Who and how to screen for Cushing's syndrome: the position statement of the Italian Society of Endocrinology.

INTRODUCTION: This Position Statement presents the recommendations of the Italian Society of Endocrinology (SIE) to identify patients with Cushing's syndrome in specific clinical settings. We set two overarching research questions: 1. Which subjects should be screened ("who" should be screened)? 2. Which is the most appropriate first-line test ("how" to screen) for screening? All suggestions/recommendations are evidence-based and directed at endocrinologists and other physicians who deal with patients who have presumptive Cushing's syndrome. METHODS: The recommendations, developed by a SIE co…

Extended growth charts for Indian children.

OBJECTIVES: While growth charts depicting 7 percentile lines for height and weight are useful for healthcare workers and pediatricians, endocrinologists need indication-specific z score cutoffs to plan investigations and treatment. The current Indian charts do not offer lower percentile/z scores (-2.25, -2.5, and -3 z score) lines. Also, increasing prevalence of childhood overweight and obesity necessitates a quick screening of nutritional status without calculations while using the same growth chart. Our objectives were to produce extended and user-friendly growth charts for 0-18-year-old Ind…

Long-term outcomes of treatment of hyperthyroidism in Ireland.

We investigated the long-term outcome of treatment in 159 patients with hyperthyroidism first seen between 1979 and 1992. Median duration of follow-up was 10 1/2 years. We also inquired into current practice for the follow-up of hyperthyroidism by other endocrinologists in Ireland. Seven cases of unrecognised hyperthyroidism (4 per cent) and one of unrecognised hypothyroidism were identified. Among patients with Graves' disease, of those treated with an antithyroid drug, 28 per cent were in remission, 68 per cent had relapsed and 4 per cent had become hypothyroid. Of those treated by sub-total…

CURRENT INSIGHTS IN THE SCREENING OF SECONDARY HYPERTENSION: A CARDIOLOGIST'S PERSPECTIVE.

Renovascular disease and primary hyperaldosteronism are among the leading causes of high blood pressure in young adults. Collaboration between cardiologists and endocrinologists is crucial in addressing the underlying causes of resistant hypertension. In this article, we provide updated information on the clinical context in which renal artery stenosis is suspected, and the appropriate investigation methods for confirmation. The decision to revascularize is based on the etiology of the stenosis, patient age, and comorbidities. In brief, we aimed to provide a cardiologist's perspective on a sig…

Approach to Bone Health in the Patient With Breast Cancer.

Treatment for breast cancer, including endocrine therapies, can contribute to bone loss and increase the risk of osteoporosis and fractures. Management of bone health in patients with cancer is often coordinated between oncologists, endocrinologists, and primary care physicians. In this article, we discuss the approach to screening for fracture risk among patients initiating treatments for breast cancer and recommendations for lifestyle modifications to optimize bone health. We will review 3 indications for pharmacologic bone-targeted therapies: prevention of cancer treatment-induced bone loss…

The Role of the Urologist in a Reproductive Endocrinology and Infertility Practice.

With male factor infertility accounting for up to 50% of infertility cases, demand for male fertility services has increased. Integrating a reproductive urologist within a fertility center allows for treatment of both partners simultaneously with easier, more convenient access to a comprehensive male evaluation and any indicated interventions. A joint practice allows urologists to collaborate more closely with reproductive endocrinologists, which can, in turn, improve clinical care and research endeavors. This full-service, streamlined approach translates to optimized care for the infertile co…

Multidisciplinary approach for hidradenitis suppurativa patients.

Hidradenitis suppurativa or acne inversa is an inflammatory, chronic disease of the hair follicles located in specific areas of the skin surface (axillae, submammary, inguinal and intergluteal folds, pubis, genitalia and buttocks). The diagnosis and the management of this disease are usually made by the dermatologist. However, the frequent association of hidradenitis suppurativa or acne inversa with important systemic comorbidities needs the cooperation with a number of specialists. In particular, the cooperation with rheumatologists, gastroenterologists, endocrinologists, microbiologists/infe…

Ovarian dysfunction by activating mutation of GS alpha: McCune-Albright syndrome as a model.

The McCune-Albright syndrome is characterized by cafe-au-lait spots, precocious puberty, and fibrous dysplasia. It is due to mutations in the gene encoding the GS protein alpha subunit coupling 7-transmembrane-domain receptors to adenylate cyclase, leading to constitutive adenylate cyclase activation and cAMP overproduction. Endocrinologists and gynecologists are confronted with new issues when these children reach adulthood. Gonadal function and fertility are often abnormal in women in whom puberty was precocious, owing to the persistence of a variable degree of ovarian autonomy that hinders …

[Reoperations for persistent or recurrent hyperparathyroidism].

Reoperations for persistent or recurrent primary or secondary hyperparathyroidism are demanding tasks mainly when localization of parathyroid tumor(s) was not successful. These procedures may harbor a risk for the patients and therefore need an expert surgeon who is familiar with even the most difficult situation. Important steps before surgery are confirmation of the diagnosis of persistent or recurrent disease and localization of the source of hormone access. Reoperations should be exclusively performed in centers where expert surgeons, radiologists, endocrinologists and nephrologists are av…
