The heat is on... thermal epiglottitis as a late presentation of airway steam injury.

BACKGROUND: Thermal epiglottitis is a rare but potentially life-threatening disease. Diagnosis requires a thorough history and high clinical level of suspicion, particularly in children. Thermal epiglottitis from steam inhalation can have a slow onset without oropharyngeal signs of thermal injury, findings that can hide the clinical diagnosis. OBJECTIVE: Our aim was to review the pathophysiology and clinical presentation of thermal epiglottitis and the challenges involved in diagnosis and management of this form of atypical epiglottitis. CASE REPORT: We describe the case of a 22-month-old ma

Early and late fatal complications of inhalation injury.

Severe inhalation injury causes a substantial deterioration in the prognosis and increases the general mortality of patients with extensive burns. Recently, in particular due to the development of invasive monitoring of patients and effective treatment of acute burn shock, we encounter with increasing frequency patients who survive the acute stage, including complications such as ARDS, and reach the stage of late complications. The latter include tracheooesophageal fistulas that develop on the basis of pressure ulcers and chondromalacia, usually at the site of the balloon of the tracheostomic

Tissue engineering as a tool in a novel approach to the comprehensive treatment and management of a deeply and extensively burned patient: case report.

Deep and extensive thermal burns with concurrent inhalation injuries can be associated with a high mortality rate, especially among elderly patients. Injuries of this type require treatment in highly. specialized centers. Early excision and autografting are the standard of care for extensive, deep burns. To achieve a functionally and aesthetically satisfactory burn scar, allogeneic acellular dermal matrices (ADMs) can be used as co-grafts alongside autologous split-thickness skin grafts (STSGs). Additionally, the application of in vitro-cultured autologous keratinocytes and fibroblasts has bee

Risk Factors for ARDS in Hospitalized Burn Patients: An Analysis of the National Trauma Data Bank.

Acute respiratory distress syndrome (ARDS) is a common complication among mechanically ventilated adult patients with burns; however, the risk factors associated with its development are not well understood. Conflicting evidence also exists regarding ARDS as an independent predictor of poor hospital outcomes. The purpose of this study is to identify risk factors for ARDS in mechanically ventilated adult patients with burns and characterize the impact of ARDS on hospital course. The National Trauma Data Bank was queried for patients ≥ 18 years with International Classification of Diseases (ICD)

Toxic inhalational exposures.

Respirable toxicants are a spectrum of irritant and nonirritant gases, vapors, fumes, and airborne particles that can be entrained into the body through the respiratory tract, resulting in exposures that cause pulmonary injury and/or systemic disease. Sources of respirable toxicants include structural fires, industrial accidents, domestic mishaps, and intentional releases of injurious agents on the battleground (warfare) or in civilian settings (acts of terrorism). Acute toxic inhalational exposures may result in respiratory failure, multisystem organ dysfunction, and death. Management of vict

Suicidal asphyxiation by carbon monoxide within a polythene bag.

Suicide by inhalation of carbon monoxide is not uncommon and usually involves car exhausts or burning charcoal or defective boilers. We report a case of a 25-year-old man, who committed suicide by inhaling carbon monoxide gas inside a polythene bag in a bathroom. The open carbon monoxide cylinder found inside the polythene bag was purchased online by the deceased a few days earlier. He had stated that the gas would be used for his experiment on the environment. A suicide note recovered from his trouser pocket revealed his intention for a painless death.

[Diagnosis and treatment of thermochemical injuries of the airways in patients with severe burns].

Chromobronchoscopy allows objective assessment of the size and depth of injury to tracheobronchial mucosa. The type of inhalation injury to the tracheobronchial tree is an important parameter determining the preventive use of high-frequency assisted and artificial ventilation of the lungs (HFAVL and AVL, respectively). In patients with slight injuries to the airways, inflicted by burning products, the criteria of transfer to HFAVL or AVL should be traditional. In medium severe involvement HFAVL should be started immediately through a microtracheostoma. Severe and extremely severe inhalation in

Nonoliguric vasopressin oversecretion in severely burned patients.

Eight burn patients with smoke inhalation were studied. High levels of plasma vasopressin and plasma cortisol in the presence of polyuria were observed. Stress and pain seem to be the main factors in the vasopressin oversecretion; the polyuria probably resulted from fluid administration and osmotic diuresis.

A Case Report of Deterioration in a Non-frail Octogenarian Burn Patient.

While advancements in critical care and burn treatment have improved over the decades, elderly burn victims continue to face high mortality rates. Measurements of frailty among patients have become popular tools for predicting burn outcomes over chronological age. In this report, we provide a case of a non-frail octogenarian burn victim who deteriorated rapidly during treatment, suggesting that frailty alone is not sufficient in predicting outcomes in older burn patients. An active 86-year-old male with hypertension presented to the emergency department with 35% total body surface area (TBSA)

Pathophysiologic Mechanisms and Current Treatments for Cutaneous Sequelae of Burn Wounds.

Burn injuries are a pervasive clinical problem. Extensive thermal trauma can be life-threatening or result in long-lasting complications, generating a significant impact on quality of life for patients as well as a cost burden to the healthcare system. The importance of addressing global or systemic issues such as resuscitation and management of inhalation injuries is not disputed but is beyond the scope of this review, which focuses on cutaneous pathophysiologic mechanisms for current treatments, both in the acute and long-term settings. Pathophysiological mechanisms of burn progression and w

Incidence and Risk Factors Associated with Microbial Colonization of Burn Wounds: An Observational Study.

Background: Microbial colonization of burn wounds leads to infection, which is a major cause of morbidity/mortality, prolonged admission, and cost. This study aims to investigate the incidence of positive burn wound colonization and its associated risk factors in a provincial referral center within a single-payer system. Methods: We performed a retrospective review of all adult (≥18 years) patients admitted to a single, tertiary burn center, with a primary burn diagnosis between January 2011 and 2021. Microbiology records were screened to identify patients with culture-positive burn wounds. Un

Cats with thermal burn injuries from California wildfires show echocardiographic evidence of myocardial thickening and intracardiac thrombi.

Recent increases in the prevalence and severity of wildfires in some regions have resulted in an increased frequency of veterinary burn patients. Few studies exist regarding diagnostics and management of burn wounds in veterinary patients and current knowledge is extrapolated from human literature and research models. Post-burn cardiac injury is a common finding and predictor of mortality in human patients and echocardiography is an important tool in monitoring response to therapy and predicting outcome. We describe the notable findings from cats naturally exposed to California wildfires in 20
