Selecting medications for the treatment of urinary incontinence.

In response to the growing population of older patients with incontinence, pharmaceutical companies are developing new drugs to treat the condition. Before prescribing medications for incontinence, however, physicians should determine the nature and cause of the patient's incontinence. The evaluation should rule out reversible conditions, conditions requiring special evaluation, and overflow bladder. The best treatment for urge incontinence is behavior therapy in the form of pelvic floor muscle exercises. Medications, used as an adjunct to behavior therapy, can provide additional benefit. Many

[Diagnosing urinary incontinence in the elderly woman].

The diagnostic work-up of the elderly incontinent woman should always be done on an individual basis. Temporary episodes of incontinence caused by the use of certain drugs or by difficulty in getting to a toilet can be identified by a stepwise diagnostic approach and easily dealt with, for example, by replacing the drug by another, or effecting changes in the environment that improve the patient's access to a toilet. A mandatory aspect of the systematic diagnostic work-up is an attempt to achieve a positive effect through the use, for example, of anticholinergics or local hormone replacement t

[Injectable therapy for urinary incontinence: a review].

Trans- or periurethral bladder neck injections are minimal invasive therapies for stress urinary incontinence which can be performed under local anaesthetic as an outpatient procedure. So far the best documented substance is collagen of bovine origin but other substances as silicone, autologous fat, chondrocytes and ethylenevinylalcohol are available. Success rates are between 60 and 80% with deteriorating long term results of approximately 30% requiring reinjection usually after 18 to 24 months.

Assessment of the female patient with urinary incontinence.

Understanding the pelvic floor anatomy and all of its components is crucial when performing pelvic floor examination and assessing patients suffering from urinary incontinence. It is important for urology nurses to understand the pelvic floor, including the muscle, nerve, and fascia involved; external and internal examination of the vulva; and global assessment of the patient.

Treatment of neuropathic urinary and faecal incontinence.

The author presents 14 children after meningomyelocoele repair with faecal and urinary incontinence, aged from 6 to 17 years who have undergone the MACE procedure. MACE with synchronous Mitrofanoff continent stoma creation was performed in one child with severe stenosis of the urethra. MACE with simultaneous bladder augmentation was used in 10 patients. 5 of them underwent colocystoplasty, 3 had ileocystoplasty and 2 ureterocystoplasty. Of these 10 children, three have additionally undergone the Mitrofanoff procedure. Patients' follow-up ranged from six months to two years. Three children had …

Urinary incontinence in a dog with an ectopic ureterocele.

A 7-month-old, female English cocker spaniel was examined because of a complaint of urinary incontinence. Excretory urography revealed a small right kidney and right-sided hydroureter, ectopic ureter, and ureterocele. Ureteronephrectomy and ovariohysterectomy were performed, but the distal ureter and ureterocele were left in situ. Recurrent urinary tract infections and intermittent urinary incontinence persisted after surgery. Vaginourethrography demonstrated the presence of a urethral diverticulum associated with the ureterocele. Ureterocelectomy was performed, and the dog remains continent 4…

[Clinical evaluation of urinary incontinence in the woman].

Clinical assessment of stress and urge incontinence in women is always necessary before making therapeutic decisions. During the full bladder examination stress incontinence triggered by cough may suggest cervicourethral hypermobility. Leakage during the Vasalva manoeuvre may reveal incontinence due to intrinsic sphincter deficiency. The pad test provides an objective quantitative measurement of the degree of incontinence. Symptom scores enable intra- and inter-individual comparisons. Psychosocial implications are studied with specific quality-of-life scales. Together, these tests improved the

Electrical stimulation for the treatment of urinary incontinence.

OBJECTIVES: To describe the rationale for the use of electrical stimulation (ES) for the treatment of urinary incontinence, and to summarize the results of clinical studies assessing these techniques. METHODS: The effect of ES on the lower urinary tract (LUT) is explained, and the literature on clinical application of this technique is reviewed. RESULTS: The use of ES is aimed at altering LUT function by stimulation of the sacral autonomic or somatic nerves. Two types of ES have been used: chronic stimulation and acute maximal functional electrostimulation. The frequency used depends on the …

[Urodynamic studies in the child with urinary incontinence].

Voiding disorders in children are frequent. To establish an exact anamnesis, it has to be distinguished between enuresis and incontinence and this may consequently already have a therapeutical effect. Enuresis is defined as a normal void occurring at an inappropriate time or place. Incontinence is an involuntary loss of urine and a pathological voiding habit, often in combination with urinary infections. The causes of an enuresis are always functional, the causes for incontinence may be organic or functional. Organic causes are neurogenic bladder dysfunctions or structural anomalies of the low

[Role of endoscopy in surgery for urinary incontinence].

In France, approximately 2 or 3 million women suffer from urinary incontinence. A consensus has been reached about the safety of treatment of genuine urinary stress incontinence by retropubic colposuspension (Burch procedure). The cure rate for the abdominal Burch procedure is 85% after 5 years. Since 1991, laparoscopic retropubic colposuspension has been advocated for the treatment urinary stress incontinence. The procedure can be performed either by laparoscopic intraperitoneal or extraperitoneal approaches or by associated laparoscopic and vaginal routes. The feasibility of laparoscopic col…

[The psychological status of the female with urinary incontinence].

In the former western part of Germany, in 1989 about three million people claimed social costs because of urinary incontinence. 80 percent of them were women. The psychic situation of these women is mainly determined by social consequences of their incontinence. Behavioural changes to avoid incontinence lead to social isolation. A coherence between incontinence or urgency and agony or depression is acknowledged. Especially in elderly people, who have to change their well-known environment due to nursing problems, incontinence is a major problem, which often deteriorates with the new situation.…

Surgical treatment and complications of urinary incontinence.

Numerous surgical approaches to the management of incontinence, particularly genuine stress incontinence, have been reported. No single operation is a panacea for the condition, and many factors must be taken into account before deciding which procedure is best tailored to an individual's needs. The procedures vary in their results, morbidity, and associated complications. Awareness of these previous complications encourages their avoidance and permits better management should they occur.

[Surgical therapy of urinary incontinence in the male].

From 1973 to 1990 a total of 340 male patients suffering from incontinence of various etiologies were treated by implantation of an artificial urinary sphincter. In 165 patients the sphincter was placed at the bladder neck, while 175 needed an implantation at the bulbous urethra. Because of the better results with regard to continence, we prefer the bladder neck implantation. The youngest patient treated was 12 years old and the oldest, 91. Most of the patients were between 40 and 80 years old. The highest continence rates were achieved in patients with postprostatectomy urinary incontinence. …

Medical correlates of urinary incontinence in the elderly.

A probability sample of noninstitutionalized elderly people in Washtenaw County, Michigan, was interviewed to determine the relationship between urinary incontinence and various health conditions. The results show that between both male and female respondents physical mobility problems, specific neurologic symptoms, lower urinary tract problems, bowel problems, respiratory problems, and history of genital surgery are more prevalent among those who are incontinent than among those who are continent. Additional factors associated with incontinence in females are: history of parent and sibling in…

Genital prolapse with and without urinary incontinence.

Procidentia, vaginal vault prolapse and severe cystocele may be associated with potential urinary incontinence, which becomes overt only after surgical repair of the genital prolapse. The normal support of the pelvic organs is provided by the pelvic diaphragm (levator ani and coccygeus muscles). The levator plate is a firm, muscular plate between the coccyx and anus formed by fusion of the levator ani muscles on each side. Recent investigators have indicated that the main mechanism for weakening the pelvic muscles occurs as a result of childbearing, when stretch injury of the pudendal nerve ca…

Role of ultrasound in urinary incontinence evaluation.

Transrectal ultrasound was used to assess anatomic support of the urethrovesical junction (UVJ) in continent and stress incontinent women. UVJ drop on straining of less than 1 cm as assessed by transrectal ultrasound correlated well with good support to the UVJ. Drop of UVJ of more than 1 cm on straining correlated with poor support to bladder neck and stress urinary incontinence. The transrectal technique is quick and easy to perform and interpret.
