Maxillary osteotomies as a component of orthognathic surgery disrupt the normal anatomy and function of the sinus. The osteotomy with advancement of the inferior component of the sinus leaves a bony and mucosal opening in the sinus. Immediately after surgery, nasal drainage is impeded because of intranasal swelling. Acute and chronic maxillary sinusitis would be expected; however, its incidence as an expected complication is not well documented. A systematic review and meta-analysis was completed using PubMed to determine the incidence of sinusitis after maxillary orthognathic surgery. Studies

OBJECTIVES: To describe the surgical orthodontic technique of correcting transverse maxillary deficiency associated with nasal obstruction and malocclusion.
SURGICAL PROCEDURE: The surgical-orthodontic correction of transverse maxillary deficiency consists in an incomplete Le Fort I procedure, without down-fracture, associated with a sagittal palatal osteotomy. This procedure is called SARE (Surgically Assisted Rapid Expansion). A pre-fabricated orthodontic appliance is cemented to premolars and molars prior to the surgical procedure. The patient activates the appliance allowing a 1 mm wideni

In this study, we investigated how method of mandibular fixation influenced longterm postoperative stability of the maxilla in Class III cases. In particular, we investigated change in the maxillary occlusal plane after Occlusal Plane Alteration. Therefore, we focused on change in the palatal plane to evaluate stability of the maxillary occlusal plane, as the position of the palatal plane affects the maxillary occlusal plane. This study included 16 patients diagnosed with mandibular protrusion. Alteration of the occlusal plane was achieved by clockwise rotation of the maxilla by Le Fort I oste

An approach for total maxillectomy with endoscopic transection of the pterygoid process via the contralateral maxillary sinus is described. In total maxillectomy, the resection of the pterygoid process of the sphenoid is a key step for successful resection. However, a conventional craniofacial approach requires extensive incision in the face, elevation of the lateral cheek flap. Even after elevation of the lateral cheek flap, visualization of this region is not good. An endoscopic approach through the contralateral maxillary sinus improved visualization of the pterygoid process, and osteotomy

A modified technique of Le Fort II osteotomy is described for the correction of naso-maxillary hypoplasia. The surgical access was via a combination of bicoronal flap and transoral sulcular incision. Any periorbital skin incision is avoided. This is made possible by having the medial orbital cut made through the lacrimal groove. The anterior limb of the medial canthal ligament is detached, but leaving the posterior and superior limbs intact. Following mobilization, the osteotomy segment is stabilized by miniplates without intermaxillary fixation. This method eliminates the need for facial inci

INTRODUCTION: Usually, patients suffering from Crouzon syndrome have synostosis of coronal sutures, exophthalmia, hypertelorism, and hypoplasia of the middle third of face. Sometimes maxillary retrusion is absent, so these patients have class I or II relationship. In these cases, frontofacial monobloc advancement, which is the gold standard, increases the maxillo-mandibular dysmorphia. Therefore we propose orbitofrontal monobloc advancement minus dental arch, without splits of the pterygoid plates.
CASE REPORT: A 12-year-old girl with Crouzon syndrome had intracranial hypertension, exophthalm

BACKGROUND: Stress relieving corticoto mies during the treatment of maxillary expansion are needed in adult patients.
METHODS: Three-dimensional (3D) finite element model was prepared, and finite element analysis was processed to evaluate the stress distributions within the skull and maxillary teeth during surgically assisted rapid maxillary expansion (SARME) treatment.
RESULTS: Expansion forces generated more stress on the corticotomy-applied part of the maxilla. The stress levels decreased dramatically above the corticotomy line.
CONCLUSION: Asymmetric transveral maxillary expansion might

BACKGROUND: We studied the influence on healing of a resorbable membrane covering the osteotomy site after maxillary sinus grafting, evaluated in different regions of the augmented area.
METHODS: Maxillary sinus augmentation was performed in 24 New Zealand rabbits. Osteotomy, 4 × 6 mm, were performed bilaterally. A collagenated cortico-cancellous porcine bone was used to fill the elevated region. A collagen membrane was randomly placed over the osteotomy site on one side (MG), and the other side was left uncovered (NMG). The animals were euthanized after 2, 4, and 8 weeks; and histomorphometr

BACKGROUND: Le Fort I osteotomy can lead to significant changes in nasal soft tissue, potentially affecting facial aesthetics. While previous studies have explored these effects, few have used three-dimensional photogrammetry or accounted for maxillary segmentation and movement.
OBJECTIVE: This retrospective matched case-control study compares nasal soft tissue changes one year after segmented versus non-segmented Le Fort I osteotomies using 3D stereophotogrammetry.
METHODS: Forty-nine patients who underwent segmented or non-segmented Le Fort I osteotomy between 2016 and 2021 were included.

In Crouzon syndromes, there is an hypoplasia of the midface. An intermediate midfacial osteotomy between Lefort 2 and Lefort 3, called "butterfly osteotomy", associated wit a rigid external distraction on a Pooley helmet allows to pull the entire middle maxillary, orbital and zygomatic level in one step, and to project the tip of the nose, by a single endobuccal approach. The osteotomy concerns the entire middle level of the face while respecting the upper part of the nose. The external distractor with cranial support makes it possible to reposition the face, corrects the open bite and project

The indirect sinus lift for maxillary sinus augmentation is a common, less invasive solution for placing dental implants. However, the patency of the membrane not to get perforated during the procedure is questionable. A contrast medium, for example, Iohexol, held in the osteotomy site, can be a great guide to ensure membrane integrity. In this technical note, we present an innovative use of the syringe plunger as a stop to hold the contrast media in the osteotomy site in indirect sinus lift procedures.

Le Fort I osteotomy is a common orthognathic surgery procedure, but can result in unpredictable nasolabial changes, highlighting the need for a greater understanding of its effects on the soft tissue and evaluation of alternative techniques. The aim of this study was to compare the subspinal Le Fort I osteotomy (SSLFI) to conventional Le Fort I osteotomy (cLFI) regarding their impact on the nasolabial angle (NLA), with the goal of clarifying their clinical implications and assessing the utility of the subspinal approach. This retrospective analysis included 38 patients who underwent maxillary

BACKGROUND: Le Fort I osteotomy (LFIO) may lead to nasolabial changes. Therefore, several techniques have been proposed to minimize these effects, including the subspinal osteotomy, which can preserves perinasal structures.
PURPOSE: The purpose of this study was to compare the associations between conventional and subspinal LFIO and changes in the nasolabial soft tissues.
STUDY DESIGN, SETTING, SAMPLE: A retrospective cohort study was conducted at the residency program of the João XXIII Hospital in Belo Horizonte (2019 to 2023). The sample included subjects who underwent conventional or subs

This study presents a combination of 2 different surgery procedures performed on the mandible as part of the treatment aiming to improve the facial profile and occlusal function of patients with severe skeletal class Ⅲ malocclusion and bilateral edentulous gaps. The teeth next to the edentulous gaps were found to be ankylosed. Mandibular setback by bilateral sagittal split ramus osteotomies and mandibular body osteotomies, combined with Le Fort Ⅰ level maxillary advancement were performed, since the chief complaint of the patient was a concave profile. As a result, the skeletal class Ⅲ maloccl
