Delayed Massive Hemothorax Induced by Tunneled Dialysis Catheter Placement via the Left Internal Jugular Vein: A Case Report.

The tunneled dialysis catheter is among the most commonly used vascular accesses for patients undergoing hemodialysis. Delayed hemothorax resulting from central venous perforation induced by catheterization is a rare but serious complication. However, there are few reports. This article reports the case of a 79-year-old male who developed delayed massive hemothorax following catheter placement via the left internal jugular vein. The condition was successfully managed with closed thoracostomy and conservative therapy. This case underscores the potential for delayed hemothorax after tunneled dia…

Delayed Hemothorax Following Trauma in a COVID-19 Positive Patient Requiring Video-Assisted Thoracoscopic Surgery: A Case Report.

Delayed hemothorax (DHTX) is a possible sequelae of thoracic trauma, especially in the setting of patients being treated with anticoagulation. We report the case of an 81-year-old Caucasian man with a DHTX presenting 14 days following an initial emergency department (ED) visit with multiple rib fractures due to a fall from the patient's bed. Upon presentation to the ED a second time, the patient was hospitalized, tested positive for COVID-19, and on the second day of admission underwent video-assisted thoracoscopic surgery (VATS) without bleeding or other complications. This case illustrates t…

A rare case of bilateral massive hemothorax from spontaneous rupture of a primary mediastinal mixed germ cell tumor.

Spontaneous rupture of a mediastinal germ cell tumor, while rare, is always accompanied by bleeding. In this report, we describe a case of a primary mediastinal mixed germ cell tumor that presented with bilateral massive hemothorax and hemorrhagic shock. An urgent thoracotomy, which was performed to control bleeding, confirmed bilateral hemothorax secondary to a ruptured mediastinal tumor. Pathologic diagnosis revealed the mediastinal tumor to be mixed choriocarcinoma and immature teratoma, with lung metastatic choriocarcinoma. The patient recovered well from the operation and received salvage…

[A case of hemothorax occurred two months after graft replacement of descending thoracic aorta with Hemashield arterial prosthesis].

A 70-year-old man performed graft replacement of descending thoracic aorta for chronic aortic dissection (DeBakey IIIb) with Hemashield arterial prosthesis. He also had coronary artery disease (LAD seg. 7.99% stenosis) and he had received PTCA prior to operation. He was administered anticoagulants such as Aspirin and Warfarin postoperatively. Two months after operation, he complained dyspnea and came back to our hospital. Chest X-ray showed left hemothorax. CT scan and aortogram could not revealed bleeding points such as pseudoanerysm at anastomosis site nor new dissection. He recovered after …

Diagnostic accuracy of transthoracic ultrasound for detection of pneumothorax, hemothorax, and lung contusion in trauma patients: comparison with chest radiography.

BACKGROUND: Thoracic trauma is a major cause of morbidity and mortality in trauma patients. Rapid detection of pneumothorax, hemothorax, and lung contusion is critical for timely management. This study aimed to evaluate the diagnostic accuracy of transthoracic ultrasound compared with chest radiography in thoracic trauma patients using chest computed tomography (CT) as the reference standard. METHODS: This prospective diagnostic accuracy study was conducted in the Emergency Department of Menoufia University Hospital between July 2023 and December 2024. Adult trauma patients with suspected tho…

Transcatheter arterial embolization for hemothorax caused by spinal fracture without arterial injury: a case report and review of the literature.

BACKGROUND: Spinal fractures rarely cause hemothorax, and no treatment consensus has been reached. Conservative treatment is generally selected in cases without arterial injury, but there have been some reports of uncontrolled bleeding. Here we report a case of hemothorax caused by spinal fracture without arterial injury treated with transcatheter arterial embolization. CASE PRESENTATION: An 88-year-old Japanese woman with back pain was diagnosed with hemothorax due to bleeding from an unstable fracture of the tenth thoracic vertebra. Contrast-enhanced computed tomography revealed no obvious …

Massive right hemothorax as the source of hemorrhagic shock after laparoscopic cholecystectomy - case report of a rare intraoperative complication.

A 62-year old man was referred to our institution in hemorrhagic shock after a laparoscopic cholecystectomy for acute cholecystitis, performed at an outside hospital. A chest X-ray revealed a right-sided massive pleural effusion. Urgent surgical exploration was performed through a video-assisted mini-thoracotomy which revealed active bleeding from a pleural adherence. Successful hemostasis was achieved intraoperatively and the patient had an uneventful recovery. In absence of intra-abdominal hemorrhage, a hemothorax should be considered as a potential source of major bleeding in patients who d…

Spontaneous hemothorax in a 28-day-old term neonate presenting with poor feeding and respiratory distress: A case report.

BackgroundSpontaneous hemothorax in neonates is extremely rare and usually presents acutely with respiratory distress. Most cases are associated with trauma, coagulopathy, or iatrogenic injury.Case presentationWe report a case of a 28-day-old term neonate who presented with poor feeding and respiratory distress. On examination, the infant was pale and tachypneic. Chest radiograph revealed a large left-sided pleural effusion, confirmed by ultrasound. Pleural fluid on drainage was bloody, consistent with hemothorax. There was no history of trauma, admission, or known bleeding disorder. The infan…

Emergency Endovascular Stent Grafting of an Innominate Vein Injury Causing Hemothorax: The Double Balloon on a Single Wire Technique.

PURPOSE: The VIABAHN® VBX is a versatile stent graft that can be overdilated by up to 30% of its nominal diameter. However, to reach its maximum diameter, the balloon must be exchanged for a larger one. This technical note aims to illustrate a technique designed to enhance accuracy during the deployment and overdilation of a VIABAHN VBX for treating an injury to the left innominate vein (LIV). TECHNIQUE: Two venous accesses were established in the left arm and in the right femoral vein. Using a rendezvous technique, a hydrophilic guidewire was snared and externalized from the arm. The balloon

A Case of Traumatic Hemothorax From an Isolated Thoracic Vertebral Fracture in an Elderly Patient on Combined Anticoagulant and Antiplatelet Therapy.

Traumatic hemothorax is typically easy to diagnose because of the distinct onset of trauma with significant complaints such as severe chest pains. However, in elderly patients, the clinical symptoms are less clear and the frequent use of antithrombotic therapy may prolong the bleeding from a minor fracture. We report a case of traumatic hemothorax from an isolated thoracic vertebral fracture in an elderly patient on anticoagulant and antiplatelet therapy. A 91-year-old male on anticoagulant and antiplatelet therapy was admitted to our hospital with a complaint of persistent hemoptysis after a …

Primary pleural epithelioid angiosarcoma manifesting as a loculated hemothorax: A case report and literature review focusing on CT findings.

Primary pleural angiosarcoma (PPA) is an extremely rare and clinically aggressive tumor. We report the case of a 66-year-old man having PPA with chest computed tomography (CT) scan showing a large oval-shaped, nonenhancing high attenuation cystic mass in the left hemithorax. Morphological and immunohistochemical findings supported the diagnosis of epithelioid angiosarcoma. Pleural angiosarcoma should be considered in the differential diagnosis of spontaneous hemothorax manifesting as high attenuation loculated pleural fluid on CT.

The Benefit of Ultrasound in Deciding Between Tube Thoracostomy and Observative Management in Hemothorax Resulting from Blunt Chest Trauma.

BACKGROUND: Hemothorax is most commonly resulted from a closed chest trauma, while a tube thoracostomy (TT) is usually the first procedure attempted to treat it. However, TT may lead to unexpected results and complications in some cases. The advantage of thoracic ultrasound (TUS) over a physical examination combined with chest radiograph (CXR) for diagnosing hemothorax1 has been proposed previously. However, its benefits in terms of avoiding non-therapeutic TT have not yet been confirmed. Therefore, this study is aimed to evaluate the severity of hemothorax in blunt chest trauma patients by us…
