Novel experimental and software methods for image reconstruction and localization in capsule endoscopy.

BACKGROUND AND STUDY AIMS: Capsule endoscopy (CE) is invaluable for minimally invasive endoscopy of the gastrointestinal tract; however, several technological limitations remain including lack of reliable lesion localization. We present an approach to 3D reconstruction and localization using visual information from 2D CE images. PATIENTS AND METHODS: Colored thumbtacks were secured in rows to the internal wall of a LifeLike bowel model. A PillCam SB3 was calibrated and navigated linearly through the lumen by a high-precision robotic arm. The motion estimation algorithm used data (light fallin…

Role of wireless capsule endoscopy in the follow-up of inflammatory bowel disease.

The introduction of wireless capsule endoscopy in 2000 has revolutionized our ability to visualize parts of the small bowel mucosa classically unreached by the conventional endoscope, and since the recent introduction of colon capsule endoscopy, a promising alternative method has been available for the evaluation of large bowel mucosa. The advantages of wireless capsule endoscopy include its non-invasive character and its ability to visualize proximal and distal parts of the intestine, while important disadvantages include the procedure's inability of tissue sampling and significant incompleti…

Meta-analysis of capsule endoscopy in patients diagnosed or suspected with esophageal varices.

The PillCam ESO (Given Imaging, Israel) or esophageal capsule endoscopy (ECE) is a novel technique used in the diagnostic evaluation of esophagus. Many studies have been performed to compare the accuracy of ECE against the current gold standard esophago-gastro-duodenoscopy and a meta-analysis recently published by Lu et al suggests that ECE may have an acceptable sensitivity and specificity in detecting esophageal varices. We would like to discuss the importance and implication of publication bias in this meta-analysis.

Meta-analysis of capsule endoscopy in patients diagnosed or suspected with esophageal varices.

AIM: To review the literature on capsule endoscopy (CE) for detecting esophageal varices using conventional esophagogastroduodenoscopy (EGD) as the standard. METHODS: A strict literature search of studies comparing the yield of CE and EGD in patients diagnosed or suspected as having esophageal varices was conducted by both computer search and manual search. Data were extracted to estimate the pooled diagnostic sensitivity and specificity. RESULTS: There were seven studies appropriate for meta-analysis in our study, involving 446 patients. The pooled sensitivity and specificity of CE for dete…

Use of wireless capsule endoscopy in the management of severe Henoch-Schonlein purpura.

Henoch-Schonlein purpura is a multisystem vasculitis that primarily affects children. Characteristic symptoms include purpura of the lower extremities and buttocks, abdominal pain, arthralgias, and hematuria. Gastrointestinal bleeding occurs in approximately 50% of children and, although often self-limiting, can be significant. Wireless capsule endoscopy has been found to be safe and effective for children over 10 years of age. It is now the preferred imaging modality for evaluating gastrointestinal bleeding from the small intestine. Here we report an unusual case of chronic Henoch-Schonlein p…

Outcome of capsule endoscopy in determining indication and route for push-and-pull enteroscopy.

BACKGROUND AND STUDY AIM: Video capsule endoscopy and push-and-pull enteroscopy (PPE), both allow a complete examination of the small bowel in patients with suspected intestinal disorders. Due to the invasiveness of PPE, indications should probably be selective. The aim of the present prospective study was to evaluate the outcome of an approach whereby capsule endoscopy was used to select patients in whom PPE was indicated. PATIENTS AND METHODS: 164 patients were included (90 men; age 54+/-18 years) with various indications for small-bowel investigation, such as obscure bleeding (n=88), suspe…

Wireless capsule endoscopy in a case of small bowell obstruction: not always contraindicated.

Wireless capsule endoscopy is a new diagnostic technique used especially for investigating the entire small bowel. Recent studies have demonstrated its superior ability to detect and evaluate small bowel lesions compared to other radiological examinations. Performing video capsule endoscopy is regarded as contraindicated when an intestinal occlusion is documented. We present a case of hidden intestinal bleeding which underwent wireless capsule endoscopy and was treated successfully despite documented ileal stenosis.

Endoscopic diagnosis of follicular lymphoma with small-bowel involvement using video capsule endoscopy and double-balloon endoscopy: a case series.

The aims of this study were to compare the detection rates of gastrointestinal follicular lymphoma lesions by video capsule endoscopy (VCE) and double-balloon endoscopy (DBE), and to determine the pathologic diagnostic yields of DBE-directed biopsies. A total of 27 consecutive patients were enrolled. No significant difference in detection rates was observed in 12 patients who underwent total enteroscopy at both VCE and DBE. Pathologic diagnostic yields stratified by location were 91 % in the proximal duodenum at esophagogastroduodenoscopy, 88 % in the jejunum at antegrade DBE, 52 % in the ileu…

Minimally invasive surgery for obscure idiopathic ileal varices diagnosed by capsule endoscopy and double balloon endoscopy: report of a case.

Small intestinal bleeding is difficult to detect and can be life-threatening. Capsule endoscopy (CE) is a new, minimally invasive diagnostic procedure designed to detect gastrointestinal (GI) bleeding. We report the successful management of idiopathic ileal varices by capsule endoscopy and laparoscopic surgery. Massive bleeding occurred suddenly with intermittent melena, and the patient was finally admitted to a local hospital in hypovolemic shock. Her condition was stabilized with conservative therapy but the site of bleeding was not defined by endoscopy, computed tomography, scintigraphy, or…

Are repeat upper gastrointestinal endoscopy and colonoscopy necessary within six months of capsule endoscopy in patients with obscure gastrointestinal bleeding?

BACKGROUND AND AIM: Medicare reimbursement for capsule endoscopy for the investigation of obscure gastrointestinal bleeding in Australia requires endoscopy and colonoscopy to have been performed within 6 months. This study aims to determine the diagnostic yield of repeating these procedures when they had been non-diagnostic more than 6 months earlier. METHODS: Of 198 consecutive patients who were referred for the investigation of obscure gastrointestinal bleeding, 50 underwent repeat endoscopy and colonoscopy solely to enable reimbursement (35 females and 15 males; mean age 59.4 [range: 21-82…

Endoscopic Diagnosis of Necator americanus Infection Presenting With Persistent Iron-Deficiency Anemia: Usefulness of Image-Enhanced Endoscopy and Capsule Endoscopy.

Necator americanus infection is now rare in developed countries but remains an important cause of iron-deficiency anemia and abdominal symptoms in individuals with relevant epidemiological backgrounds. A 59-year-old Filipino man with long-standing unexplained iron-deficiency anemia was admitted to our hospital for epigastric pain associated with choledocholithiasis. Although biliary enzymes improved after endoscopic treatment, his abdominal pain persisted. Subsequent gastrointestinal investigations, including upper gastrointestinal endoscopy, colonoscopy, and small-bowel capsule endoscopy, rev…

Two-stage cylindrical magnetic localization with neural network calibration for wearable capsule endoscopy.

Accurate localization of wireless capsule endoscopy is essential for reliable gastrointestinal diagnosis, yet magnetic tracking systems are often degraded by sensor distortions, misalignment, and patient motion in wearable settings. This study presents a magnetic localization framework that combines cylindrical magnetic field modelling with neural network-based sensor calibration to improve robustness under wearable operating conditions. By exploiting the structural properties of cylindrical magnetic field representations, the proposed approach decouples axial and transverse components of the …
