OBJECTIVE: To investigate the effectiveness of reversed arthroscopic subacromial decompression in the treatment of rotator cuff tears.
METHODS: Between November 2012 and January 2015, 53 patients with rotator cuff tears were treated with reversed arthroscopic subacromial decompression and rotator cuff repair. Of 53 patients, 38 were male and 15 were female, with the age of 47-61 years (mean, 53.4 years). The injury was caused by falling in 36 cases and other in 17 cases without an obvious history of trauma. All patients had shoulder pain and limited shoulder movement before operation. Visual

PURPOSE: To compare tendon-bone interface motion and cyclic loading in a single-row, triple-loaded anchor repair with a suture-tape, rip-stop, double-row rotator cuff repair.
METHODS: Using 18 human shoulders from 9 matched cadaveric pairs, we created 2 groups of rotator cuff repairs. Group 1 was a double-row, rip-stop, suture-tape construct. Group 2 was a single-row, triple-loaded construct. Before mechanical testing, the supraspinatus footprint was measured with calipers. A superiorly positioned digital camera optically measured the tendon footprint motion during 60° of humeral internal and

Single-row versus double-row repair of rotator cuff tears is currently a controversial topic. In this Level V article, we articulate why we believe that second-generation double-row repair techniques, which use bridging sutures to link the 2 rows of suture anchors together in a self-reinforcing manner, are producing superior clinical and biomechanical results.

BACKGROUND: It is generally believed that the supraspinatus is the most commonly involved tendon in rotator cuff tears. Clinically, however, atrophy of the infraspinatus muscle is frequently observed in patients with even small to medium-size rotator cuff tears. This fact cannot be fully explained by our current understanding of the anatomical insertions of the supraspinatus and infraspinatus. The purpose of this study was to reinvestigate the humeral insertions of these tendons.
METHODS: The study included 113 shoulders from sixty-four cadavers. The humeral insertion areas of the supraspinat

The purpose of this technical note is to introduce the ramp test and explain this arthroscopic technique. The ramp test is used to test the integrity of the soft tissue restraint to intra-articular subluxation of the long head of the biceps tendon. Injury to the soft tissue restraint, the hidden lesion, has been proposed as occurring in conjunction with a full-thickness rotator cuff tear. Both cadaveric dissections and arthroscopic patient assessments were conducted to develop the ramp test. In this study, 17 patients also presented with refractory anterior shoulder pain and underwent arthrosc

Rotator interval tears are an uncommon, but clinically important subtype of rotator cuff tear. As MRI assumes an ever increasing role in the imaging evaluation of the painful shoulder, radiologists must be aware of this entity and appreciate the difficulty in diagnosing these tears with MRI. Differentiation of a true rotator interval tear from normal synovium and capsule in this space is most likely not possible with MRI, and symptoms may be referred and misleading. It is therefore important to correlate the MR findings with the clinical history and physical examination.

Rotator cuff injuries, especially in athletes, can be very disabling. A case of an isolated rupture of the subscapularis tendon in an arm wrestler is reported. Preoperative arthrogram and CT scan with intraoperative pictures are used to illustrate the pathology. Recommendation for treatment is surgical repair.

Shoulder arthroscopy and rotator cuff repair techniques are frequently used by most practicing orthopaedic surgeons. A thorough patient history and physical examination can often confirm the presence of a rotator cuff tear, and imaging can be used to evaluate the extent of the injury. The indication for rotator cuff repair is a painful shoulder refractory to nonsurgical management. Arthroscopic techniques, including capsular and coracohumeral ligament releases to decrease tension on the repair, facilitate successful rotator cuff repair. Biomechanically, a double-row transosseous-equivalent rot

Multiple studies have established the trend toward arthroscopic rotator cuff repair in the US. While this study further establishes this trend, the literature supporting this change remains inconclusive. Given the increased cost of arthroscopic repair, further studies to support this technique would be desirable.

Augmentation of rotator cuff repairs has been suggested via a variety of techniques, including autograft, allograft, xenograft, and synthetic materials. Autograft is desirable due to the lack of antigenicity and low cost. Although the biceps smash technique represents an autograft technique that fulfills these requirements, high-level clinical studies are still necessary to substantiate its use.

Massive irreparable rotator cuff tears remain a persistent challenge. Reverse shoulder arthroplasty is increasingly performed, yet complications and long-term durability are concerns in younger patients. Outcomes of a subacromial balloon spacer have been conflicting. Biologic tuberoplasty with dermal allograft provides another joint preservation option. While longer follow-up and comparative data are required, early results of biologic tuberoplasty are encouraging.

SYNOPSIS: In the absence of diagnostic accuracy (clinical and imaging), specific shoulder structures cannot be implicated as the definitive source of shoulder symptoms. Patients report that the (1) worst pain is unexplained pain, and (2) to engage in a treatment, the treatment must make sense. The diagnostic label, nonspecific shoulder pain does not tick either box: subacromial pain syndrome is confusing, and as nondescript as suprahumeral pain syndrome. The rotator cuff tendons are not the definitive and sole source of shoulder symptoms, which calls into question the validity of terms such as

BackgroundSurgical management of rotator cuff syndrome (RCS) is increasingly common in the world. After surgery, the return to work (RTW) process is often difficult for many patients, indicating the presence of factors that facilitate or delay this process.ObjectiveTo identify factors influencing RTW after rotator cuff (RC) surgery.MethodsArticles regarding RTW after RC surgery were systematically searched via PubMed, Embase, and ScienceDirect databases. The review was performed according to the Joanna Briggs Institute (JBI) methodology for scoping reviews.ResultsOf the 323 initial studies, 21

BACKGROUND: Bone marrow stimulation (BMS) has been proposed to augment healing at the time of arthroscopic rotator cuff repair (ARCR) by creating several bone marrow vents in the footprint of the rotator cuff, allowing mesenchymal stem cells, platelets, and growth factors to cover the area as a "crimson duvet."
PURPOSE: To perform a meta-analysis of randomized controlled trials (RCTs) to compare the outcomes after BMS and a control for those undergoing ARCR.
STUDY DESIGN: Meta-analysis; Level of evidence, 1.

Massive rotator cuff tears are a challenge to treat, with options available from arthroscopic rotator cuff repair to reverse shoulder arthroplasty. Arthroscopic repair may still be an option even in the setting of advanced Goutallier changes and Hamada grades and regardless of healing. As the lifetime risk of revision for a 55-year-old patient having a reverse shoulder arthroplasty is approximately 1 in 3, arthroscopic surgical management of massive rotator cuff tears should not be discounted. An individualized approach should consider the patient's radiographs (severity of arthritis), functio

Although many surgeons discard the long head of the biceps (LHB) tendon during arthroscopic rotator cuff repairs, several authors have argued that the LHB tendon may serve as a biologic and structural augment for massive repairs. There are multiple benefits of incorporating the LHB tendon autograft in difficult-to-repair massive rotator cuff tears. First, by maintaining the glenoid origin, the tendon likely retains a significant native blood supply, as well as tenocytes, which may support the healing process. In addition, there are several biomechanical benefits. The LHB may act as a dynamic h

In theory, extrinsic anterolateral acromial impingement of the rotator cuff can contribute to cuff tearing. Thus, acromioplasty may be performed concomitantly with arthroscopic rotator cuff repair (ARCR). A review of recent randomized controlled trials confirms that patient long-term outcomes after ARCR are superior when acromioplasty is performed when compared to no concomitant acromioplasty, while complication and retear rates are similar. This is in contrast to previous reviews identifying no significant clinical differences in postoperative functional outcomes, patient-reported outcomes, o

BACKGROUND: Accurately diagnosing supraspinatus tears based on magnetic resonance imaging (MRI) is challenging and time-combusting due to the experience level variability of the musculoskeletal radiologists and orthopedic surgeons. We developed a deep learning-based model for automatically diagnosing supraspinatus tears (STs) using shoulder MRI and validated its feasibility in clinical practice.
MATERIALS AND METHODS: A total of 701 shoulder MRI data (2804 images) were retrospectively collected for model training and internal test. An additional 69 shoulder MRIs (276 images) were collected fr

BACKGROUND: Rotator cuff tear size, fatty infiltration, and scapular morphology are correlated with tendon healing and functional outcomes after arthroscopic repair; however, the association between anteroposterior acromial coverage and the clinical outcomes of anteroposterior massive rotator cuff tears (AP-MRCTs; involving all 3 tendons) remains unclear.
