A Case of Penetrating Head Wound Due to Helicopter Rotor Blade Injury in a 34-Year-Old Naval Helicopter Pilot Who Returned to Active Service 5 Years Later.

BACKGROUND Head trauma, defined as damage to the brain, skull, or scalp when the head is hit by an external force, is a major cause of mortality in military personnel. Therefore, we report a novel case involving a naval helicopter pilot who sustained a helicopter propeller rotor blade injury. CASE REPORT We describe a case involving a pilot struck on the head by a helicopter rotor blade. He received care from medical staff shortly after the injury and was en route to the nearest trauma center. Cranial computed tomography (CT) scans revealed a comminuted fracture of the right occipital bone, wi

Establishment of a Systemic Inflammatory Response Syndrome Model and Evaluation of the Efficacy of Umbilical Cord Mesenchymal Stem Cell Transplantation.

Based on the characteristics of modern weapon injury, a repetitive model of traumatic systemic inflammatory response syndrome (SIRS) and an evaluation system were established. The models were treated with GFP-labeled tree shrew umbilical cord mesenchymal stem cells (UCMSCs). Forty out of 50 tree shrews were used to make a unilateral femoral comminuted fracture. Lipopolysaccharide was injected intravenously to create a traumatic SIRS model. The other 10 shrews were used as normal controls. After the model was established for 10 days, 20 tree shrews were injected intravenously with GFP-labeled U

Detection and DNA quantification of Enterococcus casseliflavus in a foal with septic meningitis.

CASE DESCRIPTION A 3-month-old 180-kg (396-lb) Hanoverian colt was examined because of fever, lethargy, inappetence, drooping of the left ear, and stiff neck posture. Initial treatment included empirical antimicrobial treatment and NSAIDs. CLINICAL FINDINGS Initial findings were consistent with CNS anomalies. Endoscopy revealed hyperemia, ecchymosis, and some mucopurulent exudate in the right guttural pouch. Hematologic findings were consistent with neutrophilic inflammation. On the third day of hospitalization, severe neurologic signs were observed. Computed tomography of the skull revealed a

Variable pitch headless compression screw treatment of distal phalangeal nonunions.

Nonunions of the shaft of the distal phalanx, as opposed to the tuft, are usually symptomatic enough to warrant treatment. One method that has proven successful while minimizing complications is compression fixation using variable pitch headless screws inserted percutaneously. Eighteen patients (15 males, 3 females) with a mean age of 48 years were so treated under digital block anesthesia. Seven of 18 (39%) patients were smokers. All initial injuries were crush trauma to the distal phalanx with 10/18 (56%) initially open fractures. Treatment was to 3 thumbs, 6 index fingers, 6 long, 2 ring, a

Congenital hip disease in adults. Classification of acetabular deficiencies and operative treatment with acetabuloplasty combined with total hip arthroplasty.

We describe three distinct types of congenital hip disease in adults. The first type is dysplasia, in which the femoral head is contained within the original true acetabulum. The second type is low dislocation, in which the femoral head articulates with a false acetabulum, the inferior lip of which contacts or overlaps the superior lip of the true acetabulum, giving the appearance of two overlapping acetabula. The third type is high dislocation, in which the femoral head has migrated superoposteriorly and there is no contact between the true and the false acetabulum. We describe and classify t

Minimal Pain Decrease Between 2 and 4 Weeks After Nonoperative Management of a Displaced Midshaft Clavicle Fracture Is Associated with a High Risk of Symptomatic Nonunion.

BACKGROUND: The main long-term benefit of operative treatment of displaced midshaft clavicular fractures is the reduction in nonunion risk, and as this risk is generally low, the ideal approach would be to operate only patients at high risk of nonunion. However, most current surgical decision models use baseline variables to estimate the nonunion risk, and the value of these models remains unclear. Pain in the early weeks after fracture could be potentially be an indirect measurement of fracture healing, and so it is a potential proxy variable that could lead to simpler prediction models. QUE

[Management of complex intra-articular distal radius fractures with open reduction and internal fixation with double dorsal locking plates].

AIM: In this prospective study we have analysed the clinical outcome of complex distal radius fractures that were reduced by a dorsal approach and fixed with 2 dorsal locking plates. METHODS: In the period from January 2006 to June 2008 60 patients with a distal intra-articular instable radius fracture, who were treated with the above mentioned procedure, were prospectively included in this study. All fractures showed signs of a complex and dorsal instability. X-rays and CT scans were taken preoperatively. Follow-up examinations and X-rays were conducted after 6 weeks, 6 months and 1 year. Th

Management of Grade 3C Compound Injury of Lower Limb with Floating Knee - Salvage versus Amputation (Case Series).

INTRODUCTION: Severe open injuries of limbs, especially of the femur and tibia when associated with vascular injuries, present major challenges in management. The decision to amputate or salvage can often be a difficult one even for experienced surgeons. Mangled lower extremity results due to high-energy trauma, especially due to motor vehicle accidents, and is defined as injury to three of the four systems in the extremity that is soft tissues, bone, vascular, and nerve. Open fractures are classified by Gustilo and Anderson's classification in which type 3B is an injury where soft-tissue loss

Subaxial cervical spine fractures with facet joint dislocation: Surgical management, technique, and results in a retrospective series of 36 patients.

OBJECTIVE: Subaxial cervical spine fractures with facet dislocation are burdened by a high risk of neurological impairment. The surgical management of this severe clinical condition is poorly standardized, concerning the role of closed reduction and the use of anterior, posterior, or circumferential approaches. We performed a retrospective evaluation of our series to assess the safety and efficacy of our surgical strategy. MATERIALS AND METHODS: Transcranial traction was progressively applied, followed by anterior decompression and arthrodesis. Whether adequate reduction was not obtained with

From suspected necrotizing fasciitis to diagnosed drug-induced fever: a diagnostic maze and reflections on anti-infection management in a case of postoperative fever after femoral fracture.

Postoperative fever represents a common clinical challenge in orthopedics, and the differential diagnosis of its etiology directly influences anti-infective decision-making and patient outcomes. In the context of anti-infective therapy, "drug fever" induced by antimicrobial agents themselves can easily be mistaken for uncontrolled or recurrent infection, leading to antibiotic overuse. This article reports the case of a 49-year-old male with a comminuted fracture of the right femur. Preoperatively, he presented with post-traumatic absorption fever. Postoperatively, due to intraoperative observa

A Case of Fibula Regeneration after Below-the-knee Amputation in an Adult.

We report the case of an adult with fibula regeneration after below-the-knee amputation. Fibula regeneration conventionally occurs at the donor site of children after autogenous fibula transplantation when the periosteum is preserved. However, the patient was an adult, and the regenerated fibula was 7-cm long and grew directly from the stump. A 47-year-old man was referred to the plastic surgery department owing to stump pain. He had an open comminuted fracture of the right fibula and tibia due to a traffic accident when he was 44 years old and underwent below-the-knee amputation and negative

Influence of additional cement augmentation on endplate stability in circumferential stabilisation of osteoporotic spine fractures.

BACKGROUND: Anterior stabilisation of osteoporotic spine fractures is uncommon but necessary in the case of complex vertebral body comminution. The purpose of this study was to investigate the effect of additional cement-augmentation on the endplate stability. METHODS: Twelve human cadaveric lumbar spines were divided in two groups: (A) posterior cement-augmented pedicle screw/rod-based instrumentation of L3 to L5, posterior decompression of L4/5 and partial corpectomy of L4 and (B) same experimental setup with additional cement-augmentation of the adjacent endplates. A cyclic loading test wa

Reconstruction of the coronoid process using the tip of the ipsilateral olecranon.

BACKGROUND: Autograft reconstruction of the coronoid using the tip of the olecranon has been described as a treatment option for comminuted coronoid fractures or coronoid nonunions that are not repairable. The purpose of this in vitro biomechanical study of the coronoid-deficient elbow was to determine whether coronoid reconstruction using the tip of the ipsilateral olecranon would restore elbow kinematics. METHODS: An elbow motion simulator was used to perform active and passive extension of six cadaveric arms in the horizontal, valgus, varus, and vertical orientations. Elbow kinematics were

What is your diagnosis? Blood smear from an injured red-tailed hawk.

An injured juvenile red-tailed hawk (Buteo jamaicensis) was evaluated at the Veterinary Medical Teaching Hospital at the University of California, Davis. The hawk was quiet, alert, and emaciated, and had a closed comminuted, mid-diaphyseal ulnar fracture. CBC results included heterophilia with a left shift, monocytosis, and increased plasma fibrinogen concentration. The blood smear included rare heterophils containing small, dark blue inclusions approximately 1-2 mum in diameter that ranged from round to coccobacillary in shape and formed variably shaped aggregates; the morphology of the inclu

Open Reduction and Internal Fixation Versus Total Elbow Arthroplasty for Intra-articular Distal Humerus Fractures in Patients 65 Years or Older: A Meta-analysis.

BACKGROUND: Distal humerus fractures (DHF) in the elderly pose a challenge due to osteoporotic bone, comminution, and potential for poor functional recovery. Surgical management typically involves open reduction and internal fixation (ORIF) or total elbow arthroplasty (TEA). However, inconsistent findings across studies have led to uncertainty regarding which intervention optimizes outcomes, underscoring the need for a rigorous meta-analysis to guide clinical decision making. METHODS: A systematic literature search was conducted across PubMed, Scopus, Cochrane Library, and Google Scholar for

Clinical and radiographic results of locking plate with medial support screw in Proximal Humerus fracture - the more, the better?

BACKGROUND: The calcar of the proximal humerus is a fundamental structure for medial humeral column support. This study aimed to assess the outcome of osteosynthesis across cases of unstable proximal humerus fractures (PHFs) with medial calcar comminution, following treatment with a PHILOS locking plate and medial support screw (MSS). METHODS: Between January 2010 and December 2018, we retrospectively analyzed the outcomes of 121 cases of osteosynthesis for PHFs with medial column disruption. For the medial support, at least one oblique screw was inserted within 5 mm of the subchondral bone i
