BACKGROUND: Gentamicin is a broad-spectrum aminoglycoside antibiotic produced by Micromonospora purpurea bacteria, effective against Gram-negative bacterial infections. Major fractions of the gentamicin complex (C1, C1a, C2, C2a) possess weak antifungal activity and one of the minor components (A, A1-A4, B, B1, X), gentamicin B1 was found to be a strong antifungal agent.
METHODS: This work uses in vitro and in vivo dilution methods to compare the antifusarial, antiaspergillic and anticryptococcal effects of gentamicin derivatives and structurally-related congeners.
RESULTS: The in vitro anti

Mycoses are serious diseases with potentially fatal outcome. The introduction of immunosuppressive treatments and life support techniques has led to a growing prevalence of different degrees of immunosuppression. Compromised immune response is the primary risk factor for the development of opportunistic mycoses. Early diagnosis and treatment are crucial for improving prognosis. However, isolation in cultures or identification using antigen detection techniques cannot distinguish between colonization and invasive infection, and the clinical status of the patient often prevents biopsy sampling. …

We report the isolation of a novel agent of subcutaneous mycosis from a 45-year-old Indian male immigrant in the United States. Phylogenetic analysis of partial small ribosomal subunit and large ribosomal subunit, internal transcribed spacer, partial translation elongation factor (TEF1), and RNA polymerase second largest subunit (rPB2) loci revealed that the strain was identical to another isolate previously reported as "Madurella mycetomatis." Both strains clustered in the order Pleosporales, nested in the family Arthopyreniaceae/Roussoellaceae. The fungus differed from known species and henc

Invasive fungal infections are a major cause of morbidity and mortality among organ transplant recipients, despite many progresses concerning diagnosis, preventions and treatment. Risk factors for invasive fungal infections in transplanted recipients include type and severity of immunosuppression, especially in life-saving organs as lung or liver, older age at transplantation, and technical complexity of surgery, living in endemic areas or exposure to a contaminated environment. Superficial fungal infections are caused by Candida, Dermatophytes, and Malassezia. In invasive mycoses, skin lesion…

OBJECTIVES: Flexible bronchoscopy with exfoliative cytology is an important tool for the diagnosis of pulmonary fungal infections. The question of colonization versus true fungal infection is of critical importance.
STUDY DESIGN: A 5-year retrospective analysis of all cases of pulmonary fungal infection diagnosed using exfoliative cytology was performed. Clinical, radiological, bronchoscopy and histopathology findings were recorded.
RESULTS: A total of 69 cases of mycoses were retrieved. The most common fungal organism identified was Aspergillus followed by Candida and Pneumocystis. Most cas…

This article presents two cases of opportunistic mycoses (OMs) of the central nervous system (CNS) caused by Cryptococcus neoformans and Aspergillus nidulans, respectively. The patients were hospitalised in local hospitals between 2009 and 2011 because of unspecific symptoms (fever, headache, and/or weight lost). Duration of symptoms varied from 4 days to over 2 weeks. The patients were treated with antibiotics and symptomatically. OM was not suspected in any of them. The patients became critically ill with symptoms of CNS involvement and were transferred to the Intensive Care Unit (ICU) of th…

Fungal foot infections belong to the most frequent infectious diseases in industrialized countries. In Germany, an estimated one third of th … d on underlying symptoms and localization of the infection, there are three principal forms of foot mycoses. The most common type is the intertriginous type characterized by peeling, maceration and fissuring mostly affecting the lateral toe clefts. The rarer vesiculobullous form is characterized by vesicles and blisters often located on the soles. The squ …

We performed a retrospective cohort study, using the 2002 Nationwide Inpatient Sample, a national database of hospital inpatient stays, to describe the incidence and epidemiology of endemic mycoses requiring hospitalization. An estimated 332 pediatric and 6003 adult patients with endemic mycoses required hospitalization (4.6 and 28.7 cases per 1 million children and adults, respectively). Crude mortality rates were 5% and 7% among children and adults, respectively.

Clinical applications and in vitro studies show that all antimycotics are most effective against infection when applied as early as possible. An early diagnosis is, therefore, essential. At this time for aspergillosis and particularly candidosis, a statement is only possible by combining several diagnostic methods. Inflammatory parameters like procalcitonin, C-reactive protein and proinflammatory cytokines are most important evidence of infection. Antigen tests are more significant by higher sensitivity. Attention should be focused on the detection of mannan by ELISA test, beta-glucan and D-ar

For most opportunistic mycoses no optimum duration of antifungal therapy has been defined. Although a long course of therapy is prudent, especially for mycelial fungal infections, excessively and unnecessarily extended courses of treatment incur a risk of toxicity and the expense of the therapeutic regimen. On the basis of existing reports on the pathogenesis and the effect of duration of therapy and other variables on outcome and response rate in fungal infections, this review proposes guidelines that may facilitate a rational approach to decision-making about the duration of antifungal thera…

Amphotericin B (AmB) remains the 'gold standard' for systemic antifungal therapy and newer drug delivery systems have been effectively devel … etically superior to the conventional dosage form of itraconazole, and is particularly effective in mycoses following cytotoxic chemotherapy and bone marrow transplants. Newer potential antifungals such as echinocandins, benanomicins/pradimicins, aureobasidins and nikkomycins are under clinical trials. The role of -immunomodulation in systemic anti-fungal …

Dermatomycoses are an important sanitary problem. Our country counts with partial epidemiological data only referred to a few geographical areas. The aim of our work was to perform a multicenter study with the participation of 15 medical centers located at the Capital City and in 11 Provinces of Argentina with different socioeconomical and cultural characteristics. Between October and December 1998, 851 ambulatory patients of both sexes (58% female, 42% male) with lesions compatible with mycoses in skin, nails or scalp were studied. Only 51.6% of the patients had mycoses confirmed by direct mi…

PURPOSE: To describe the clinical presentation and outcomes of treatment with itraconazole in patients with sporotrichosis.
METHODS: A culture for Sporothrix schenckii or compatible histopathology was required for inclusion in the study. Patients with both cutaneous and systemic sporotrichosis were treated. Patients received from 100 to 600 mg of itraconazole daily for 3 to 18 months. Patients were classified as responders or nonresponders. Responders were further classified as remaining on treatment, relapsed, or free of disease. Nonresponders included patients who failed to respond or progr

Zygomycosis is characterized by a very high lethality, often favoured by rapid fungal growth in blood vessels causing the formation of thrombi and infarcts in several organs, a disease poor in symptoms. A disseminated mycosis normally is a complication of a granulocytopenia which is in our days more frequently observed as localized manifestation, whereas the typical rhinocerebral manifestation complicating diabetic ketoacidosis has become more rare. The diagnosis of zygomycosis can apparently be obtained easier by histology than by culture. The prognosis of zygomycosis can be improved by: 1. a
