Review of local anaesthetic agents.

The currently available local anaesthetic agents are capable of providing high quality nerve blockade in a wide variety of clinical circumstances. Our understanding of the mechanisms and consequences of toxicity is increasing rapidly. Knowledge of the chemistry of local anaesthetics has enabled clinicians to exploit the increased safety of single isomer agents. However, the extent, if any, of this improvement in toxicity has yet to be proven. Established toxicity may be very difficult to treat and no specific reversing therapy is yet available. Until this occurs it is essential that practition

Unilateral third nerve palsy caused by Guillain-Barré Syndrome.

BACKGROUND: Highly asymmetric clinical signs in a patient suggest the need for caution in making the diagnosis of Guillain-Barré Syndrome (GBS). METHODS: Case report and literature review. We present a case of strictly unilateral left third cranial nerve palsy in a patient with GBS, review other highly asymmetrical cranial nerve palsies previously reported in this condition, and suggest an appropriate alternative differential diagnosis. CONCLUSION: Unilateral third cranial nerve palsy is an exceptionally rare manifestation of GBS, expanding the spectrum of clinical signs and neuraxis involve

[Sensory neuropathy and primary biliary cirrhosis].

Primary biliary cirrhosis (PBC) may associate an axonal neuropathy, a somatic and autonomic neuropathy and a very infrequently sensory neuropathy (with or without xanthomata). The aim of this paper is to describe the case of a 46 year old man diagnosed with PBC in stage I-II and a progressive sensory neuropathy (axonopathy) confined to his upper limbs with distal predominance. It had progressed slowly an began asymmetrically. A complete clinical study excluded other causes of neuropathy. We followed him clinically and electromyographically and he remains stable after two years evolution. The s

Reversible combined cognitive impairment and severe polyneuropathy resulting from primary hyperparathyroidism.

Central and peripheral nervous systems may be involved in primary hyperparathyroidism (PHP). The efficacy of parathyroidectomy in reversing neurological symptoms is still a matter of controversy. We describe the case of a 71-year-old white male with a 10-year history of PHP who developed progressive cognitive dysfunction and severe sensorimotor axonal polyneuropathy. Successful parathyroidectomy reversed with a different temporal course both the central and peripheral nervous system involvements.

[A case of neuroborreliosis with unilateral radiculoneuropathy and elevated serum antibodies for Borrelia garinii and Borrelia afzelii].

A 24-year-old male patient was admitted to our hospital because of dysesthesia and weakness of the right leg. Six weeks before admission he traveled to southern China. A week later he noticed multiple papules with itching on his right leg that were suspected to be insect bites. Four days later numbness of the right leg developed. Then he began to limp because of the leg weakness. Two weeks before admission severe pain in the right leg developed. On admission neurological examination revealed hypesthesia, muscle weakness and atrophy, areflexia, radicular pain and Lasègue sign of the right leg,

Significance of A-waves recorded in routine motor nerve conduction studies.

The occurrence of A-waves during routine F-wave studies was investigated in 556 consecutive patients referred to the Department of Clinical Neurophysiology at the University Hospital in Uppsala for various neuromuscular disorders. Altogether, 2367 nerves in the upper and lower extremities were studied. An A-wave, with a nearly constant latency and a uniform shape on consecutive stimulations, could be recorded in 184 nerves (7.8%) out of 124 patients (22.3%). More than 50% of patients with A-waves had various types of polyneuropathies. Of all patients with polyneuropathy, 65% had at least one n

Comparison of sensory mid-palm studies to other techniques in carpal tunnel syndrome.

This prospective study was done to assess a technique of comparing Median-Ulnar sensory latency differences across the wrist (Sensory Palm [M-U]) to diagnose carpal tunnel syndrome (CTS) and to compare the relative sensitivities of other previously reported techniques: Median-Radial sensory difference (Sensory Thumb [M-R]), Median-Ulnar mixed palm difference (Mix Palm [M-U]) and Median-Ulnar sensory digit difference (Sensory Digits [M-U]). 90 Hands met the electrodiagnostic (EDX) criteria for mild CTS. The Sensory Thumb (M-R) was abnormal in 90% of the hands followed in order by Mix Palm (M-U)

Electrophysiological study of diaphragmatic myoclonus.

This is the first reported detailed electrophysiological study of diaphragmatic myoclonus. An 86 year old woman had rapid, intermittent epigastric pulsations. Neurological examination and imaging studies of the brain and spinal cord were normal. Needle EMG showed rhythmic contractions of the diaphragm and external intercostal muscles at 4 to 5 Hz. These contractions were often associated with suppression of normal breathing and were capable of maintaining adequate ventilation. Both diaphragms were involved but showed considerable variability in their relative latencies. Automated interference

Surface EMG and muscle fibre conduction during attacks of hypokalaemic periodic paralysis.

Surface EMG, muscle fibre conduction velocity (MFCV), muscle force, and biochemical variables were investigated in a 13-year-old boy with familial hypokalaemic periodic paralysis during and after three attacks of paralysis. After normalisation of the serum potassium values, strength rapidly returned to interictal values, but the integrated EMG and to a lesser extent the MFCV recovered more slowly. These findings suggest that a complete electrophysiological recovery is not necessary for a restoration of muscle force and that the pathogenetic defect is localised in the muscle membrane.

Amine modulation of electrical coupling in the pyloric network of the lobster stomatogastric ganglion.

1. The neurons of the pyloric network of the lobster (Panulirus interruptus) stomatogastric ganglion organize their rhythmic motor output using both chemical and electrical synapses. The 6 electrical synapses within this network help set the firing phases of the pyloric neurons during each rhythmic cycle. We examined the modulatory effects of the amines dopamine (DA), serotonin (5HT) and octopamine (Oct) on coupling at all the electrical synapses of the pyloric network. 2. Electrical coupling within the pacemaker group [anterior burster (AB) to pyloric dilator (PD), and PD-PD] was non-rectifyi

[Multifocal axonal motor neuropathy associated with anti-ganglioside antibodies].

We report a patient with asymmetrical patchy weakness of the limbs, and with autoantibodies against gangliosides GM1, GD1b, asialo GM1. Although electrophysiological studies did not reveal conduction block, treatment with prednisolone resulted in clinical improvement. A 52-year-old man was admitted to Kyoto University Hospital, because of gait disturbance. Neurological examination revealed a patchy distribution of weakness in the limbs. Deep tendon reflex was normal at the right knee, and was depressed at the right biceps. Other deep tendon reflexes were absent. There was a slight decrease in

Congenital carpal tunnel syndrome: case report of autosomal dominant inheritance and review of the literature.

Carpal tunnel syndrome (CTS) is unusual in childhood, and familial occurrence has been reported infrequently. A case of CTS in a seven-year-old boy, associated with abnormal thickening of the transverse carpal ligament and aplasia of the median nerve distal to this ligament, is described. Clinical, electrodiagnostic, and surgical findings are presented: all were consistent with absence of the median nerve distal to the transverse carpal ligament. Family history of CTS was positive in a pattern consistent with autosomal dominant transmission in three generations. Thickening of the transverse ca

Charcot-Marie-Tooth disease: a case presenting with hyperreflexia.

We report a 10-year-old girl with progressive weakness of lower extremities, feet deformity, and sensory impairment on both feet for 3 years. Absent ankle tendon reflex, exaggerated knee jerk and prolonged nerve conduction velocity were noted by physical and electrophysiological examination. Nerve and muscle biopsy showed demyelination and neurogenic changes respectively and supported the diagnosis of Charcot-Marie-Tooth disease, type I. The point that the diagnosis of Charcot-Marie-Tooth disease remains possible even in the absence of family history and the typical clinical picture is stresse

Single fiber electromyography in chronic progressive external ophthalmoplegia.

We have reviewed the electromyographic (EMG) studies of 17 patients with chronic progressive external ophthalmoplegia (CPEO). In 13 of 17 patients, conventional concentric needle EMG demonstrated a "myopathic" pattern, usually predominating in the shoulder muscles. Single-fiber EMG showed increased jitter and/or blocking in at least one muscle in 13 of 16 patients. Jitter was increased in the frontalis muscle in 10 of 13 patients and in an arm muscle in 5 of 12. When both muscles were tested, jitter was greater in the frontalis muscle in 5 patients and in the arm muscle in 2. These observation

Tarsal tunnel syndrome: electrophysiological study.

Tarsal tunnel syndrome (TTS) is a rare compression neuropathy of the posterior tibial nerve. Typical symptoms are burning pain and paresthesia in the toes and along the sole of the foot. The presence of Tinel's sign and objective sensory loss in the territory of any of the terminal branches of the posterior tibial nerve are diagnostically helpful. The terminal latency and sensory nerve conduction velocity in medial and lateral plantar nerves were studied in 20 normal controls and 21 cases of TTS in 17 patients. Prolonged terminal latency was observed in 11 cases, with TTS, while sensory nerve
