Remodeling of the External Respiratory System in Chronic Heart Failure - a Factor of Pathogenesis and a Therapeutic Target.

During the development of chronic heart failure (CHF), the lungs and the external respiratory system (ERS) as a whole undergo significant re … es (RM) due to the decreased compliance of the lung tissue and airways, and, accordingly, increased work of breathing typical of patients with CHF. In such conditions, tachypnea becomes the only available mechanism for maintaining the required ventilation volume. As the exercise load increases, the inability to adequately increase the depth of breathing contributes …

Patients' perceptions of asthma control and impact on attitudes and self-management.

The objective of this work was to describe the incidence of chronic breathing problems, particularly asthma-related breathing problems, in a cross-section of the United States population and to assess the perceived impact of these problems on daily living. An initial screening survey was used in a nationwide panel of 30,000 households; an in-depth follow-up questionnaire was sent to a random sample of respondents who reported a breathing problem. A sample (n = 2,685) of respondents who reported persistent cough, shortness of breath, or wheezing within the previous 2 years were sent a newly dev

Breathing exercise combined with cognitive behavioural intervention improves sleep quality and heart rate variability in major depression.

AIMS AND OBJECTIVES: The aim of this study was to investigate the effects of a cognitive behavioural intervention combined with a breathing relaxation exercise on sleep quality and heart rate variability in patients with major depression. BACKGROUND: Depression is a long-lasting illness with significant effects not only in individuals themselves, but on their family, work and social relationships as well. Cognitive behavioural therapy is considered to be an effective treatment for major depression. Breathing relaxation may improve heart rate variability, but few studies have comprehensively e

Analysis of a Broad Range of Carbonyl Metabolites in Exhaled Breath by UHPLC-MS.

Analysis of volatile organic compounds (VOCs) in exhaled breath (EB) has shown great potential for disease detection including lung cancer, infectious respiratory diseases, and chronic obstructive pulmonary disease. Although many breath sample collection and analytical methods have been developed for breath analysis, analysis of metabolic VOCs in exhaled breath is still a challenge for clinical application. Many carbonyl compounds in exhaled breath are related to the metabolic processes of diseases. This work reports a method of ultrahigh-performance liquid chromatography coupled with high-res

Viral Induced Hypersecretion of Mucous (VIper): Proposal for a Distinct Phenotype of Viral Induced Lower Respiratory Tract Infection in Children.

Some children with an acute viral lower respiratory tract illness present with a phenotype that overlaps with, but is distinct from, that of asthma. These children have shortness of breath with increased work of breathing, wet cough, hypoxia out of proportion to the work of breathing, incomplete resolution of hypoxia with supplemental oxygen, worsening of oxygen saturation after salbutamol and improvement of oxygen saturation with coughing. The pathophysiology appears to relate to relate to excess mucous secretion in the airways causing ventilation:perfusion (V/Q) mismatching, which is then ex…

Classification of chest wall diseases.

Several disorders of the thoracic cage are known to cause respiratory failure, by means of relatively simple mechanisms, such as the increased work of breathing, which results in alveolar hypoventilation. A variety of pathogenic mechanisms may be considered, as functions of the types of thoracic disorders present. As causes of these additional potential mechanisms, we considered the following: 1) ventilation-perfusion (V/Q) inhomogeneity; 2) inability to cough; 3) malformation or acquired defect of the respiratory centres; and 4) excess blood volume and fluid retention, which aggravate work of…

Interventions for apnoea of prematurity: a personal view.

AIM: To review treatments for apnoea of prematurity (AOP). METHODS: Literature Review and description of personal practice. RESULTS: Provided that symptomatic apnoea has been ruled out, interventions to improve AOP can be viewed as directed at one of three underlying mechanisms: (i) a reduced work of breathing [e.g. prone positioning, nasal continuous positive airway pressure (CPAP)], (ii) an increased respiratory drive (e.g. caffeine), and (iii) an improved diaphragmatic function (e.g. branched-chain amino acids). Most options currently applied, however, have not yet been shown to be effect…

Advanced high pressure hyperbaric techniques in tunnelling.

Work in compressed air and diving are both occupational activities that have been around since the mid-19th century, and those undertaking their work under elevated pressure. Meeting the demand to go to "higher pressure for longer" in tunneling has lagged in diving, but both activities have found it necessary to adopt mixed gas breathing and saturation exposure techniques. This paper explains how work in hyperbaric conditions at high pressure is undertaken in tunneling and is illustrated by the hyperbaric activity likely to be involved in constructing a large-diameter road tunnel below a body

Multidimensional aspects of dyspnea in obese patients referred for cardiopulmonary exercise testing.

We investigated the contributions of obesity on multidimensional aspects of dyspnea on exertion (DOE) in patients referred for clinical cardiopulmonary exercise testing (CPET). Ratings of perceived breathlessness (RPB, Borg scale 0-10) were collected in obese (BMI ≥ 30; n = 47) and nonobese (BMI ≤ 25; n = 27) patients during two (one lower: ∼30 W; and one higher: ∼50 W) 4-6 min constant load cycling bouts. Multidimensional dyspnea profiles (MDP) were collected in the final 26 obese and 14 nonobese patients of the sample. RPB was greater (p = 0.05) in obese (3.3 ± 2.2 vs 2.4 ± 1.4) at lower wor

Effects of high relative humidity and dry purging on VOCs obtained during breath sampling on common sorbent tubes.

Offline breath analysis by thermal desorption gas chromatography mass spectrometry (TD-GC-MS) requires the use of sorbent traps to concentrate and store volatile compounds. The selection of which sorbent to use and best practices for managing high relative humidity are important considerations to allow for reproducible, untargeted, biomarker discovery in water saturated breath samples. This work aims to assess three commonly used sorbent materials for their use in breath volatile sampling and determine how the high relative humidity inherent in such samples effects the capture of volatile comp

A unidirectional breathing pattern improves breathing efficiency in subjects with severe COPD.

BACKGROUND: Unidirectional breathing (UB), nose-in mouth-out (NMB) or vice versa, is thought to create PEEP, stabilize small airways, and increase expiratory flow and exhaled tidal volume (VT) in patients with expiratory obstructive disorders. However, the exact mechanism providing the benefits of UB remains unknown. Our hypothesis was that the benefits of UB are achieved mainly through reduction of upper airway dead space. METHODS: Sixteen stable COPD patients requiring oxygen use at home were enrolled in this prospective study at a tertiary health care center. A nasal mask and a mouthpiece

Sleep-disordered breathing - clinical spectrum.

The clinical spectrum of sleep-disordered breathing comprises a range of diverse conditions including obstructive sleep apnoea, central sleep apnoea and sleep-related hypoventilation syndromes. These distinct conditions have specific diagnostic features and are managed differently from one another. Therefore, it is useful for dental practitioners to have a working knowledge of sleep-disordered breathing beyond that of uncomplicated obstructive sleep apnoea (OSA). This review paper summarizes the diagnosis and management of commonly encountered clinical sleep-disordered breathing syndromes, wit

Breathing pattern variability during bronchial histamine and methacholine challenges in asthmatics.

Breathing pattern variability was determined in 10 asthmatic adolescents during repeated bronchial histamine and methacholine challenges (HiCh/MeCh). The purpose was to provide information on ventilatory control in asthmatics by comparing the variability of the various breathing pattern parameters at rest and during induced bronchial obstruction. Changes in variability during bronchial obstruction might be explained by either anxiety effects causing increased variability or by the minimization of the work of breathing causing decreased variability. Ventilation was monitored by respiratory indu…

Quantitative mapping of cerebrovascular reactivity amplitude and delay with breath-hold BOLD fMRI when end-tidal CO 2 quality is low.

Cerebrovascular reactivity (CVR), the ability of cerebral blood vessels to dilate or constrict in response to a vasoactive stimulus, is a clinically useful measure of cerebrovascular health. CVR is often measured using a breath-hold task to modulate blood CO2levels during an fMRI scan. Measuring end-tidal CO2(PETCO2) with a nasal cannula during the task allows CVR amplitude to be calculated in standard units (vascular response per unit change in CO2, or %BOLD/mmHg) and CVR delay to be calculated in seconds. The use of standard units allows for normative CVR ranges to be established and for CVR

Characteristics of breathing-adapted gating using surface guidance for use in particle therapy: A phantom-based end-to-end test from CT simulation to dose delivery.

To account for intra-fractional tumor motion during dose delivery in radiotherapy, various treatment strategies are clinically implemented such as breathing-adapted gating and irradiating the tumor during specific breathing phases. In this work, we present a comprehensive phantom-based end-to-end test of breathing-adapted gating utilizing surface guidance for use in particle therapy. A commercial dynamic thorax phantom was used to reproduce regular and irregular breathing patterns recorded by the GateRT respiratory monitoring system. The amplitudes and periods of recorded breathing patterns we

[The hyperventilation syndrome].

Acute hyperventilation (HV) rarely poses diagnostic or therapeutic problems. Chronic hyperventilation, however, with vague and multiple symptomatology due to respiratory alkalosis and increased breathing work, is often overlooked or misinterpreted, though it is a very common disorder of the general patient population. Chronic HV is frequently associated with emotional disturbances such as anxiety, panic and depression, or with psychosomatic disorders such as irritable bowel, effort syndrome and chronic pain. The diagnosis of chronic HV relies primarily on taking of a thorough history and is co

Short term evaluation of respiratory effort by premature infants supported with bubble nasal continuous airway pressure using Seattle-PAP and a standard bubble device.

BACKGROUND: Almost one million prematurely born infants die annually from respiratory insufficiency, predominantly in countries with limited access to respiratory support for neonates. The primary hypothesis tested in the present study was that a modified device for bubble nasal continuous positive airway pressure (Bn-CPAP) would provide lower work of spontaneous breathing, estimated by esophageal pressure-rate products. METHODS: Infants born <32 weeks gestation and stable on Bn-CPAP with FiO2 <0.30 were studied within 72 h following delivery. Esophageal pressures during spontaneous breathing
