Don't Miss This! Red Flags in the Pediatric Eye Exam: Pupils.

The pupillary exam in the pediatric population is a vital part of any clinician's workup. In the right clinical setting, pupillary abnormalities such as anisocoria, light-near dissociation, an afferent pupillary defect, and paradoxic pupillary constriction in the dark can be red flags that trigger further examination and workup. Through both careful physical examination and detailed history-taking and observation, potentially vision- and life-threatening conditions can be detected.

An Interesting Case of Gradenigo Syndrome.

Gradenigo syndrome is a rare complication of acute otitis media infections, and early diagnosis and treatment of the condition are imperative. In this report, we present a case of a 22-month-old girl who developed acute, acquired esotropia associated with recurrent episodes of otitis media, which resolved after antibiotic treatment. To our knowledge, our patient is one of the youngest children reported in the literature to present with presumed Gradenigo syndrome and the only case of recurrent episodes of acquired esotropia and concurrent otitis media within the short time span of 1 month. We

The Non-surgical Management of Ophthalmoplegia.

Ophthalmoplegia can result from damage or dysfunction of the supranuclear eye movement pathways, the brainstem internuclear pathways, or the ocular motor nerves. Diplopia and impaired eye movements are commonly associated symptoms. The goal of non-surgical treatment is usually management of symptoms, and the primary tool used is prism. Results of recent studies in the neurorehabilitation literature have suggested that eye muscle exercise may do more than compensate for symptoms of ophthalmoplegia. They may accelerate recovery for those conditions in which recuperation is possible, and restore

Red Flags in the Assessment of Adult Ophthalmoplegia.

Acquired ophthalmoplegia can represent a harbinger of vision and life-threatening disease. Recognition of select clinical "red flags" on history or examination can facilitate early diagnosis. This article reviews relevant clinical pearls for the detection of dire causes of acquired ophthalmoplegia in adults including myasthenia gravis, carotid cavernous fistula, giant cell arteritis, and thiamine deficiency. Prompt treatment of all of these disorders can either reverse or halt the progression of visual manifestations.

Spasm of the Near Reflex: Literature Review and Proposed Management Strategy.

BACKGROUND AND PURPOSE: Spasm of the near reflex (SNR) can be a variable, unpredictable condition. There is no standard management strategy. The purpose of this paper is to summarize the pertinent information from a literature review to better identify, explain, and manage this condition. PATIENTS AND METHOD: Case presentations and retrospective literature review Results: The presumed cause of SNR can be grouped into four categories: psychogenic, head trauma, organic/neurologic, and other causes. Etiology is often presumed rather than confirmed and may be multifactorial. A number of treatment

Isolated Ocular Motor Nerve Palsies.

Isolated cranial nerve (CN) palsies are frequently encountered by strabismologists. Management decisions for patients with isolated ocular motor nerve palsies must take into account multiple factors, including patient age, historical features, the specific CN affected, examination findings, and coexistent medical diagnoses. In general, isolated ocular motor nerve palsies in children are assessed and managed differently than adults. Furthermore, older adults with vascular risk factors may be managed differently than younger adults and children. The need for urgent neuroimaging in older adults w

Ophthalmoplegia and Congenital Cranial Dysinnervation Disorders.

Some forms of ophthalmoplegia are congenital and fall into the category of Congenital Cranial Dysinnervation Disorders (CCDDs). These disorders arise from a primary defect of cranial nucleus/nerve development or guidance. Many have substantial limitations of ocular motility with or without other associated features. The type and degree of ophthalmoplegia can be similar between CCDD subtypes as well as with non-congenital forms of ophthalmoplegia. Therefore diagnostic confirmation often requires neuro-imaging and/or genetic investigations. The clinician should consider this category in cases of

Long-Term Follow-up of Spasmus Nutans.

PURPOSE: Spasmus nutans is an acquired form of nystagmus that is classically associated with torticollis and titubations of the head, often presenting in the first year of life and spontaneously resolving within the next 2 years. The purpose of our study was to record and characterize the long-term prognosis of children diagnosed with spasmus nutans. METHODS: All patients under the age of 21 years with a diagnosis of spasmus nutans were included. Parameters of age, nystagmus symmetry and quality, presence of titubations, torticollis, and strabismus, and findings on CT or MRI were recorded. R

Medical interventions for traumatic hyphema.

BACKGROUND: Traumatic hyphema is the entry of blood into the anterior chamber, the space between the cornea and iris, following significant injury to the eye. Hyphema may be associated with significant complications that uncommonly cause permanent vision loss. Complications include elevated intraocular pressure, corneal blood staining, anterior and posterior synechiae, and optic nerve atrophy. People with sickle cell trait or disease may be particularly susceptible to increases in intraocular pressure and optic atrophy. Rebleeding is associated with an increase in the rate and severity of comp

Successful extraocular muscle re-resection for a strabismus surgery complication: a "snapped" [severed, inadvertently] and retrieved inferior rectus muscle: a case report.

A 26- year-old woman who had a left head tilt since childhood was undergoing left inferior rectus resection to correct her left inferior rectus paresis. During the surgery, when the inferior rectus was isolated and engaged with an eye muscle hook, the muscle tore (snapped) completely into two pieces, 8 mm posterior to insertion. Fortunately, we were able to find the proximal portion of the muscle and, after a 3 mm resection, of the distal yet attached 8 mm portion, the proximal and distal portions were sutured together with a non-absorbable suture. After nine months followup there was signific

Stage III Intraoperative Adjustmentof Eye Muscle Surgery (Under General Anesthesia) for Neuroparalytic and Mechanical (Restrictive) Incomitant Strabismus: Report of Results in a Series: Outcomes in 20 Eye Muscle Surgeries in Twelve Patients.

Techniques for the adjustment of surgery intraoperatively (especially those termed Stage I and II techniques) have proven maximally successful in improving surgical results for comitant strabismus. Stage III adjustments (end-operative) have been described but not studied. In a retrospective study of 20 eye muscle procedures in 12 patients with neuroparalytic and mechanical strabismus, the usefulness of various intraoperative adjustment techniques Stage I, II, and III was investigated for the first time. Stage I adjustments (adjusting the surgical plan based on the binocular misalignment follow

Inferior oblique muscle palsy with "paradoxical" v-pattern strabismus.

BACKGROUND AND PURPOSE: A- and V-patterns are commonly encountered with cyclovertical muscle palsies. Inferior oblique (IO) muscle palsy produces and A-pattern caused mainly by the decreasing abducting action of the IO in upgaze and an increasing abduction action of the superior oblique in downgaze. V-pattern association with an IO palsy has not been, to our knowledge, reported before. The purpose of our paper is to report on a patient iwth IO palsy and a paradoxical V-pattern and explain the proposed pathophysiology behind the V-pattern. CASE REPORT: We report a 67 year old female with a 3 y

Strabometry precision: intra-examiner repeatability and agreement in measuring the magnitude of the angle of latent binocular ocular deviations (heterophorias or latent strabismus).

BACKGROUND AND PURPOSE: This study was designed to compare heterophoria measurements obtained using the methods: prisms cover test (prism alternate cover test), von Graefe technique, Maddox rod test and modified Thorington test. Given the different methodological features of these tests, our working hypothesis was that these tests would not be interchangeable and repeatability would vary. METHODS: Horizontal deviation measurements were made at: far distance (six meters) and near distance (40 centimeters) on two occasions in 61 young subjects of mean age 19.7 years (range 18 -32 years), with e
