The Treatment of IgA Nephropathy.

BACKGROUND: IgA nephropathy (IgAN) is a very common glomerulonephritis worldwide. Nevertheless, treatment options for primary IgAN are still largely based on opinion or weak evidence. There is a lack of large randomized controlled trials (RCT) that provide a definitive immunosuppressive protocol for IgAN. The recent KDIGO Clinical Practice Guidelines for Glomerulonephritis have assigned low levels of evidence for almost all recommendations and suggestions related to this nephropathy. SUMMARY: In this article, we review different treatment options and emphasize that the key to therapeutic deci

[Ultrastructural heterogeneity of "minimal changes" in the kidney glomeruli, detected by light optics].

The ultrastructural study of "minimal changes" found at the light microscopic examination of renal biopsies from patients with nephrotic syndrome revealed heterogeneity of morphological changes. A number of diseases included into this group manifest clinically as nephrotic syndrome: hypoplastic dysplasia, lipoid nephrosis, focal-segmentary glomerular hyalinosis, membranous, mesangiomembranous, mesangioproliferative glomerulonephritis. Each of these diseases has its own pathogenetic mechanism resulting in a specific morphological manifestation and consequently in a specific treatment.

Clinical and morphological correlations in mesangial glomerulonephritis in children.

The present series comprises 36 cases of mesangial glomerulonephritis in children aged from six months to 14 years. On the basis of morphological criteria, types with considerable and slight mesangial proliferation were distinguished. Slight mesangial proliferation was more typical of younger children. Clinically they showed more often the nephrotic syndrome (sometimes difficult to differentiate from lipoid nephrosis). Older children developed mesangial proliferation rather of greater intensity: renal insufficiency and erythruria predominated in this group.

Fibrinogen and FDP-E antigen deposits in nephritic kidneys.

Glomerular localization of fibrinogen reactive antigens (FRA) and fibrinogen degradation products reactive antigens (FDP-E) was investigated in 184 renal biopsy specimens of diseased kidneys by an immunofluorescent method. There was clear dissociation of FRA and neoantigen deposits in the glomeruli in a moderately active stage of acute glomerulonephritis, chronic glomerulonephritis and in all 5 cases of lipoid nephrosis. In lupus nephritis and membranous nephropathy there were almost coincident deposits of these antigens.

Sclerosing glomerulonephritis and malignant hypertension in a patient with congenital renal dysplasia: A case report.

A patient with a single functioning kidney presented with the nephrotic syndrome. On the basis of highly selective proteinuria, a diagnosis was made of lipoid nephrosis. Steroid therapy over a 2-year period did not control the disease. The patient eventually developed end-stage renal failure and malignant hypertension. Nephrectomy was performed to control the hypertension. Histological examination showed congenital dysplasia in one kidney and sclerosing glomerulonephritis, malignant nephrosclerosis, as well as dysplastic changes in the other.

[Clinical signs of food allergy].

To investigate the clinical signs of food allergy is to open new horizons for the understanding of worrying acute symptoms for the patients and too often only symptomatic treatment. It is also possible to take control in a more logical way of illnesses that are classified as chronic and so incurable. The clinical examination requires great attention and much listening. Urticaria, eczema, rhinitis, asthma, colitis are the most frequent symptoms. More rarely, there may be certain migranes or lipoid nephrosis in children.

Association of class II antigens of HLA with primary glomerulopathies.

HLA antigens of Japanese patients with primary glomerulopathies were determined, and the frequency of each HLA antigen was compared with that of Japanese normal controls. IgA nephropathy with HLA-DR4, idiopathic membraneous glomerulopathy with DR2, lipoid nephrosis with DRw53 and poststreptococcal glomerulonephritis with DR1 were definitely or probably associated. It was suggested that primary glomerulopathies might all be associated with HLA class II and not class I antigens. Japanese patients with HLA-Bw48 and -DRw8 were less susceptible to primary glomerulopathies.

AN ELECTROPHORETIC STUDY OF NEPHROTIC SERA AND URINE.

The electrophoretic patterns of the sera and urine of two cases of lipoid nephrosis have been obtained and have been compared with a typical pattern of normal serum. The patterns of the pathological sera deviated widely from the normal, indicating relatively low albumin and high globulin content. The comparison of the patterns of nephrotic sera cleared by centrifugation and by ether extraction shows that a large portion of the beta globulin consisted of a labile lipo-protein. The pattern of the nephrotic urine proteins resembled that of normal serum, with, however, significant differences.

[Clinico-anatomical forms of primary glomerulonephritis].

The various clinicopathological forms of primary glomerulonephritis (GN) in humans include lipoid nephrosis with its histological variants (minimal change disease, focal segmental glomerular sclerosis, diffuse mesangial proliferation), membranous GN, IgA nephropathy, membranoproliferative GN (type I and type II) and crescentic GN. Each of these entities is characterised by pathological, clinical, and immunological features. Although the precise etiology of primary GN remains unknown, important progress from experimental models has been recently obtained in the understanding of the mechanisms o

Secondary minimal change disease.

The great majority of patients identified as having a 'minimal change lesion' accompanying the nephrotic syndrome have a primary or 'idiopathic' disorder. Nevertheless, it is quite apparent that a similar or identical lesion can appear consequent to a growing number of underlying diseases; it is then known as 'secondary minimal change disease'. The predisposing conditions include neoplastic diseases, toxic or allergic reactions to drugs, infections, auto-immune disorders and other miscellaneous entities. These disorders are reviewed and catalogued in this contribution.
