The "Trimeric" Anterolateral Thigh Free Flap: A Novel Technique for the Reconstruction of Complex Cranio-Orbital and Middle Skull Base Defects.

BACKGROUND AND IMPORTANCE: The anterolateral thigh (ALT) free flap is a versatile reconstructive option for complex 3-dimensional defects of the head and neck. It can be tailored for cutaneous, fasciocutaneous, musculocutaneous, or flow-through configurations based on patient needs. Chimeric flaps comprise distinct tissue components on a shared vascular pedicle and allow simultaneous functional and aesthetic reconstruction in a single stage. We present a novel extension of this concept incorporating a third vascularized component to address multilayer skull base defects. CLINICAL PRESENTATION

Prognostic Indices for Mortality Prediction in Fournier's Gangrene: A Comparative Study Introducing PCT-ALLY.

BACKGROUND: Fournier's gangrene is a life-threatening necrotizing fasciitis with mortality rates of 20%-50%. Early identification of high-risk patients is essential, yet practical prognostic tools derived from routine laboratory tests remain limited. AIM: To compare seven prognostic indices and develop a novel procalcitonin-based index (PCT-ALLY) for mortality prediction in patients with Fournier's gangrene. METHODS: In this single-center retrospective cohort, 220 surgically treated patients (January 2007-September 2025) were evaluated. Preoperative laboratory data were used to calculate neu

Development of polymerase chain reaction assays specific for individual pathogenic Aeromonas species.

The genus Aeromonas, a group of Gram-negative bacilli, inhabits a wide variety of aquatic environments, including rivers, lakes, and coastal seas. Among Aeromonas species, A. hydrophila, A. caviae, A. veronii, and A. dhakensis are clinically important species that cause infections in humans, leading to diarrhea associated with gastroenteritis, necrotizing fasciitis, and sepsis. A. dhakensis is a newly established species that was reclassified in 2013, and it is considered more pathogenic than other Aeromonas species. However, accurate identification of Aeromonas species by biochemical characte

Additional biomarkers and emm types associated with group A streptococcal toxic shock syndrome: a Japanese nationwide observational study.

PURPOSE: The incidence of invasive Group A Streptococcus (iGAS) infection and streptococcal toxic shock syndrome (STSS) is increasing. Early detection and diagnosis of cases that may progress to STSS are currently difficult. In this study, we aimed to identify biomarkers and emm type, one of the virulence factors, associated with STSS development. METHODS: In this multicentre observational study including patients with iGAS infection (n = 305), we investigated the relative associations of host factors, clinical manifestations, biomarkers, and emm type with STSS. RESULTS: The overall mortalit

Fibrosis Management Algorithm in Secondary Liposculpture.

BACKGROUND: Liposuction is considered one of the most common procedures in plastic surgery. However, major postoperative complications such as visceral injury, fluid overload, and necrotizing fasciitis still occur. Likewise, minor complications such as ecchymosis, seromas, infections, and contour irregularities that do not threaten the life of the patient do generate significant dissatisfaction. Current evidence regarding the management of fibrosis after previous liposuction remains limited. OBJECTIVES: The objective of this article is to standardize a management algorithm based on the extens

Fournier's gangrene originating from the perianal region or the urogenital area: does it make a difference?

OBJECTIVE: Fournier's gangrene (FG) is a rare and serious disorder which is associated with high mortality. In the literature, there is no study evaluating clinician-, patient- and disease-related factors affecting disease outcomes according to aetiological variation in FG. In our study, laboratory results and Uludag Fournier's Gangrene Severity Index (UFGSI) score, clinical characteristics and mortality rates were compared between FG originating from perianal or from urogenital regions. METHOD: Platelet-to-lymphocyte ratio, neutrophil-to-lymphocyte ratio, Laboratory Risk Indicator for Necrot

Reconstruction with bilateral posteromedial thigh (PMT) flaps after Fournier's gangrene.

Fournier gangrene (FG) is a deadliest condition affecting genitoperineal area in predisposed patients. A late diagnosis, thus a delayed surgical treatment, leads often to death. LRINEC score and CT scan can help in suspect, despite definitive diagnosis needs surgical exploration and histological findings. Furthermore, FG determines wide defects of genitoperineal area, thus reconstructive surgery is pivotal to restore form and function of the affected patient. Aim of this article is to discuss the use of posteromedial thigh (PMT) fasciocutaneous flap in FG reconstruction, based on authors' pers

Post-partum rectus sheath hematoma complication: case report.

Pregnancy, surgery, and trauma are known risk factors for the development of rectus sheath hematomas (RSHs). The exact incidence of RSH during pregnancy is unknown, however this disease carries a risk of complication and even mortality. The role of infection in this condition is less known with only a few reported case studies in medical literature. In our case a 26-year-old G4P2113 1-month status post cesarean section presented with 2 weeks of progressive lower abdominal pain after her 50-pound daughter jumped on her. Upon further work up a computed tomography (CT) scan found a 15-cm RSH. Exp

Prepatellar Bursitis with Abscess due to Corynebacterium ulcerans.

Corynebacteria are ubiquitous and reside as skin and mucosa commensals in animals. They are considered contaminants in clinical specimens, but significant clinical data points to their virulence and pathogenic potential over the last two decades. Corynebacteria can cause both community-acquired and nosocomial infections. Corynebacterium diphtheriae (C. diphtheriae) responsible for diphtheria has declined over the previous two decades with an increase in a similar clinical syndrome by Corynebacterium ulcerans (C. ulcerans) in Europe. As per recent studies, C. ulcerans shares similar virulence f
