedation or surgical assistance, which limits their use in younger children and in resource-constrained environments. We report our experience with the fulcrum-assisted "pencil technique" for closed reduction in four ch

Background Ankle fractures are prevalent lower limb injuries that often necessitate surgical intervention to restore joint stability and functionality. Delays in surgical management can extend hospital stays and elevate the risk of complications. This study aims to evaluate the effect of surgical timing on the postoperative length of hospital stay in patients with closed ankle fractures managed through open reduction and internal fixation (ORIF). Objective The objective of this study was to determine whether early surgical intervention (within eight hours of injury) reduces the mean postoperat

Background: The anatomical reconstruction of the wrist is the aim when treating distal radius fractures. Current literature on the importance of preoperative reduction in fractures that are treated operatively is limited. Methods: This study investigated the effect of the preoperative closed reduction of distal radius fractures on the day of trauma and the time to surgery on postoperative palmar inclination. A total of eighty patients (48 females and 32 males, mean age 55.6 years) were studied retrospectively. All patients were treated with an open reduction and internal fixation. The palmar i
