[Histological investigation of nail growth in human embryos and regrowth of nail after its removal in adult monkeys--with reference to clinical observation in injuries to the nail and its surrounding tissues].

As the basis to develop adequate treatments which can preserve the nail plate or assure its regrowth in frequently encountered injuries to the nail-surrounding tissues, a histological investigation was made of nail growth in human embryos and regrowth of nail after its removal in adult monkeys. The author's investigation in these two series indicated the following. Firstly the nail plate composed of parakeratotic layer is produced solely of "nail matrix", thus, supporting Zaias' and opposing Lewis' theory as to the nail production. Secondly though the normal, completely formed nail matrix and

Use of the dorsal digital sensate free flap for reconstruction of volar soft tissue defect of digits.

Sensate cross-finger flaps and homodigital and heterodigital island flaps can usually be used for sensory reconstruction in the volar aspect of the digit. However, when the donor areas are damaged by concomitant injuries, these flaps are not available. The free dorsal digital flap, including both dorsal branches of the proper digital nerves, can be used as an alternative to resolve this problem. The flap was used in 16 patients with soft tissue defects on the volar aspect of the digits. For comparison, we collected a consecutive series of 31 patients with similar defects treated using a cross-

MMP-14 and MMP-2 are key metalloproteases in Dupuytren's disease fibroblast-mediated contraction.

Dupuytren's disease (DD) is a common fibrotic condition of the palmar fascia, leading to deposition of collagen-rich cords and progressive flexion of the fingers. The molecular mechanisms underlying the disease are poorly understood. We have previously shown altered expression of extracellular matrix-degrading proteases (matrix metalloproteases, MMPs, and 'a disintegrin and metalloprotease domain with thrombospondin motifs', ADAMTS, proteases) in palmar fascia from DD patients compared to control and shown that the expression of a sub-set of these genes correlates with post-operative outcome.

Should unstable extra-articular distal radial fractures be treated with fixed-angle volar-locked plates or percutaneous Kirschner wires? A prospective randomised controlled trial.

Fractures of the distal radius are commonly treated with cast immobilisation; however, those potentially unstable injuries with dorsal comminution may need operative intervention. This intervention is usually with manipulation and Kirschner wires but advances in locking-plate technology have enabled surgeons to achieve anatomical reconstruction of complex fracture patterns, even in poor-quality osteoporotic bone.To ascertain if fixed-angle volar-locked plates confer a significant benefit over manipulation and Kirschner-wire stabilisation, we prospectively randomised 56 adult patients with isol

Long-term results of forearm shortening and volar radiocarpal capsulotomy for wrist flexion deformity in children with amyoplasia.

PURPOSE: To perform a long-term review of results of forearm shortening osteotomies, tenotomy, and volar wrist capsulotomy for treatment of typical wrist flexion deformities in children with the amyoplasia form of arthrogryposis, a procedure originally reported and described as "most rewarding" by Lexington Shriners Hospital for Children in 1965. METHODS: A retrospective review of operating room and clinic records was done. An attempt to contact all patients produced 1 current follow-up. RESULTS: Nine extremities in 6 patients had undergone the procedure. Of the 9 extremities, 3 subsequently

Volar plate repair for posttraumatic hyperextension deformity of the proximal interphalangeal joint.

Traditionally, flexor digitorum superficialis tenodesis has been recommended for surgical correction of posttraumatic proximal interphalangeal (PIP) joint hyperextension deformity resulting from recurrent volar plate (VP) disruption. In contrast, VP repair has been used sparingly to restore joint stability, because of concerns regarding excessive scarring, insufficient substance, and the often long time between injury and repair. In the study reported here, we critically evaluated the long-term functional outcome of isolated VP repairs for chronic dorsal instability of the PIP joint performed

Management of stable proximal interphalangeal joint volar plate injuries with figure-of-8 orthoses: A parallel-group randomized controlled trial.

BACKGROUND: Volar plate injuries of the proximal interphalangeal (PIP) finger joint are common. Conservative treatment involves orthoses to limit hyperextension at the PIP joint yet allow movement of the joints to prevent joint stiffness and deformity. Custom-made dorsal blocking orthoses are recommended treatments. Previous research also supports the use of a figure-of-8 orthosis, although the comparative effectiveness of these orthoses is not currently known. PURPOSE: This study aimed to compare the figure-of-8 orthosis and dorsal blocking orthosis for changes in the range of movement, pain

Results of conservative treatment of volar plate sprains of the proximal interphalangeal joint with and without avulsion fracture.

The prognosis of isolated volar plate sprains of the proximal interphalangeal (PIP) joint is related to the extension deficit. Some consider an associated avulsion fracture as a sign of severity. The goal of this study was to find out whether the outcomes of conservative treatment in PIP volar plate injury was impacted by the presence of an avulsion fracture. Our series included 75 patients, 27 years old on average, of which 58 were men. All sprains were stable. The X-rays were normal in 52 cases (group 1); an avulsion fracture was found in 23 cases (group 2). The patients were treated by budd

Malunited extra-articular distal radius fractures: corrective osteotomies using volar locking plate.

BACKGROUND: Multiple techniques for corrective osteotomy have been developed in recent years with the same aims: to improve the radiographic parameters and improve motion, pain and grip strength. Volar fixed-angle plates have added a new concept to the treatment of distal radius fractures thanks to the low morbidity of the surgical approach and the strength of the final construct, allowing early mobilization and return to function. MATERIALS AND METHODS: Between 2005 and 2012, 20 patients with symptomatic dorsally malunited extra-articular fractures of the distal radius underwent corrective o

[Complex fragmentation of the distal radial articular surface. Reconstruction with subchondral Kirschner wires and bone grafts].

BACKGROUND: Complex fragmentation of the distal radial articular surface often results in three major problems: some fragments are too small for stabilisation by standard plates or screws; in addition, substantial loss of articular surfaces and metaphyseal/subchondral bony defects frequently occur. MATERIAL AND METHODS: To solve these problems the following strategy was developed: the fixation of small articular fragments was performed by small K-wires that were placed subchondrally and countersunk in the bone. In case of lost articular surfaces a silicone foil was inserted intra-articularly

Evaluation of flexor pollicis longus tendon attrition using color Doppler imaging after volar plate fixation for distal radius fracture.

PURPOSE: Early diagnosis of attritional wear of the flexor pollicis longus (FPL) tendon is essential in preventing subsequent tendon rupture. There are currently few objective methods of assessing FPL attrition. We hypothesized that color Doppler imaging could visibly detect FPL tendon attrition, and analyzed our results. METHODS: We evaluated ultrasound imaging of the contact between the FPL tendon and a volar locking plate using the real-time B-mode and Doppler waveforms of the FPL tendon using the continuous Doppler wave mode in 40 patients who underwent fixation of the distal volar lockin

Hybrid Fixation With a Low-Profile Locking Plate and Cannulated Compression Screw for Thumb Carpometacarpal Arthrodesis: A Retrospective Case Series.

INTRODUCTION: Thumb carpometacarpal (CMC) fusion offers durable pain relief and enhanced pinch stability for patients with advanced CMC arthritis. However, concerns regarding nonunion have hindered its widespread adoption. Hybrid fixation using an anatomically contoured low-profile locking plate and a cannulated compression screw (CCS) is intended to combine direct interfragmentary compression with angular and rotational stability; however, clinical evidence regarding this construct remains limited. This study aimed to evaluate the radiographic and functional outcomes of this hybrid construct.

Large Cervical Vagus Nerve Tumor in a Patient with Neurofibromatosis Type 1 Treated with Gross Total Resection: Case Report and Review of the Literature.

INTRODUCTION: Rolando fractures being intra-articular fractures of the most mobile joint of the thumb, assume significance because any residual incongruity of the articular surface may result in loss of motion as well as secondary osteoarthritis and hence serious disability. This fracture continues to pose difficulties to the treating surgeons and although several treatment options have been described for these fractures, there are no definite guidelines. METHODS: The present study describes the results of open reduction and internal fixation of nine Rolando fractures with large and single fr

[Early corrective osteotomy after secondary displaced distal radius fractures in children].

Secondary fracture displacement before osseous consolidation of distal radius fractures in children occasionally leads to restricted forearm rotation. So far, there is no consistent treatment recommendation to correct this complication. We report on 5 children with an age of 8-13 years (mean age 12.3 years, 4 boys, 1 girl) with secondary displaced distal radius fractures and high functional deficits in forearm rotation (mean ROM for pro-/supination 70-0-30°) after osseous consolidation. We performed corrective osteotomies of the distal radius using a palmar approach after a mean of 38 days. St

Distal radius fracture fixation with a volar locking plate and endoscopic carpal tunnel release using a single 15mm approach: Feasibility study.

Distal radius fractures (DRF) may trigger, reveal or decompensate acute carpal tunnel syndrome (CTS) in 0.5-21% of cases. Internal fixation and median nerve release must then be carried out urgently. Less invasive approaches have been described for both the median nerve release using an endoscopic device and for the DRF fixation using a volar locking plate. We assessed the feasibility of DRF fixation and median nerve release through a single, minimally-invasive 15mm approach on a series of 10 cases. We reviewed retrospectively 10 consecutive cases of DRF associated with symptomatic CTS in 8 wo

Fibrin/fibrinogen and fibrinolytic activity of the palmar fascia in Dupuytren's contracture.

Considering the proved interaction of fibrin with fibroblasts and the seemingly decisive role of structural and functional changes ("modulation") of these cells in the evolution of Dupuytren's contracture, research has been carried out in order to investigate the fibrinolytic capacity and the possible presence of fibrin/fibrinogen in the palmar fascia of subjects operated upon for Dupuytren's Disease. Fibrin/fibrinogen were detected by a direct immunofluorescence technique and fibrinolytic activity was assessed by a fibrin plate method. A remarkable decrease of fibrinolytic activity and the pr

Carpal tunnel decompression through the flexor carpi radialis approach: a cadaveric comparative study.

INTRODUCTION: Carpal tunnel syndrome may occur in association with distal radial fractures, and decompression is sometimes considered at the time of fixation. The flexor carpi radialis approach used for anterior plating may allow decompression without extending the incision into the palm. This study compared intracarpal canal pressure reduction achieved through this approach and the standard technique. METHODS: Twelve fresh-frozen upper extremities were studied. Within each pair, one limb underwent carpal tunnel release through the flexor carpi radialis approach and the contralateral limb thr

Significance of the Sparing Pronator Quadratus for Volar Plating of Distal Radius Fractures: A Prospective Study.

Background Pronator quadratus (PQ) acts as the pronator of the wrist and stabiliser of the distal radioulnar joint; it also provides a protective cover over the edge of the plate, preventing friction and subsequent rupture of flexor tendons. The repair of PQ is often difficult, and its durability is questionable; hence, preserving the PQ while volar plating distal radius fractures is advocated. Methods In this prospective randomised trial, 60 patients with a fracture of the distal end of the radius of AO-type (Arbeitsgemeinschaft für Osteosynthesefragen) 23 A2, A3, B1, B3, C1, and C2 were trea
