The role of surgical resection per se and the type of surgery in the management of multimodality treated malignant pleural mesothelioma remains controversial. Patient selection for either extrapleural pneumonectomy or radical pleurectomy depends not only on the cardiopulmonary status of the patient, tumor stage and intraoperative findings but is also strongly influenced by surgeons' preference, experience and philosophy. The aim of this review is to compare extrapleural pneumonectomy and radical pleurectomy with regard to surgical technique, morbidity, mortality and survival.

Hypopharyngeal perforation is a rare complication of endotracheal intubation. It most commonly occurs at the hands of the less experienced physician in emergency situations. The site most commonly perforated is the pharynx, posterior to the cricopharyngeal muscle; the second most common site is the piriform sinus. I report here a case of piriform sinus perforation secondary to intubation. Literature support is provided for the philosophy of early surgical intervention along with intravenous antibiotics as the treatment of choice.

The aim of this article is to explore violence among an understudied group of women, who attended Mutual-Aid Residential Centers for Addiction Treatment and experienced stigma both as women and addicts. George Kelly's "The psychology of personal constructs" put forward a new psychology that viewed people as actively constructing and anticipating their worlds. This paper considers personal construct theory and its philosophy; personal construct assessment techniques; the personal construct view of psychological disorder and its treatment; and the wide range of other applications of personal construct theory. It is concluded that personal construct psychology remains a radical approach over half a century after Kelly published his magnum opus.

A description of the International Criminal Police Organization and its role in disaster victim identification is provided along with a summary of the standards developed and circulated to responders in INTERPOL member countries (188 throughout the world) to insure evidence-based DVI practices. Following the INTERPOL-mediated DVI response in 2005 to the SE Asia tsunami, many lessons learned have been recorded. Based on these current standards, INTERPOL's approach to DVI reflects a modern approach and philosophy.

This article compares the outcomes following spinal fusion and disk replacement for degenerative conditions of the lumbar and cervical spine. The short-term outcomes of lumbar and cervical total disk replacement are equivalent to that following spinal fusion. Long-term follow-up studies of total disk replacement are necessary to confirm its potential benefit in reducing or preventing adjacent level degeneration. Also discussed is the philosophy of the surgical management of degenerative conditions of the lumbar and cervical spine.

Tendon transfers in cerebral palsy are commonly described as a method to improve function in the upper limb. The field can be considered by some to be more of an art than science due to the unique challenges involved, which include poor volitional control and spasticity. Surgical outcomes can be uncertain and, in certain circumstances, worsened. This presentation looks at the rationale behind tendon transfers for patients with cerebral palsy and the application of this rationale (science vs. art) to a philosophy of treatment.

A strict and rational approach to Fallot's tetralogy with pulmonary atresia and major aortopulmonary collateral arteries allows to achieve optimal results. Rehabilitative and unifocalization strategies do not constitute separate philosophies; instead the surgical strategy should be tailored to each individual patient. Based on our previous experience, the ability to achieve definitive intracardiac repair is the real determinant of both improved survival and adequate systolic right ventricular performance on mid-term follow-up.
