Plug-based vascular closure device in transfemoral transcatheter aortic valve implantation.

OBJECTIVES: Failure of vascular closure device (VCD) is the most common cause of access-site vascular complications in transfemoral transcatheter aortic valve implantation (TAVI). The authors sought to determine if the systematic use of arteriotomy-site ballooning with concomitant manual compression following the delivery of a plug-based VCD (MANTA, Teleflex) can optimize toggle-plug assembly apposition to the common femoral artery (CFA) wall and improve the final hemostatic efficacy. METHODS: In this prospective, observational, single-center study, 323 consecutive patients undergoing transfe

A Novel Femoral Artery Compression Device Compared with Manual Compression for Hemostasis After Femoral Artery Access in Coronary Artery Catheterizations.

BACKGROUND: Femoral arterial access remains widely used in contemporary coronary catheterization procedures. Following the removal of the femoral sheath, hand compression is commonly employed to achieve hemostasis. Nevertheless, this method is time-consuming and imposes a burden on both the operator and the patient. OBJECTIVE: This study aims to investigate the hemostatic efficacy of the novel external femoral artery compression device (the TU device). MATERIALS AND METHODS: Patients enrolled in this study were randomly assigned in a 1:1 ratio to either the TU device (with a 3-2-1 protocol f

Impact of closure devices on vascular complication and mortality rates in TAVI procedures.

BACKGROUND: Currently two closure devices are available for the vascular access in TAVI procedures. Their impact on vascular complications and mid-term mortality is yet unknown. METHODS: Between 2009 and 2014, 398 patients underwent TAVI TF procedures in which two different closure devices were used, Prostar® XL (n=215) and Perclose-Proglide® (n=183). In the cases with Prostar we used one device and in cases with Perclose-Proglide, two devices. The two groups were compared with respect to the criteria of the Valve Academic Research Consortium (VARC) II. The mean follow-up period was 679.7±481

A soft thermal sensor for the continuous assessment of flow in vascular access.

Hemodialysis for chronic kidney disease (CKD) relies on vascular access (VA) devices, such as arteriovenous fistulas (AVF), grafts (AVG), or catheters, to maintain blood flow. Nonetheless, unpredictable progressive vascular stenosis due to neointimal formation or complete occlusion from acute thrombosis remains the primary cause of mature VA failure. Despite emergent surgical intervention efforts, the lack of a reliable early detection tool significantly reduces patient outcomes and survival rates. This study introduces a soft, wearable device that continuously monitors blood flow for early de

Transcatheter aortic valve replacement in obese patients: procedural vascular complications with the trans-femoral and trans-carotid access routes.

OBJECTIVES: Obesity may increase the risk of vascular complications in transfemoral (TF) transcatheter aortic valve replacement (TAVR) procedures. The transcarotid (TC) approach has recently emerged as an alternative access in TAVR. We sought to compare vascular complications and early clinical outcomes in obese patients undergoing TAVR either by TF or TC vascular access. METHODS: Multicentre registry including obese patients undergoing TF- or TC-TAVR in 15 tertiary centres. All patients received newer-generation transcatheter heart valves. For patients exhibiting unfavourable ileo-femoral an

Tunneled Ipsilateral Sheath: A Novel Technique in Endovascular Management of Superficial Femoral Artery Ostial Lesions.

Ostial lesions of the superficial femoral artery preclude the use of ipsilateral common femoral artery antegrade approach. Access via either the contralateral common femoral or the brachial arteries are the 2 alternative classical approaches. Conversely, using an ipsilateral antegrade approach, through a partially inserted sheath, usually leaves the sheath insecure and frequently hits ostial lesion itself during insertion. Dislodgment from the artery frequently occurs, resulting in loss of access. We describe a technique whereby we insert 2 to 3 cm of the sheath into the common femoral artery

Conservative management of iatrogenic superior vena cava (SVC) perforation after attempted dialysis catheter placement: case report and literature review.

A 68-year-old male with end-stage renal disease, congestive heart failure, and facial and bilateral arm swelling was referred for placement of a tunneled dialysis catheter. Distal left subclavian vein access was obtained. The procedure was complicated by iatrogenic perforation of the superior vena cava (SVC). This resulted in rapid development of a right-sided hemothorax and hemodynamic instability. A right-sided thoracostomy tube was placed to drain the pleural cavity. Extrapericardial perforation of the SVC can be managed conservatively in select cases without endovascular balloon dilatation

From Donor Liver to Recipient Pulmonary Artery: Embolization of a Transected Central Venous Catheter During Liver Transplantation, A Rare Complication of Organ Procurement.

Central venous catheters (CVCs) are essential in transplantation but can rarely fracture and embolize, with donor-to-recipient transmission … branch of the left lower lobe pulmonary artery, consistent with an embolized CVC tip. All recipient vascular access devices were removed intact, strongly suggesting a donor-origin catheter fragment transplanted with the liver graft. Alternative explanations, including unrecognized recipient catheter fracture and pre-existing foreign body, were considered but deemed less …

A minimally invasive technique for closing an iatrogenic subclavian artery cannulation using the Angio-Seal closure device: two case reports.

INTRODUCTION: In the two cases described here, the subclavian artery was inadvertently cannulated during unsuccessful access to the internal jugular vein. The puncture was successfully closed using a closure device based on a collagen plug (Angio-Seal, St Jude Medical, St Paul, MN, USA). This technique is relatively simple and inexpensive. It can provide clinicians, such as intensive care physicians and anesthesiologists, with a safe and straightforward alternative to major surgery and can be a life-saving procedure. CASE PRESENTATION: In the first case, an anesthetist attempted ultrasound-gu

The evolving role of drug-coated balloons for the treatment of complex femoropopliteal lesions.

Conventional therapies for transcatheter treatment of patients with infrainguinal obstructive disease remain compromised by high restenosis rates. Drug-coated balloons (DCB) offer a novel therapeutic alternative targeting the source of neo-intimal hyperplasia, without the need for a permanent endovascular scaffold and their inherent limitations. A systematic research of the medical databases (Pubmed) has been conducted for this up-to-date review. Key words, such as "drug-coated balloons" (DCB), "drug-eluting balloon," "in-stent-restenosis" (ISR), "de-novo stenosis," "plain old angioplasty," "a

Fully Endoscopic Mitral Valve Repair with Percutaneous Cannulation of Groin Vessels.

Endoscopic mitral valve surgery (EMS) has become a standard of care at specialized heart centers, further reducing surgical trauma compared to a traditional minimally invasive, thoracotomy-based approach. Exposure of the groin vessels for the establishment of cardiopulmonary bypass (CPB) via surgical cutdown in minimally invasive surgery (MIS) may result in wound healing disorders or seroma formation. The avoidance of surgical exposure of the groin vessels by using fully percutaneous techniques for the insertion of a CPB cannula with the implementation of vascular pre-closure devices has the p

Safety and Effectiveness of Ultrasound-Guided Percutaneous Versus Open Brachial Artery Access: Results of the Multicenter Prospective ARCHIBAL Study Percutaneous or Open Brachial Artery Access.

BACKGROUND: Although brachial access (BA) has become increasingly used in percutaneous endovascular procedures, results from head-to-head comparisons between open brachial access (OBA) and ultrasound-guided percutaneous brachial access (PBA) are limited in the literature. The objective of our multicenter, prospective, noninterventional ARCHIv BrachiAL study was to compare the safety and efficacy of OBA and PBA to perform peripheral arterial interventions. METHODS: From July 2019 to January 2021, all patients needing peripheral endovascular procedures with brachial artery access were consecuti

Transradial Endovascular Treatment of Acute External Iliac Artery Rupture in Hemorrhagic Shock.

Iliac artery rupture is a rare but life-threatening complication of transfemoral vascular procedures that typically requires urgent intervention. Traditional approaches use transfemoral or transbrachial access; however, transradial access may offer a safer alternative in selected patients. We report the case of a 68-year-old man with ST-segment elevation myocardial infarction and shock who underwent primary percutaneous coronary intervention via right radial access with intra-aortic balloon pump (IABP) support through the right femoral artery. Following staged coronary intervention and IABP re

Transcaval access for the emergency delivery of 5.0 liters per minute mechanical circulatory support in cardiogenic shock.

OBJECTIVES: The purpose of this study was to describe the feasibility and early outcomes of transcaval access for delivery of emergency mechanical circulatory support (MCS) in cardiogenic shock. BACKGROUND: Vascular access for implantation of MCS in patients with cardiogenic shock is often challenging due to peripheral arterial disease and vasoconstriction. Transcaval delivery of MCS may be an alternative. We describe a series of patients we implanted an Impella 5.0 device, on-table without CT planning, through a percutaneous transcaval access route. METHODS: Ten patients with progressive or

Intrastent HeROgraft deployment as a catheter-sparing procedure.

Current vascular access (VA) practice adopts arteriovenous fistulas (AVF) as first option for haemodialysis, relegating arteriovenous grafts (AVG) for patients with exhausted upper limb venous patrimony. The Hemodialysis Reliable Outflow graft (HeRO®) is a device assuring direct venous outflow to the right atrium, thus avoiding central venous obstructive disease. Its use together with early access grafts avoids the need for central venous catheters (CVC) bridging periods. We report the deployment of the HeRO device using a previous stentgraft as pathway for the placement of the outflow compone

A Six-Step Approach for Optimizing Ultrasound-Guided Femoral Artery Access.

Femoral arterial access is fundamental to interventional cardiology, especially for complex procedures that require large-bore devices. However, vascular complications arising from suboptimal puncture techniques highlight the need for meticulous practice. This article presents a systematic, ultrasound-guided approach for achieving an optimal common femoral artery (CFA) puncture, defined by five key criteria: access within the CFA, anterior wall-only penetration, centralized lumen entry, puncture above the femoral head, and entry into a healthy arterial segment. We detail a six-step protocol in
