Optimizing non-opioid pain control after implant-based breast reconstruction: a review of the literature and proposed pain control algorithm.

Despite the intense focus on the opioid epidemic and its known association with surgical procedures, there is a paucity of evidence-based literature on pain management in implant-based breast reconstruction (IBR). Herein, we present an updated review of the literature aimed at identifying pain treatment protocols to minimize narcotic use and its associated potential addiction in IBR. A comprehensive review of the published English literature was conducted using Ovid Medline/PubMed Database without timeframe limitations. The inclusion criteria of selected articles presented in this review inclu

Mobilization of Mechanically Ventilated Patients in Switzerland.

PURPOSE: Growing evidence suggests that early mobilization benefits intensive care unit (ICU) patients. However, national practices and the culture of individual ICUs influence mobilization activities. MATERIALS AND METHODS: In a 1-day, Swiss point prevalence study conducted in 35 ICUs (representing 45% of all ICUs), the highest level of mobilization for mechanically ventilated patients was characterized using the validated ICU Mobility Scale, along with data collection for potential safety events and mobilization barriers. RESULTS: Among 161 mechanically ventilated patients, a total of 33%

Comparison of adverse events and postoperative mobilization following knee extensor mechanism rupture repair: A systematic review and network meta-analysis.

BACKGROUND: Extensor mechanism rupture (EMR) of the knee is a rare but potentially debilitating injury that often occurs due to trauma. While a wide variety of surgical treatments have been reported, there is currently no consensus on the most successful treatment method. The timing of post-operative joint mobilization is also critical for successful recovery after EMR repair. Despite the traditional method of complete immobilization for 6 weeks, there is an increasing trend towards early post-operative knee mobilization. The purpose of this network meta-analysis was to compare adverse event r

Creating a culture of mobility: an interdisciplinary approach for hospitalized patients.

The risks of bed rest and decreased activity during hospitalization are substantial. Immobility is a significant cause for delay of discharge, suboptimal care and can contribute to poor patient outcomes. An interdisciplinary team approach was implemented to change an organization culture on early patient mobility. The program goals were to focus physical and occupational therapy services for patients who would benefit from their skilled expertise, and to develop nursing protocols, physician order sets, expectations, and education to promote movement of all patients according to their needs and

Management of prolonged post-operative ileus: evidence-based recommendations.

BACKGROUND: Prolonged post-operative ileus (PPOI) occurs in up to 25% of patients following major elective abdominal surgery. It is associated with a higher risk of developing post-operative complications, prolongs hospital stay and confers a significant financial load on health-care institutions. Literature outlining best-practice management strategies for PPOI is nebulous. The aim of this text was to review the literature and provide concise evidence-based recommendations for its management. METHODS: A literature search through the Ovid MEDLINE, EMBASE, Google Scholar and Cochrane databases

[The Dutch Institute for Health Care Improvement (CBO) guideline for the diagnosis and treatment of aspecific acute and chronic low back complaints].

Under the auspices of the Dutch Institute for Healthcare Improvement (CBO), a guideline has been developed for the diagnosis and treatment of aspecific low-back pain, based on the recent scientific literature. So-called 'red flags' are used to identify physical disorders. To obtain insight into psychosocial factors, 'yellow flags' are used. Acute low-back pain (0-12 weeks) is treated in a time-contingent manner. Staying active is better than bed rest. If chronicity threatens, exercise therapy can be advised. As part of an activating management, manipulation can be used. For pain relief, parace

[Sprain and dislocation of the hip joint].

Traumatic dislocations of the hip, while very infrequent injuries, must be treated with the utmost urgency. The later the adjustment is made, the sooner aseptic necrosis of the femoral head and periarticular ossification have to be reckoned with; these may lead to dysfunction. The results of 40 cases are reported, one-half of which was treated with two-week extension and a long period of non-weight bearing, the other half without extension, early mobilisation and partial weight bearing up the 11th week. The results in each group are equally good. Thus, we endorse Lorenz Böhler's proposal to fo

Intramedullary stabilization of neoplastic destructive disease involving the subtrochanteric region of the femur.

Neoplastic destructive disease involving the subtrochanteric region of the femur is a difficult condition to treat. This is a retrospective study of 11 femurs in 10 patients with subtrochanteric destructive lesions or pathologic fractures that were stabilized with the Zickel intramedullary device. The study investigated underlying disease process, ambulatory status, operative parameters, time to death, and associated metastasis. The average survival time of the patients who died was 4.7 months. All of the patients involved in this study could be mobilized, but only 3 out of 10 were ambulatory.

Is obligatory bed rest after lumbar puncture obsolete?

After lumbar puncture (LP) an epidural CSF leakage caused by delayed closure of a dural defect leads to a decrease in CSF pressure. The resultant venous dilatation as well as downward shift of the brain with traction on pain-sensitive blood vessels and nerves frequently evokes post-lumbar puncture headache (PLPH), when the patient assumes the upright position. In previous studies differing opinions have been expressed about the prophylactic value of the posture taken by the patient after LP. The present study was designed to evaluate the benefit of the decrease of hydrostatic CSF pressure on t

[Rehabilitation measures following myocardial infarct].

The term "rehabilitation" in its original meaning is compared to the new definition of "cardiac rehabilitation" by the World Health Organization. The formal early inpatient rehabilitation program ("Anschlussheilverfahren") is described and its indication and contraindication in the convalescent period three to eight weeks after the acute myocardial infarction are reviewed. Current concepts in the medical, occupational and psychological management are discussed. Data are presented that show the increase in working capacity at the end of an inpatient cardiac rehabilitation program, the rate of r

Bilateral femoral shortening for overgrowth in young adults.

Through 17 years ten patients were treated for psychologically unacceptable tallness by bilateral femoral shortening. All were young adults between 17 and 28 years; one was acromegalic, one had Marfans syndrome and the other were tall for unknown reasons. The amount of shortening obtained was between 6 and 16 cm. One patient was reoperated bilaterally because of insufficient stability of the pin and plate fixation. All patients healed without significant complications. For exclusive cases, which have been carefully observed over several months, bilateral shortening seems to be a safe procedure

Rapid Ultrasonography for Shock and Hypotension Protocol Performed using Handheld Ultrasound Devices by Paramedics in a Moving Ambulance: Evaluation of Image Accuracy and Time in Motion.

Источник пометил публикацию как отозванную

INTRODUCTION: Handheld ultrasound (US) devices have become increasingly popular since the early 2000s due to their portability and affordability compared to conventional devices. The Rapid Ultrasonography for Shock and Hypotension (RUSH) protocol, introduced in 2009, has shown promising accuracy rates when performed with handheld devices. However, there are limited data on the accuracy of such examinations performed in a moving ambulance. This study aimed to assess the feasibility and accuracy of the RUSH protocol performed by paramedics using handheld US devices in a moving ambulance. OBJECT

Walking ability in mature patients with spina bifida.

The ambulatory status of 50 patients, aged 15 years and older, with spina bifida is assessed and related to the level of the lesion, the stability and presence of deformity at the hip joints, and the presence of pelvic obliquity and of scoliosis. A retrospective study was made of these variables for the year the child began walking and, when applicable, the year the child stopped walking. More than two-thirds of the group were community ambulators. Lesion review, and there were no household or nonfunctional ambulators. Lesion levels of the ambulators ranged from T11 to S2-S3, whereas lesion le

BMI-z scores of boys with Duchenne muscular dystrophy already begin to increase before losing ambulation: a longitudinal exploration of BMI, corticosteroids and caloric intake.

We aimed to investigate BMI-z course in patients with Duchenne muscular dystrophy (DMD) during transition to loss of ambulation, and to explore the contribution of caloric intake and corticosteroid use. A retrospective multicenter longitudinal study was conducted. First, analyses of characteristics at first visit were carried out. Second, discontinuous change models were fitted to explore associations between BMI-z, loss of ambulation, caloric intake and corticosteroid use. 790 visits of 159 patients were collected. Cross sectional first visit analyses showed the presence of overweight and obe

Impact of early goal-directed mobilization on ICU-acquired weakness: a meta-analysis.

BACKGROUND: ICU-acquired weakness (ICU-AW) significantly impairs muscle strength and physical function in critically ill patients. Early mobilization interventions have been increasingly adopted, yet the specific effects of a structured EGDM program on ICU-AW and functional outcomes have not been systematically quantified. This meta-analysis aimed to evaluate the effects of EGDM on ICU-AW, muscle strength, and functional outcomes. Achieving a balance between mobilization and immobilization after surgical or conservative treatment of the hand.

Maintaining an appropriate balance between mobilization and immobilization is a central challenge in hand rehabilitation and has a decisive impact on tissue healing, functional recovery and patient outcomes. This narrative review examines the concept of balance in the hand by outlining risks that threaten this balance, reviewing the available evidence comparing mobilization and immobilization strategies, and highlighting the strength of the evidence and existing research gaps. The practical achievement of balance is explored with particular emphasis on the timing of mobilization, effective com
