[Instillagel in prevention of postoperative urinary tract infection--randomized trial].

UNLABELLED: The progress of operative technique, introduction of new drugs, modernizing of medical equipment are the cause of long and complicated operations. For that reason, the number of patients, who need the catheterization of the urinary bladder is increasing. The use of the vesical catheter over 5 days is the cause of the urinary tract infection (bacterial and mycotic), the bacteraemia and the extension of the time of recovery. The aim of the study was the estimation of the usefulness of the preparation Instillagel used at the time of the urinary bladder catheterization in the patients

Early Removal of the Urinary Catheter After Total or Tumor-Specific Mesorectal Excision for Rectal Cancer Is Safe.

BACKGROUND: Total or tumor-specific mesorectal excision can preserve pelvic autonomic nerves during rectal cancer surgery and minimize urinary dysfunction. However, urinary catheterization several days in duration is a common practice after total or tumor-specific mesorectal excision. OBJECTIVE: This study aimed to evaluate the optimal duration of urinary catheterization after total or tumor-specific mesorectal excision for rectal cancer. DESIGN: This is a retrospective review of patients who underwent total or tumor-specific mesorectal excision for rectal cancer. SETTINGS: This study was p

[Inquiry among general practitioners' knowledge regarding clean intermittent self-catheterization].

INTRODUCTION: Clean intermittent self-catheterization is the recommended mode of bladder voiding in patients with urinary retention. Hence, this procedure should be well known and understood by the general practitioners (GP) who are responsible for monitoring such patients. OBJECTIVE: To assess GP' state of knowledge regarding clean intermittent self-catheterization and to make recommendations for dealing with problems that occur during patient follow-up. METHOD: A questionnaire focusing on the practice of clean intermittent catheterization was sent to 910 family GP in the Franche-Comté regi

[Catheter-associated urinary tract infection].

Nosocomial urinary tract infections are the most common nosocomial infections. Their main risk factor is the urinary catheter The daily incidence of acquisition of microorganisms in the urine of a patient with a urinary catheter is 3 to 10% per day of catheterization, representing a cumulative risk of 100% after 30 days. The Gram-negative bacilli account for about two-thirds of isolated microorganisms led by Escherichia col. To prevent infections occurring with urinary catheterization, indications of the latter should be limited. When catheterization is essential, strict hygiene rules and prop

The effect of urethral introducer tip catheters on the incidence of urinary tract infection outcomes in spinal cord injured patients.

PURPOSE: We attempted to determine whether an introducer tip catheter reduces urinary tract infection in spinal cord injured patients on intermittent catheterization. MATERIALS AND METHODS: The introducer tip catheter bypasses the colonized 1.5 cm. of the distal urethra. Enrolled patients were prospectively entered into the study in alternate groups depending on whether they reflex voided: group 1--on intermittent catheterization with the introducer tip catheter but not voiding spontaneously or wearing an external urinary catheter, group 2--same as group 1 but using a nonintroducer tip cathet

Catheterization: a need for improved patient management.

The complications arising from catheterization of the urinary bladder have been extensively researched and reviewed. It is widely accepted that, because of inherent risks, catheterization is a last resort in the management of urinary incontinence. However, anecdotal evidence suggests that the incidence of long-term catheterization for this purpose is increasing, particularly for the management of highly dependent patients in the community. Although catheterization is a common procedure, a high level of nursing knowledge and skill is required to achieve effective and safe management. This artic

Clean intermittent catheterization and prevention of renal disease in spinal cord injury patients.

Upper urinary tract complications have been reported in about 20-30% of spinal cord injury patients. Their pathogenesis is linked to the presence of high-pressure uninhibited detrusor contractions, high leak point pressure and low bladder compliance. The aim of this study was to evaluate the incidence of upper urinary tract complications in two homogeneous groups of spinal cord injury patients with different bladder emptying modalities (intermittent catheterization vs. tapping, abdominal straining, Crede's manoeuvre) and the relationship with clinic and urodynamic features. A total of 17 patie

Urinary catheter in laparoscopic cholecystectomy: is it necessary?

The aim of this prospective study was to evaluate the necessity or urinary catheterization in elective laparoscopic cholecystectomy. From April 1996 to April 1998, 261 patients undergoing elective laparoscopic cholecystectomy at a county hospital were randomized to either receive or not receive preoperative urinary bladder catheterization. Data analyzed included age and gender of patients, length of surgery, and intraoperative and perioperative complications such as visceral injury, urinary tract infection, and urinary retention. Our results showed, although not statistically significant, more

Is it better to avoid urethral catheterization at hysterectomy and caesarean section?

This study involved 329 patients who had either a Caesarean section or a hysterectomy. A comparison has been made between 70 patients who were never catheterized and 251 who had a urethral catheter perioperatively. The absence of recognized urinary tract infections in those without a catheter was significant when compared with the 21 urinary infections identified in the catheterized group (p<0.05). The absence of urinary tract infections in the uncatheterized group clearly demonstrates the benefit of avoiding catheterization when possible.

Urinary retention in adults: diagnosis and initial management.

Urinary retention is the inability to voluntarily void urine. This condition can be acute or chronic. Causes of urinary retention are numerous and can be classified as obstructive, infectious and inflammatory, pharmacologic, neurologic, or other. The most common cause of urinary retention is benign prostatic hyperplasia. Other common causes include prostatitis, cystitis, urethritis, and vulvovaginitis; receiving medications in the anticholinergic and alphaadrenergic agonist classes; and cortical, spinal, or peripheral nerve lesions. Obstructive causes in women often involve the pelvic organs.

Intrapartum risk factors for postpartum urinary retention: a case-control study.

INTRODUCTION: Urinary voiding dysfunction is a common postpartum condition. Increased knowledge of risk factors for postpartum urinary retention could improve early identification of women at risk and lead to enhanced postpartum surveillance. We sought to identify intrapartum factors that contribute to postpartum urinary retention. METHODS: This retrospective case-control study compared subjects who developed postpartum urinary retention requiring indwelling catheterization after vaginal delivery to a control group who did not require catheterization. The control group was randomly selected i

Reducing use of indwelling urinary catheters and associated urinary tract infections.

BACKGROUND: Use of indwelling urinary catheters can lead to complications, most commonly catheter-associated urinary tract infections. Duration of catheterization is the major risk factor. These infections can result in sepsis, prolonged hospitalization, additional hospital costs, and mortality. OBJECTIVES: To implement and evaluate the efficacy of an intervention to reduce catheter-associated urinary tract infections in a medical intensive care unit by decreasing use of urinary catheters. METHODS: Indications for continuing urinary catheterization with indwelling devices were developed by u

Published evidence favors the use of suprapubic catheters in pelvic colorectal surgery.

INTRODUCTION: Bladder catheterization is routine during pelvic colorectal surgery, and transurethral catheterization is the norm. However, in gynecologic surgery suprapubic catheters are commonly used and are reported to be superior to urethral catheters. METHODS: A review of published studies comparing urethral and suprapubic catheters in patients undergoing colorectal surgery is presented. Level one evidence from randomized, controlled trials is sparse. Five randomized, controlled trials, most with small numbers, have been published comparing urinary tract infection, urinary retention, dura
