Coronary artery bypass grafting for postinfarction angina pectoris.

The variables that predict increased operative risk and postoperative morbidity among patients who undergo coronary artery bypass grafting ( … ays (p less than 0.01). Significantly more patients undergoing emergent CABG required postoperative intra-aortic balloon pumping and inotropic support than those patients who underwent urgent or elective operation (23.8% vs. 4.6% and 52.4% vs. 10%, respectively) (p less than 0.01). Other studies confirm that preoperative left ventricular dysfunction, presence of cardiogenic s …

Percutaneous ventricular assist device placement during active cardiopulmonary resuscitation for severe refractory cardiogenic shock after acute myocardial infarction.

Cardiogenic shock after acute myocardial infarction is associated with a high mortality rate despite modern reperfusion methods and intra-aortic balloon pump support. For myocardial infarction patients in cardiogenic shock that is refractory to intra-aortic ballon pump counterpulsation and pressors (severe refractory cardiogenic shock), there are limited means to rapidly provide additional hemodynamic support. We present the case of a 49-year-old man who presented with an anterior wall acute myocardial infarction complicated by cardiogenic shock. After resuscitation and stabilization with intr

Successful Bentall Surgery in a Moyamoya Disease Patient with Intraoperative Brain Protection via IABP-Induced Pulsatile Flow and CO2 Management during Cardiopulmonary Bypass.

INTRODUCTION: Moyamoya disease is an occlusive cerebrovascular disorder in which maintaining cerebral blood flow during cardiac surgery is challenging. No reports have described Bentall surgery in such patients. We present a successful case using an integrated brain protection strategy combining intra-aortic balloon pumping (IABP)-induced pulsatile flow, carbon dioxide management, and moderate hypothermia. CASE PRESENTATION: A 51-year-old male with symptomatic moyamoya disease was referred for severe aortic regurgitation due to left coronary cusp leaflet shortening and a sinus of Valsalva ane…

Acute torrential mitral regurgitation during transcatheter aortic valve replacement: a case report.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is a minimally invasive approach to aortic valve replacement. However, critical ca … demonstrated the mechanisms of MR, which was successfully bailed out by left ventricular pacing and intra-aortic balloon pumping. CONCLUSIONS: MR can be seriously exaggerated by various and complicated mechanisms during TAVR and should be rapidly assessed and appropriately managed depending on its mechanisms.

Primary Percutaneous Coronary Intervention Followed by Valve Surgery for Acute Coronary Syndrome at Left Main Trunk Complicated With Severe Aortic Stenosis.

An 89-year-old woman appeared to have acute coronary syndrome at the left main trunk (LMT) complicated with severe aortic stenosis, moderate-severe mitral regurgitation, depressed left ventricular (LV) function, and multivessel disease. Because of sustained hypotension even under intra-aortic balloon pumping support during emergency coronary angiograhy, we performed primary percutaneous coronary intervention solely for the LMT lesion using a bare metal stent, leading to recovery from the shock state. On the second hospital day, based on our heart-team consensus, we performed aortic valve repla…

Ventricular septal rupture after delayed STEMI: A modern management approach.

BACKGROUND: Myocardial infarction associated ventricular septal rupture (VSR) is a potentially catastrophic complication. Though surgical repair remains the definitive treatment, outcomes are poor with high mortality rates.Case and Management: We present the case of a 62-year-old male who presented with a delayed STEMI leading to a VSR and cardiogenic shock. His management strategy included early percutaneous VSR closure and use of an intra-aortic balloon pump and inotropes. This served as a bridge to definitive surgical VSR patch repair while allowing hemodynamic stabilization, end-organ reco

Surgical treatment of ventricular septal defect following myocardial infarction: a case report.

Ventricular septal defect after myocardial infarction is a rare but often life-threatening mechanical complication. The keys of management are a prompt diagnosis of ventricular septal defect and an aggressive approach to stabilize patient's hemodynamics. Invasive monitoring, judicious use of inotropes and vasodilators, and an intra-aortic balloon pump are recommended for the optimal support of patient's hemodynamics. The best results are achieved if optimally medically managed patients survive at least 4 weeks before elective surgery necessary for scar formation in a friable infarcted tissue.

Mechanical Circulatory Support for Complex High-Risk Indicated Percutaneous Coronary Intervention: Network Meta-Analysis of Randomized Trials.

BACKGROUND: Mechanical circulatory support (MCS) is increasingly used during complex high-risk indicated percutaneous coronary intervention, yet whether different support strategies improve clinical outcomes remains uncertain. We sought to compare the efficacy and safety of individual MCS strategies with standard care and with each other at short- and long-term follow-up. METHODS: This frequentist random-effects network meta-analysis included randomized trials of patients undergoing nonemergent complex high-risk indicated percutaneous coronary intervention and comparing percutaneous left vent

Mechanical Circulatory Support in Patients Without Cardiogenic Shock Undergoing Percutaneous Coronary Intervention.

BACKGROUND: The use of mechanical circulatory support devices for high-risk percutaneous coronary intervention (PCI) has increased over the past decade despite limited data of benefit. We sought to examine the association between intravascular microaxial left ventricular assist device (LVAD) versus intra-aortic balloon pump use in patients without cardiogenic shock (CS) undergoing PCI. METHODS AND RESULTS: This retrospective study analyzed claims data from a large, insured population who underwent PCI without CS from April 1, 2016 to July 31, 2022. Using inverse probability treatment weightin

[Complications of IABP in the postoperative management of open-heart surgery--a review of 108 cases in the last 10 years].

During the past decade from May 1978 to July 1988, intraaortic balloon pumping (IABP) was used in a total of 108 patients following open-heart surgery at our department. Eleven complications depend on IABP have occurred in nine of theses patients, i.e. five of circulatory disorder on account of ischemia of the lower extremity ipsilateral to balloon catheter insertion, three of ischemia of the abdominal viscera, one of aorto-esophageal fistula, one of gas embolism resulting of balloon rupture and one of damage of the abdominal aorta caused by a Fogarty balloon catheter. Five of these patients d

A counterpulsation technique for use during complicated aortic surgery with residual thoracoabdominal aortic aneurysm.

Mechanical circulatory support can be difficult for patients with thoracoabdominal aortic disease. We herein describe a counterpulsation technique using an intra-graft balloon pump that can be safely used in the setting of thoracoabdominal aortic aneurysm remaining immediately after aortic root and arch replacement coupled with coronary artery bypass grafting.

Aortic Perforation and Ascending Dissection Due to Intra-Aortic Balloon Counterpulsation Implantation in Cardiac Surgery.

The intra-aortic balloon pump continues to be a useful ventricular assist device in cardiac surgery. Complications are estimated to be 7% to 40%, significantly high to catastrophic. We describe an aortic injury associated with the use of the device and an interdisciplinary management for the diagnostic and therapeutic approach. (Level of Difficulty: Intermediate.).

Mechanical Circulatory Support in Acute Decompensated Heart Failure and Shock.

An array of interventional therapeutics is available in the modern era, with uses depending on acute or chronic situations. This article focuses on support in acute decompensated heart failure and cardiogenic shock, including intra-aortic balloon pumps, continuous aortic flow augmentation, and extra-corporeal membrane oxygenation.
